PTH and arterial hypertension
Parathormon (PTH) jest od wielu lat postrzegany
jako potencjalny czynnik hipertensynogenny. Pogląd
ten opiera się jednak tylko na dowodach pośrednich,
takich jak podwyższone stężenie PTH w surowicy
u chorych z nadciśnieniem tętniczym pierwotnym oraz
zwiększona częstość występowania nadciśnienia tętniczego
u pacjentów z pierwotną nadczynnością
przytarczyc (pHPT). Parenteralne podanie PTH powoduje
krótkotrwały rozkurcz naczyń oraz wzrost
natriurezy, co przemawia przeciwko udziałowi PTH
w patogenezie nadciśnienia tętniczego. Wpływ PTH
na ciśnienie tętnicze może być następstwem pogorszenia
funkcji śródbłonka naczyń, proliferacji komórek
mięśni gładkich naczyń oraz przerostu mięśnia
sercowego. Wątpliwości dotyczące udziału PTH
w etiopatogenezie nadciśnienia pierwotnego powstają
przy uwzględnieniu czynności wydalniczej nerek,
która często jest upośledzona nie tylko u chorych
z pHPT, ale również u pacjentów z nadciśnieniem
tętniczym związanym z nefropatią nadciśnieniową
i starzeniem się organizmu. Należy więc zwrócić
szczególną uwagę na niewątpliwy niekorzystny
wpływ przewlekłej ekspozycji nerek na zwiększone
stężenie PTH i wapnia w surowicy u pacjentów
z pHPT. Niezależnie od nieudowodnionej roli PTH
w etiopatogenezie nadciśnienia tętniczego, pHPT
wiąże się niewątpliwie ze zwiększonym ryzykiem
schorzeń sercowo-naczyniowych.
Celem niniejszego, krytycznego przeglądu literatury
jest przedstawienie aktualnego stanu wiedzy na temat
zależności pomiędzy PTH a nadciśnieniem tętniczym.Parathormon (PTH) has been recognized as a potential
factor in the etiopathogenesis of essential hypertension.
The higher prevalence of arterial hypertention in primary
hyperparathyroidism (pHPT) than in the general population
and higher serum PTH concetration in patients
with essential hypertensive are the only indirect evidence
confirming this assumption. Infusion of PTH in
man results in transient vasodilatation and increased
urinary sodium excretion that gives evidence against its
hypertensive effect. On the contrary, impaired endothelial
function, proliferation of vascular smooth cells and
heart hypertrophy may cause increase of blood pressure.
Question concerning the role of PTH in the
pathogenesis of arterial hypertension arises when excretory
renal function, which is often impaired not only in
patients with pHPT but also with arterial hypertension
as a result of hypertensive nephropathy and ageing, is
taken under consideration. It is important to remember
about the negative consequences of long-term kidney
exposition to increased PTH and serum calcium level
in patients with pHPT. However, the role of PTH in
the etiopathogenesis of arterial hypertension is still unclear,
pHPT is undoubtedly associated with increased
cardiovascular risk.
The aim of this critical review is to summarize current
arguments concerning the relationship between PTH and
arterial hypertension

Current imaging methods for assessing Graves` orbitopathy activity with particular emphasis on FDG-PET
The most frequent extrathyroidal Graves’ disease manifestation is Graves’ orbitopathy (GO). The treatment of GO is determined by its severity and activity. There is currently no reliable, impartial method for assessing it clinically or distinguishing fibrosis from active inflammatory disorders. Today, imaging methods including orbital ultrasound (US), computed tomography (CT), and magnetic resonance imaging (MRI) are frequently employed to show pathological abnormalities in the ocular adnexa of GO patients. In addition, a not widely accepted technique – 99mTc-DTPA SPECT – has some potential to evaluate retrobulbar inflammation in GO patients. However, FDG-PET/CT is possibly superior to other imaging modalities in detecting inflammation in GO and it may be useful in assessing disease activity in case of clinical or serological uncertainty. It might also act as an early indicator of GO development and its aggravation before irreversible tissue alterations take place and may be used in the differential diagnosis of inflammatory disorders of the orbit. However, before FDG-PET/CT could be applied in daily clinical practice, the methodology of GO activity assessment with defined cut-off values for radionuclide concentration – standardized units of value (SUV) have to be established and validated. In addition, the limitations of this technique have to be recognized

Ograniczenia terapii raka piersi u chorych z przewlekłą niewydolnością nerek
Rak piersi jest najczęściej występującym nowotworem złośliwym u kobiet. Dotyczy najczęściej kobiet po 50. roku życia. W związku ze stosunkowo późnym szczytem zapadalności choroba ta dotyka kobiety obciążone również innymi schorzeniami przewlekłymi, między innymi przewlekłą chorobą i niewydolnością nerek, w tym leczonych nerkozastępczo. Ryzyko zachorowania na raka piersi u chorych hemodializowanych jest większe niż w populacji ogólnej. W niniejszym artykule podsumowano aktualną wiedzę na temat możliwości zastosowania uzupełniającego i paliatywnego leczenia systemowego oraz hormonoterapii w raku piersi u chorych dializowanych. Uwzględniono zmienioną farmakokinetykę leków, toksyczność i ryzyko działań niepożądanych oraz tolerancję leków. Przedstawione dane literaturowe wskazują na możliwość prowadzenia chemioterapii, immunoterapii i hormonoterapii u chorych hemodializowanych
Hypercalcemia in Patients Treated with Oral Bisphosphonates for Tumor-Induced Osteolysis
Objective: Hypercalcemia as the consequence of an excessive bone resorption is a common complication in patients with cancer. The aim of the study was to analyze the prevalence of hypercalcemia in patients with tumor-induced osteolysis starting therapy with bisphosphonates.Methods: The questionnaire-based survey (data collected during three consecutive examinations within a 3-month period) was conducted among 1,450 patients treated with bisphosphonates for tumor-induced osteolysis.Results: Hypercalcemia was found in 8.7% respondents starting the treatment with bisphosphonates. The most common cause of malignancy-associated hypercalcemia was prostate cancer, multiple myeloma and breast cancer. On the other hand, hypercalcemia was the most prevalent among patients with multiple myeloma, metastatic cancer of an unknown primary origin and bladder cancer. Metastases were reported in 342 patients, while pathological fractures in 37. The normalization of calcium level was obtained in 91.4% of the patients treated with bisphosphonates, mostly clodronate. During the bisphosphonate therapy, pathological fractures occurred in 4.6% of patients and the percentage of the patients reporting bone pain decreased from 79.9% to 30.9%.Conclusion: Multiple myeloma, prostate and breast cancer are the most common causes of hypercalcemia of malignancy in patients with tumor-induced osteolysis starting therapy with bisphosphonates
