    A Diabetes Crash Course in a Primary Care Setting to Improve Diabetes Self-Care: A Pilot Project

    Diabetes (DM) is a deadly disease affecting millions. Lack of proper DM self-care management can lead to numerous complications and even death. The American Diabetes Association (ADA) recommends lifestyle change as a first-line treatment along with diabetes self-management education (DSME). However, according to the Centers for Disease Control and Prevention (CDC) only a fraction of those diagnosed attend. The purpose of this project was to improve DM self-management behaviors following a patient focused unique DMSE designed to meet community needs. A customized crash course style DM educational session was offered to all DM patients in a rural primary care clinic. The Diabetes Self-Management Questionnaire (DSMQ) was utilized to assess mean DM self-management changes within four sub-categories and self-management sums using a pre-post intervention design. Paired t tests reveal a statistically significant change in glucose management (p = 0.13) among six voluntary participants. All other measures found no statically significant change. Although small, this change supports use of brief DMSE to improve DM self-management practices. For continuing adequate health care, the perspective of social determinants of health (SDOH) has been recognized as indispensable intervention target for evaluating health equity. SDOH include socio-economic situation, neighbourhood and physical circumstance, food environment, health care, social context with social support capital and relationship. For contextual factors, American Diabetes Association (ADA) established the writing committee and reviewed the detail of SDOH.     Regarding the diet treatment of diabetic nephropathy, protein restriction has been recommended. American Diabetes Association (ADA) proposed protein restriction guidelines in the 2008 edition. However, this comment was deleted in the 2013/2019 edition, because of insufficient evidence. A recent report showed that the intake of plant protein has a protective effect on the decrease of estimated glomerular filtration rate (eGFR), and the intake of animal protein has neither protection nor deterioration. There are controversies about the relationship between protein intake and the reduction of renal function.
