    Evaluation of depression and anxiety in patients with thalassemia: a comparative study

    Background: More studies in the field of thalassemia patients focus on medical treatment and less attention to the psychosocial problems aspect. Objective: The aim of this study was evaluation of depression and anxiety in these patients in Qazvin province. Methods: This case-control study has been conducted in Qods Hospital of Qazvin (2016). The study population included thalassemia major patients and control group were participating in this study (n=147 in each group). All subjects completed the hospital anxiety and depression scale (HADS). In addition to clinical history, socioeconomic and demographic characteristics of patients collected and analyzed using proper statistical methods independent t-test, chi-square test. Findings: Borderline abnormal (borderline case) and abnormal states of anxiety were reported 27.2% and 42.2% in case group compared with 22.4% and 6.8% in the control group (P<0.001). In addition, borderline and abnormal states of depression were significantly different in the case group than the control group (respectively 20.4% and 59.2% vs 24.5% and 12.9%, P<0.001). Among patients with thalassemia, depression and anxiety scores significantly were associated with noncompliance and iron chelation, marital status (single) and socioeconomic state (P<0.001). In addition, Moreover, in terms of depression individuals with diabetes, headaches and skin disorders significantly were higher scores of depression and in terms of anxiety individuals with low back pain, headaches and skin disorders were higher scores. Conclusion: The prevalence of depression and anxiety in patients with thalassemia is considerable and related to socioeconomic and demographic characteristics, so it is essential that psychiatric consultation systems created in thalassemia clinics of Qazvin province

    Zolpidem Dependency and Withdrawal Seizure: A Case Report Study

    Introduction: Zolpidem is a short acting inducer of sleep and thought to lack benzodiazepine properties such as anxiolysis, anticonvulsion, muscle relaxation and side effects such as dependency. Recently, some cases of Zolpidem abuse and dependency have been reported. In review of literature, we found that the lowest reported dosage of Zolpidem, which caused dependency, was 160 mg daily. Case Presentation: We reported a 30-year-old unmarried Iranian woman with dysthymic disorder and chronic insomnia treated with Zolpidem irregularly. She started to use Zolpidem with 5mg per day irregularly since a year ago but augmented its daily dosage gradually to 100 to 150 mg per day in divided doses. After a period of 16 hours without taking Zolpidem she developed a withdrawal syndrome, with generalized tonic-clonic seizures for two times. She was managed with supportive care and recovered completely. Conclusions: Zolpidem dependency and withdrawal seizure can occur with a dosage under last reported doses. Therefore, possibility of mentioned problems cannot be excluded at any dosage and physicians should pay more attention to potential of Zolpidem to create these adverse effects.     Behavioral Modification of Parents With the Barkley Method to Increase Resilience of Mothers of Children With Attention Deficit/Hyperactivity Disorder

    Background: Mothers of children with attention deficit/hyperactivity disorder are faced with more stress than mothers of healthy children and this extreme stress can affect their ability to train their children effectively. Psychologists believe that there is a moderating factor between stressful events and psychological disorders that cause stressful events to have different effects on individuals. A type of this moderating factor is resiliency. Objectives: To determine the efficacy of behavioral modification with Barkley method on resilience of mothers of children with attention deficit/hyperactivity disorder (ADHD). Patients and Methods: Twenty-four mothers of children diagnosed with ADHD were randomly enrolled in this study. The mothers were selected from the 22nd of Bahman hospital of Qazvin city. Mothers with a low score in resilience with children aged between four and twelve years were assigned to experimental and control groups. In this study, a questionnaire was used to measure resilience. The data were analyzed by covariance analysis. Results: The behavioral modification of mothers with the Barkley method significantly enhanced their resiliency with children of ADHD in the experimental group compared to the control group. Conclusions: The behavioral modification was found to be effective for the resilience of mothers of children with ADHD.     Association of depression and anxiety with osteoarthritis

