    A retrospective study on endometrial patterns in abnormal uterine bleeding

    Background: To determine the type of endometrial patterns of the different age categories of women who presented as a case of abnormal uterine bleeding.Methods: This is a retrospective study conducted on 105 patients who presented with abnormal uterine bleeding who underwent fractional curettage in our hospital. The data on their age, presenting complaints, and comorbidities of all the women were collected. The patterns of endometrial changes were studied and classified.Results: The most common histopathological findings were anovulatory shedding (34.3%) and irregular shedding (18.1%). The other findings include irregular ripening, papillary endocervicitis, endocervicitis, pill endometrium, atrophic endometrium, squamous metaplasia, and endometrial hyperplasia. The most common malignant change seen was endometrioid carcinoma which was seen in women over 40 years of age.Conclusions: Histopathological examination of the endometrium shows a clear-cut differentiation between physiological and malignancy changes in the endometrium. Hence, endometrial sampling is considered the golden tool for accurate analysis of the endometrium

    Changing trends of causative factors in antenatal mothers with bad obstetric history: a retrospective study

    Background: The death of an infant, in utero or after birth has been a devastating experience for parents and clinicians alike. Various efforts are being made to make an accurate diagnosis of such pregnancy losses at the earliest to prevent the emotional and physical stress that the woman undergoes. This study aims to assess the changing trends in risk factors and outcome of pregnancies in cases of bad obstetric history (BOH) and to identify the newly emerging maternal and fetal factors contributing to incidence of pregnancy loss.Methods: This study is a retrospective analysis of antenatal women with BOH attending the obstetrics and gynecology OPD at Chettinad hospital. The study was conducted during a time period of 1 year (May 2019 to May 2020).Results: The medical complications and underlying causes for BOH were analysed among the study group. Out of 41 women, 6 were diagnosed to have gestational diabetes mellitus (GDM), 8 were found to be hypertensive and 1 APLA positive. Neonatal mortality was encountered in 2 cases, attributing to Inborn error of metabolism. However, the cause for BOH remained unexplained in 3 individuals.Conclusions: A full work-up can be initiated after two consecutive losses to identify and treat the various causes responsible for BOH. Lot of further studies and research are being conducted to unravel the mystery in these cases. Despite these efforts, the male factors contributing to recurrent pregnancy loss have remained largely unexplored

    Pregnancy and perinatal outcomes in women with polycystic ovarian syndrome

    Background: To study the pregnancy and perinatal outcomes in women with PCOS with normal women and to study the incidence of complications like menstrual irregularities, mode of conception, gestational diabetes mellitus, pre eclampsia, preterm labour and NICU admissions in women with PCOS.Methods: It was prospective comparatives study done in 100 women to compare outcome in pregnancy with PCOS and normal women.  A  detailed history, general and obstetric examination, antenatal investigations, routine dating scan, glucose challenge test at 24 to 28 weeks in study and control groups, blood pressure recording and urine for proteinuria after 20 weeks to evaluate preeclampsia. After delivery, birth weight and APGAR score at 1 and 5 minutes were recorded. Body mass index was calculated and pregnancy outcome studied and compared.Results: Out of 50 women with PCOS 13 developed GDM, 10 developed pre eclampsia, 6 had abortion, 4 went into preterm labour. 15 babies required NICU admission.Conclusions: Pregnancy complications like GDM, pre eclampsia, preterm labour, abortions and neonatal complications like NICU admissions are higher in women with PCOS when compared with normal women. Hence PCOS needs to be diagnosed early and treated accordingly to prevent pregnancy complications

    Incidence of umbilical cord around the neck and its effects on mode of delivery and fetal outcome at tertiary care hospital

    Background: Umbilical cord is one of the fetal membranes. It is developed from the yolk sac. Umbilical cord is tubular structure. Approximate length of the cord is 50 cm. Umbilical cord around the neck is called as the nuchal cord. The aim of the study is to find out the incidence of umbilical cord around the neck at the time of delivery and its fetal outcomes.Methods: It is a retrospective observational study. This study was conducted in the department of obstetrics and gynecology at a tertiary care hospital over a period of 6 months (Feb-Jul 2020). Out of 542 patients, 388 were enrolled in this study after the inclusion and exclusion criteria, of which 101 were delivered with umbilical cord and the remaining were 287 patients in control group.Results: Present study showed 38.4% incidence of umbilical cord at the time of delivery. Incidence is more among the primigravida and it is statistically significant. Both intrapartum and postpartum risks are not related to umbilical cord around the neck. Mean length of the cord in patients delivered with cord around the neck is more when compared to the control group and has significant p value.Conclusions: This study concludes that the presence of umbilical cord, alone is not an indication for caesarean section. Umbilical cord with 3 loops was associated with increased operative delivery and low Apgar score, proper intrapartum surveillance and portogram leads to good fetal outcome

    Evaluation of induction of labour in a tertiary care hospital

    Background: The objective is to evaluate the preferred methods for labor induction, including incidence of caesarean section, operative and normal vaginal delivery rate, need for oxytocin augmentation, and Apgar score at 1 and 5 min.Methods: This was a hospital-based study carried out in 110 inductions of labour during the study period. misoprostol 50 mcg was inserted in posterior fornix of vaginal in cases with PROM and intracervical 0.5 mg Dinoprostone for other causes in whom induction was decided. Maternal and neonatal outcomes were observed. collected data were analyzed using SPSS and MS excel.Results: Most preferred methods of induction of labour were observed to be Dinoprostone and Misoprostol, augmentation was done by oxytocin. normal delivery rate was 54% in Dinoprostone gel and 58.3% in misoprostol group. Caesarean and operative vaginal delivery rates were 42.8%, 3% in Dinoprostone gel group and 33.3%, 8.3% respectively in misoprostol group. Mean Apgar scores were observed to be similar in both methods of induction at 1st and 5th minutes.Conclusions: It was found that misoprostol had higher percentage of normal and operative vaginal delivery and need for oxytocin augmentation than Dinoprostone. Neonatal outcome as predicted by Apgar score were similar in both groups
