    An MRI volumetric study for leg muscles in congenital clubfoot

    PURPOSE: To investigate both volume and length of the three muscle compartments of the normal and the affected leg in unilateral congenital clubfoot. METHODS: Volumetric magnetic resonance imaging (VMRI) of the anterior, lateral and postero-medial muscular compartments of both the normal and the clubfoot leg was obtained in three groups of seven patients each, whose mean age was, respectively, 4.8 months, 11.1 months and 4.7 years. At diagnosis, all the unilateral congenital clubfeet had a Pirani score ranging from 4.5 to 5.5 points, and all of them had been treated according to a strict Ponseti protocol. All the feet had percutaneous lengthening of the Achilles tendon. RESULTS: A mean difference in both volume and length was found between the three muscular compartments of the leg, with the muscles of the clubfoot side being thinner and shorter than those of the normal side. The distal tendon of the tibialis anterior, peroneus longus and triceps surae (Achilles tendon) were longer than normal on the clubfoot side. CONCLUSIONS: Our study shows that the three muscle compartments of the clubfoot leg are thinner and shorter than normal in the patients of the three groups. The difference in the musculature volume of the postero-medial compartment between the normal and the affected side increased nine-fold from age group 2 to 3, while the difference in length increased by 20 %, thus, showing that the muscles of the postero-medial compartment tend to grow in both thickness and length much less than the muscles of the other leg compartments

    Tendon quality in small unilateral supraspinatus tendon tears. Real-time sonoelastography correlates with clinical findings

    PURPOSE: To investigate the feasibility of real-time sonoelastography in the assessment of the mechanical tendon properties in small unilateral supraspinatus tears, to describe the sonoelastographic properties of the torn supraspinatus tendons and to correlate real-time sonoelastography findings with clinical results and demographic data. METHODS: All the patients presenting for a unilateral rotator cuff tear were prospectively scrutinized. Clinical evaluation included complete physical examination, VAS, Quick DASH, Constant-Murley score, Simple Shoulder Test, ASES score and UCLA score. Radiological evaluation was performed with conventional ultrasounds and real-time sonoelastography; this is a noninvasive method that uses ultrasounds to evaluate the mechanical properties of tissues, reflecting their quality, that can be semi-quantitatively estimated using the strain index. During the enrolment period, 92 patients were scrutinized, and 50 were included in the study. RESULTS: A negative correlation between strain index and VAS for pain, a strong positive correlation between strain index and Constant-Murley score and ASES score were found. Comparable results were observed in male and female patients, but in males, we found a strong positive correlation also for Simple Shoulder Test and UCLA score. CONCLUSIONS: Real-time sonoelastography was a feasible method applicable in the assessment of tendon quality in small supraspinatus tears, and its findings correlated with the clinical results of the patients. In day-by-day clinical practice, this information is very important because quality of tendons is one of the most important prognostic factors for surgeons performing rotator cuff repair.     Liver elasticity as assessed by real-time elastography (RTE) has been shown to be correlated to liver fibrosis in various chronic liver diseases. The aim of our study was to assess the RTE performance in the evaluation of liver fibrosis in nonalcoholic steatohepatitis (NASH), as well as the histopathologic variables determining the eventual discordance between the RTE-predicted and the biopsy-proven fibrosis. Fifty-two consecutive biopsy proven NASH patients and 20 controls were studied. Liver tissue elasticity measurements were performed using the Hitachi EUB-8500 sonographer and the EUP-L52 Linear (3–7 MHz) probe. RTE liver tissue mean elasticity (TME) values were calculated and correlated to the histologic fibrosis, activity and steatosis scores. A decrease in TME was observed with increasing fibrosis (r = −0.75). Similarly, TME varied together consistently with steatosis (r = −0.3). In contrast, TME did not show any correlation with the severity of inflammation. Multiple regression analysis showed that fibrosis was the only variable able to significantly (p < 0.0001) modify TME values. The diagnostic accuracy of TME measurement for F > 0 evaluated by AUC-ROC analysis was 0.86. The diagnostic accuracy of TME measurement for F ≥ 2 was 0.92. We suggest that RTE could be used as a complementary imaging method to evaluate liver fibrosis in NASH patients. Future studies of larger patient cohorts are necessary for the validation of the technique