    Background: Depression and anxiety disorders are among the most common psychiatric disorders in general population. Chronic diseases like osteoarthritis can accompany with increased level of anxiety and depression and may negatively affect patients’ mental health by related chronic pain and disability. Objective: The aim of this study was to determine the association of anxiety and depression with osteoarthritis. Methods: This case control study was conducted in 100 patients with osteoarthritis referred to the rheumatology clinic of Avicenna hospital and 100 healthy controls without any history of known psychiatric disorders or taking medication during 2013. Depression and anxiety were assessed by the Beck Depression Inventory and Ketel Anxiety questionnaire, respectively. Demographic information and pain years were measured using a questionnaire. Data were analyzed using Chisquare test, student T-test and Pearson’s correlation coefficient. Findings: 66% of the patients with osteoarthritis and 58% of the controls were diagnosed to have depression and the difference was not statistically significant. 7% of the case group and 2% of the control group had major depression. Frequency of anxiety was 45% in the patients with osteoarthritis and 42% in the controls. 17% of the case group and 12% of the control group had severe anxiety. Conclusion: With regards to the high frequency of depression and anxiety among patients with osteoarthritis and their impact on patients’ compliance, early diagnosis and treatment of them are necessary.     Prevalence of Anxiety and Depression in Diabetic Patients: A Comparative Study

    Background: Living with diabetes and managing it can have substantial emotional burden on individuals. These changes might affect individuals’ lives in terms of stress and depression. The purpose of this study was to determine the prevalence of stress and depression among diabetic women who referred to endocrine clinic of Qazvin in 2014. Methods: For this purpose, 250 patients (125 individuals suffering from diabetes and 125 individuals as a control group) participated in this study. All individuals completed the beck depression inventory and the cattell anxiety inventory. In addition to these, demographic and clinical records were collected from their medical records and were analyzed by appropriate statistical methods. Results: In terms of the Maximum of mild anxiety there were 52 diabetic individuals (41.6%) versus 69 individuals of the control group (55.2%); in terms of Moderate-severe anxiety there were 73 cases (58.4%) versus 56 patients (44.8%) (P value = 0.031). In studying the Maximum of mild depression, there were 43 patients (34.4%) versus 92 (73.6%); in terms of Moderate-severe depression, there were 82 patients (65.6%) versus 33 (26.4%) (P value = 0.001). On a closer examination among age, type of diabetes, duration of diabetes, and insulin injections; only the duration of having diabetes was significantly associated with depression as one of the mental health variables. Conclusions: This study showed that anxiety and depression are significantly more common among diabetic patients in comparison to the control group in the city of Qazvin; therefore, it is necessary to develop primary care by a system based on the reaction, so that an effective treatment for mental health would take place and, ultimately, the impact of these interventions should be studied.     Background: Parents of children with cancer face numerous physical, social and economic problems during the care process. Their family, marital, occupational and social life is negatively affected by the process of caring children with cancer which in turn leads to reduced quality of life. The present study examines the effectiveness of eye movement desensitization and reprocessing (EMDR) on quality of life in parents of children with cancer.Methods: This is a quasi-experimental study based on pretest-posttest and follow-up design with a control group. Accordingly, 30 mothers of children with cancer were selected based on targeted sampling method and divided into control and experiment groups. Both groups were pre-tested using the quality of life questionnaire (QoL) (SF=36). Then the experiment group was treated for EMDR for 8 sessions while the control group received no treatment. Following, both groups were post tested and collected data were analyzed using one-way ANOVA on SPSS21.Results: Findings of the study showed that the scores of mothers in experiment group on quality of life increased compared to the control group and maintained at follow-up. The highest increase was on general health, vitality, emotional health, social function while more reduction was on emotional problems of the experiment group (P&lt; 0.01).Conclusion: Regarding the fact that illness of children affects all aspects of parents’ life, and also results of the present study indicating the effectiveness of EMDR on increasing the quality of life in parents of children with cancer, it is suggested to consider psychological training and treatment as the top priority for children with cancer to improve the family function.
