Proceedings from the 1984 Tax Institute Symposium: Cash or Deferred Arrangements Under I.R.C. Section 401(k)
This article examines the reasons why an employer might consider a cash or deferred profit sharing or stock bonus plan. It looks at the rules for a cash or deferred arrangement (CODA) under Section 401(k) of the Internal Revenue Code. It covers the four primary requirements under the I.R.C., and then fifth, and biggest requirement, nondiscrimination
Scalable background-limited polarization-sensitive detectors for mm-wave applications
We report on the status and development of polarization-sensitive detectors
for millimeter-wave applications. The detectors are fabricated on
single-crystal silicon, which functions as a low-loss dielectric substrate for
the microwave circuitry as well as the supporting membrane for the
Transition-Edge Sensor (TES) bolometers. The orthomode transducer (OMT) is
realized as a symmetric structure and on-chip filters are employed to define
the detection bandwidth. A hybridized integrated enclosure reduces the
high-frequency THz mode set that can couple to the TES bolometers. An
implementation of the detector architecture at Q-band achieves 90% efficiency
in each polarization. The design is scalable in both frequency coverage, 30-300
GHz, and in number of detectors with uniform characteristics. Hence, the
detectors are desirable for ground-based or space-borne instruments that
require large arrays of efficient background-limited cryogenic detectors.Comment: 7 pages, 3 figures, Presented at SPIE Astronomical Telescopes and

Silicon-Based Antenna-Coupled Polarization-Sensitive Millimeter-Wave Bolometer Arrays for Cosmic Microwave Background Instruments
We describe feedhorn-coupled polarization-sensitive detector arrays that
utilize monocrystalline silicon as the dielectric substrate material.
Monocrystalline silicon has a low-loss tangent and repeatable dielectric
constant, characteristics that are critical for realizing efficient and uniform
superconducting microwave circuits. An additional advantage of this material is
its low specific heat. In a detector pixel, two Transition-Edge Sensor (TES)
bolometers are antenna-coupled to in-band radiation via a symmetric planar
orthomode transducer (OMT). Each orthogonal linear polarization is coupled to a
separate superconducting microstrip transmission line circuit. On-chip
filtering is employed to both reject out-of-band radiation from the upper band
edge to the gap frequency of the niobium superconductor, and to flexibly define
the bandwidth for each TES to meet the requirements of the application. The
microwave circuit is compatible with multi-chroic operation. Metalized silicon
platelets are used to define the backshort for the waveguide probes. This
micro-machined structure is also used to mitigate the coupling of out-of-band
radiation to the microwave circuit. At 40 GHz, the detectors have a measured
efficiency of 90%. In this paper, we describe the development of the 90 GHz
detector arrays that will be demonstrated using the Cosmology Large Angular
Scale Surveyor (CLASS) ground-based telescope

Cardiac Biomarkers and Risk of Atrial Fibrillation in Chronic Kidney Disease: The CRIC Study.
Background We tested associations of cardiac biomarkers of myocardial stretch, injury, inflammation, and fibrosis with the risk of incident atrial fibrillation (AF) in a prospective study of chronic kidney disease patients. Methods and Results The study sample was 3053 participants with chronic kidney disease in the multicenter CRIC (Chronic Renal Insufficiency Cohort) study who were not identified as having AF at baseline. Cardiac biomarkers, measured at baseline, were NT-proBNP (N-terminal pro-B-type natriuretic peptide), high-sensitivity troponin T, galectin-3, growth differentiation factor-15, and soluble ST-2. Incident AF ("AF event") was defined as a hospitalization for AF. During a median follow-up of 8 years, 279 (9%) participants developed a new AF event. In adjusted models, higher baseline log-transformed NT-proBNP (N-terminal pro-B-type natriuretic peptide) was associated with incident AF (adjusted hazard ratio [HR] per SD higher concentration: 2.11; 95% CI, 1.75, 2.55), as was log-high-sensitivity troponin T (HR 1.42; 95% CI, 1.20, 1.68). These associations showed a dose-response relationship in categorical analyses. Although log-soluble ST-2 was associated with AF risk in continuous models (HR per SD higher concentration 1.35; 95% CI, 1.16, 1.58), this association was not consistent in categorical analyses. Log-galectin-3 (HR 1.05; 95% CI, 0.91, 1.22) and log-growth differentiation factor-15 (HR 1.16; 95% CI, 0.96, 1.40) were not significantly associated with incident AF. Conclusions We found strong associations between higher NT-proBNP (N-terminal pro-B-type natriuretic peptide) and high-sensitivity troponin T concentrations, and the risk of incident AF in a large cohort of participants with chronic kidney disease. Increased atrial myocardial stretch and myocardial cell injury may be implicated in the high burden of AF in patients with chronic kidney disease

