Does endovascular treatment with curative intention have benefits for treating High-grade AVM versus radiosurgery? Efficacy, safety, and cost-effectiveness analysis

BACKGROUND: The treatment of high-grade arteriovenous malformations (AVMs) remains challenging. Microsurgery provides a rapid and complete occlusion compared to other options, but is associated with undesirable morbidity and mortality. The aim of this study was to compare the occlusion rates, incidence of unfavorable outcomes, and cost-effectiveness of embolization and stereotactic radiosurgery (SRS) as a curative treatment for high-grade AVMs.METHODS: A retrospective series of 57 consecutive patients with high-grade AVM treated with embolization or SRS, with the aim of achieving complete occlusion, was analyzed. Demographic, clinical, and angio-architectonic variables were collected. Both treatments were compared for the occlusion rate and procedure-related complications. In addition, a cost-effectiveness analysis was performed.RESULTS: Thirty (52.6%) patients were men and 27 (47.4%) were women (mean age 39 years). AVMs were unruptured in 43 (75.4%) patients, and ruptured in 14 (24.6%) patients. The presence of deep venous drainage, nidus volume, perforated arterial supply, and eloquent localization were more frequent in the SRS group. Complications such as hemorrhage or worsening of previous seizures were more frequent in the embolization group. No significant differences were observed in the occlusion rates or in the time necessary to achieve occlusion between the groups. The incremental cost-effectiveness ratio for endovascular treatment vs. SRS was $ 53.279.CONCLUSION: Both techniques achieved similar occlusion rates, but SRS carried a lower risk of complications. Staged embolization may be associated with a greater risk of hemorrhage, while SRS was demonstrated to have a better cost-effectiveness ratio. These results support SRS as a better treatment option for high-grade AVMs

Herramientas de Software de Simulación para Redes de Comunicaciones

Actualmente los sistemas de redes de comunicación son parte esencial del funcionamiento de las empresas, y una de las tareas más importantes de los diseñadores de redes. El estudio del desempeño de las redes de comunicaciones a través de la simulación es una forma de abordar estas redes de comunicaciones. Las herramientas de simulación Open Source con licencia GNU, son apropiadas para los estudiantes universitarios. En la Sede Regional Orán de la Universidad Nacional de Salta se han ido incorporando estas herramientas, no solo en el ámbito de la red en producción, sino también en el ámbito académico. Estudio de la influencia de métodos de simulación en la enseñanza del tema direccionamiento IP

Este artículo resume un estudio cuyo propósito fue determinar la influencia del uso de software de simulación en la enseñanza de conceptos y fundamentos en la asignatura Redes de Computadoras I en la Universidad Nacional de Salta. Si bien el estudio se circunscribe a una universidad, la situación descripta y analizada es muy similar en muchas otras. El estudio fue del tipo cuantitativo, con diseño experimental con grupo de control. Las pruebas paramétricas permitieron concluir que, existe diferencia estadísticamente significativa a favor de los estudiantes que emplearon una estrategia basada en simulación.This article summarizes a study whose purpose was to determine simulation software influence in teaching concepts and fundamentals in the subject Redes de Computadoras I at the National University of Salta. Even though the study was based on one University, the situation described and analized is similar in many others. The quantitative study was an experimental design with control group. A very simple analog control for QSW-ZVS source/sink buck converter with seamless mode transition

2018 IEEE 19th Workshop on Control and Modeling for Power Electronics (COMPEL), 25-28 junio, Padua (Italia)A simple, analog, control circuit is proposed for seamless transition between source and sink modes in a Quasi- Square-Wave Zero Voltage Switching (QSW-ZVS) source/sink buck converter. The inductor current is controlled by a variablewidth hysteretic current mode control. The upper and lower bounds of the hysteretic band are clamped to ensure QSWZVS operation with a single current command from the control loop and independently from the power flow direction. This enables the control of any PWM converter able to operate in QSW-ZVS with a single control loop. If the proposed circuit is used, using a complex multi-mode or look-up-table based digital control is no longer required, simplifying the implementation and lowering the cost of the converter. A 50 W buck converter is built to demonstrate the proposed control circuit and experimental measurements are shown to verify its correct operatio

Optimization procedure of source/sink converters for DC power distribution nano-grids

These converters should provide a high power quality and be efficient, compact and inexpensive. Furthermore, the characteristics of the loads and sources connected to these converters are not known beforehand and may change widely (there is no regulation about the DC loads, such as IEC61000-3-2 in AC grids). As a consequence, the design procedure is neither standard nor obvious. This work proposes an optimized design procedure for any Bus Provider in a DC-Power Distribution System. It is based on models, hence extensible to different Bus-Provider topologies, and takes into account the main issues in DC-Power Distribution Systems, especially the design conditions imposed by the wide variety of loads, or even sources, which may be connected to the output bus. Experimental results obtained from three designs show a close match with the analytical models and verify that the proposed procedure minimizes converter losses and complies with the requirement
