Heart Failure Follow Up Phone Calls
Increase the number of 72-hour post-discharge follow-up phone calls for patients hospitalized with a primary diagnosis of heart failure.
This is a quality measure required for Joint Commission Heart Failure Certification and also to maintain American Heart Association Gold Plus Status.https://digitalcommons.centracare.com/nursing_posters/1089/thumbnail.jp
The effect of plasma-core induced self-guiding on phase matching of high-order harmonic generation in gases
In this work we numerically study a self-guiding process in which ionization
plays a dominant role and analyze its effect on high-order harmonic generation
(HHG) in gases. Although this type of self-guiding --- termed as plasma-core
induced self-guiding in previous works --- limits the achievable cut-off by
regulating the intensity of the laser beam, it provides favorable conditions
for phase matching, which is indispensable for high-flux gas high-harmonic
sources. To underline the role of self-guiding in efficient HHG, we investigate
the time-dependent phase matching conditions in the guided beam and show how
the spatio-temporally constant fundamental intensity contributes to the
constructive build-up of the harmonic field in a broad photon-energy range up
to the provided cut-off.Comment: 14 pages, 7 figure
Effect of plasma-core-induced self-guiding on phase matching of high-order harmonic generation in gases
In this work, we numerically study a self-guiding process in which ionization plays a dominant role and analyze its effect on high-order harmonic generation (HHG) in gases. Although this type of self-guiding-termed "plasma-core-induced self-guiding" in previous works-limits the achievable cutoff by regulating the intensity of the laser beam, it provides favorable conditions for phase matching, which is indispensable for high-flux-gas highharmonic sources. To underline the role of self-guiding in efficient HHG, we investigate the time-dependent phase-matching conditions in the guided beam and show how the spatiotemporally constant fundamental intensity contributes to the constructive buildup of the harmonic field in a broad photon energy range up to the provided cutoff. Performing public credit at the eighteenth-century Bank of England
Much is known about the negotiation of personal credit relationships during the eighteenth century. It has been noted how direct contact and observation allowed individuals to assess the creditworthiness of those with whom they had financial connections and to whom they might lend money. Much less is known about one of the most important credit relationships of the long eighteenth century: that between the state and its creditors. This article shows that investors could experience the performance of public credit at the Bank of England. By 1760 the Bank was the manager of nearly three-quarters of the state's debt and housed the main secondary market in that debt. Thus, it provided a place for public creditors, both current and potential, to attend and scrutinize the performance of the state's promises. The article demonstrates how the Bank acted to embody public credit through its architecture, internal structures, and imagery and through the very visible actions of its clerks and the technologies that they used to record ownership and transfer of the national debt. The Bank of England, by those means, allowed creditors to interrogate the financial stability and reputation of the state in the same ways that they could interrogate the integrity of a private debtor.Peer reviewedFinal Accepted Versio
Modular Design of a Passive, Low-Cost Prosthetic Knee Mechanism to Enable Able-Bodied Kinematics for Users With Transfemoral Amputation
There is a significant need for low-cost, high-performance prosthetic knee technology for transfemoral amputees in India. Replicating able-bodied gait in amputees is biomechanically necessary to reduce the metabolic cost, and it is equally important to mitigate the socio-economic discrimination faced by amputees in developing countries due to their conspicuous gait deviations. This paper improves upon a previous study of a fully passive knee mechanism, addressing the issues identified in its user testing in India. This paper presents the design, analysis and bench-level testing of the three major functional modules of the new prosthetic knee architecture: (i) a four-bar latch mechanism for achieving stability during stance phase of walking, (ii) an early stance flexion module designed by implementing a fully adjustable mechanism, and (iii) a hydraulic rotary damping system for achieving smooth and reliable swing-phase control

The natural history of epilepsy in tuberous sclerosis complex
Background: Although epilepsy affects most patients with tuberous sclerosis complex (TSC), little is known about the natural history of epilepsy in this genetic disease.
Methods: A retrospective chart review of all patients with TSC seen between January 2002 and October 2008. Charts were reviewed for a history of infantile spasms (IS), seizure other than IS, refractory epilepsy, Lennox-Gastaut syndrome (LGS), anticonvulsant medication use, ages of seizure onset, last seizure, last clinic visit, clinical seizure phenotype(s), cognitive impairment, and genetic mutation.
Results: Two hundred ninety-one patients were included. Among these patients, 37.8% had a history of IS; 85.2% had a history of seizure; 54.1% developed multiple seizure types, not including IS; 63.2% had seizure onset in the first year of life; and 12.1% of adults without a seizure history developed epilepsy. Of epilepsy patients, 62.5% developed refractory epilepsy and 33.5% achieved epilepsy remission; 37.5% of these patients achieved medication freedom. IS was a risk factor for refractory epilepsy (p<0.0001) and LGS (p<0.0001). History of seizure, IS, age at seizure onset, and refractory epilepsy each correlated with poor cognitive outcome (p<0.0001). Epilepsy remission correlated with better cognitive outcome (p<0.0001). TSC2 was a risk factor for IS and epilepsy; patients without an identified mutation were more likely to achieve remission.
Conclusion: Most patients with TSC develop epilepsy and most develop multiple seizure types. Onset typically occurs in the first year of life; however, adults remain at risk. Although refractory epilepsy is common, many patients achieve seizure control. Many features of seizure history are predictive of cognitive and epilepsy outcome
Patients\u27 Willingness to Accept Social Needs Navigation After In-Person Versus Remote Screening
Social needs screening and referral interventions are increasingly common in health care settings. Although remote screening offers a potentially more practical alternative to traditional in-person screening, there is concern that screening patients remotely could adversely affect patient engagement, including interest in accepting social needs navigation