Research-based versus clinical serum creatinine measurements and the association of acute kidney injury with subsequent kidney function: findings from the Chronic Renal Insufficiency Cohort study.
Background:Observational studies relying on clinically obtained data have shown that acute kidney injury (AKI) is linked to accelerated chronic kidney disease (CKD) progression. However, prior reports lacked uniform collection of important confounders such as proteinuria and pre-AKI kidney function trajectory, and may be susceptible to ascertainment bias, as patients may be more likely to undergo kidney function testing after AKI. Methods:We studied 444 adults with CKD who participated in the prospective Chronic Renal Insufficiency Cohort (CRIC) Study and were concurrent members of a large integrated healthcare delivery system. We estimated glomerular filtration rate (eGFR) trajectories using serum creatinine measurements from (i) the CRIC research protocol (yearly) and (ii) routine clinical care. We used linear mixed effects models to evaluate the associations of AKI with acute absolute change in eGFR and post-AKI eGFR slope, and explored whether these varied by source of creatinine results. Models were adjusted for demographic characteristics, diabetes status and albuminuria. Results:During median follow-up of 8.5 years, mean rate of eGFR loss was -0.31 mL/min/1.73 m2/year overall, and 73 individuals experienced AKI (55% Stage 1). A significant interaction existed between AKI and source of serum creatinine for acute absolute change in eGFR level after discharge; in contrast, AKI was independently associated with a faster rate of eGFR decline (mean additional loss of -0.67 mL/min/1.73 m2/year), which was not impacted by source of serum creatinine. Conclusions:AKI is independently associated with subsequent steeper eGFR decline regardless of the serum creatinine source used, but the strength of association is smaller than observed in prior studies after taking into account key confounders such as pre-AKI eGFR slope and albuminuria
The Cosmology Large Angular Scale Surveyor
The Cosmology Large Angular Scale Surveyor (CLASS) is a four telescope array
designed to characterize relic primordial gravitational waves from inflation
and the optical depth to reionization through a measurement of the polarized
cosmic microwave background (CMB) on the largest angular scales. The
frequencies of the four CLASS telescopes, one at 38 GHz, two at 93 GHz, and one
dichroic system at 145/217 GHz, are chosen to avoid spectral regions of high
atmospheric emission and span the minimum of the polarized Galactic
foregrounds: synchrotron emission at lower frequencies and dust emission at
higher frequencies. Low-noise transition edge sensor detectors and a rapid
front-end polarization modulator provide a unique combination of high
sensitivity, stability, and control of systematics. The CLASS site, at 5200 m
in the Chilean Atacama desert, allows for daily mapping of up to 70\% of the
sky and enables the characterization of CMB polarization at the largest angular
scales. Using this combination of a broad frequency range, large sky coverage,
control over systematics, and high sensitivity, CLASS will observe the
reionization and recombination peaks of the CMB E- and B-mode power spectra.
CLASS will make a cosmic variance limited measurement of the optical depth to
reionization and will measure or place upper limits on the tensor-to-scalar
ratio, , down to a level of 0.01 (95\% C.L.)
Deciphering Amyotrophic Lateral Sclerosis: What Phenotype, Neuropathology and Genetics Are Telling Us about Pathogenesis
Amyotrophic lateral sclerosis (ALS) is characterized phenotypically by progressive weakness and neuropathologically by loss of motor neurons. Phenotypically, there is marked heterogeneity. Typical ALS has mixed upper motor neuron (UMN) and lower motor neuron
(LMN) involvement. Primary lateral sclerosis has predominant UMN involvement. Progressive muscular atrophy has predominant LMN involvement. Bulbar and limb ALS have predominant regional involvement. Frontotemporal dementia has significant cognitive and behavioral involvement. These phenotypes can be so distinctive that they would seem to have differing biology. But they cannot be distinguished, at least neuropathologically or genetically. In sporadic ALS (SALS), they all are characterized by ubiquitinated cytoplasmic inclusions of TDP-43. In familial ALS (FALS), where phenotypes are indistinguishable from SALS and similarly
heterogeneous, each mutated gene has its own genetic and molecular signature. Putting this together, since the same phenotypes can have multiple causes including different gene mutations, there must be multiple molecular mechanisms causing ALS and ALS is a syndrome. But since multiple phenotypes can be caused by one single gene mutation, a single molecular mechanism can cause heterogeneity. What the mechanisms are remain unknown, but active propagation of the pathology neuroanatomically seems to be a principle component. Leading candidate mechanisms include RNA processing, cell-cell interactions between neurons and non-neuronal neighbors, focal seeding from a misfolded protein that has prion-like propagation, and fatal errors introduced during neurodevelopment of the motor system. If fundamental mechanisms can be identified and understood, ALS therapy could rationally target progression and stop disease—a goal that seems increasingly achievable.Stem Cell and Regenerative Biolog
