    Analyzing the Differential Impact of Semen Preparation Methods on the Outcomes of Assisted Reproductive Techniques

    Sperm separation plays a critical role in assisted reproductive technology. Based on migration, density gradient centrifugation and filtration, a properly selected sperm could help in increasing assisted reproductive outcomes in teratozoospermia (TZs). The current study aimed to assess the prognostic value of four sperm selection techniques: density gradient centrifugation (DGC), swim-up (SU), DGC-SU and DGC followed by magnetic-activated cell sorting (DGC-MACS). These were evaluated using spermatozoa functional parameters. A total of 385 infertile couples underwent the procedure of intracytoplasmic sperm injection (ICSI), with an isolated teratozoospermia in the male partner. Semen samples were prepared by using one of the mentioned sperm preparation techniques. The improvements in the percentage of normal mature spermatozoa, rate of fertilization, cleavage, pregnancy and the number of live births were assessed. The normal morphology, spermatozoa DNA fragmentation (SDF) and chromatin maturity checked by using chromomycin A3 (CMA3) with DGC-MACS preparation were better compared to the other three methods. Embryo cleavage, clinical pregnancy and implantation were better improved in the DGC-MACS than in the other tested techniques. The DGC-MACS technique helped in the selection of an increased percentage of normal viable and mature sperm with intact chromatin integrity in patients with teratozoospermia

    Expression variation of OGG1 and HPRT gene and DNA damage in arsenic exposed industrial workers

    Arsenic exposure alters redox balance, induces DNA damage, and deregulates many genes. OGG1 gene involved in base repair mechanism, for excision of 8-oxoguanine (8-oxoG) from DNA formed as a result of accumulation of ROS in cell. HPRT gene encode transferase enzymes involved in purine recycling mechanism. The main focus of the study was to evaluate the expression variation in HPRT, OGG1 gene expression, and DNA damage of industrial workers. Blood samples of 300 occupational workers were collected from welding, brick kiln, furniture, pesticide, and paint industry (n = 60/industry) to evaluate the expression variation in HPRT, OGG1 gene expression, and DNA damage in blood cells by comet assay along with age and gender matched 300 control individuals. Blood arsenic content was higher (P\u3c0.001) in an industrial group compared to the control. OGG1 and HPRT expression were (P\u3c0.05) downregulated in exposed workers compared to controls. Spearman correlation analysis showed a significant positive correlation between HPRT vs OGG1 (P\u3c 0.0001) in exposed workers compared to controls. Altered expression of both genes was observed between workers with \u3c25years and \u3e25years of age as well as between workers with \u3c10years and \u3e10year exposure. Reduced expression (P\u3c0.05) of both genes and a high extent of DNA damage was evident in exposed smokers compared to respective non-smokers. DNA fragmentation was higher (P\u3c0.05) in the furniture, welding and brick kiln group compared to control, and other industries. The present study suggests that altered expression of OGG1 and HPRT gene induce oxidative stress, showed a negative impact on the recycling of purines leading to DNA damage which increase the vulnerability of workers to carcinogenicity

    Exploring the relationship of cognitive function with and without COVID-19 recovered schizophrenic patients

    BackgroundThe Coronavirus disease 2019 (COVID-19) is linked to the deterioration of cognitive function among individuals suffering from schizophrenia. The purpose of this study was to compare the cognitive performance of schizophrenic patients before and after COVID-19.MethodsA longitudinal cohort study involving a sample of 219 individuals diagnosed with schizophrenia was enrolled between June 2022 and May 2023. The participants were split into two groups infected with COVID-19 (n = 165) and not infected with COVID-19 (n = 54). The data were gathered via a questionnaire on demographic characteristics, the Brief Assessment of Cognition in Schizophrenia (BACS), the Positive and Negative Syndrome Scale (PANSS), the Calgary Depression Scale for Schizophrenia (CDSS), the Activities of Daily Living (ADL) scale, and the Insomnia Severity Index (ISI).ResultsThe repeated-measures ANOVA showed that Among patients diagnosed with COVID-19, there was a deterioration in global cognitive function (before COVID-19 = −2.45 vs. after COVID-19 = −3.02, p = 0.007), working memory (before COVID-19 = −2.76 vs. after COVID-19 = −3.34, p < 0.00 1), motor speed (before COVID-19 = −1.64 vs. after COVID-19 = −2.12, p < 0.001), attention and speed of information processing (before COVID-19 = −1.93 vs. after COVID-19 = −1.16, p = 0.008). multi-variable analysis showed that several factors as having a secondary grade of education (β = 0.434), experiencing insomnia (β = 0.411)and the interaction between COVID-19 diagnosis and cognition at baseline (β = 0.796) were significantly associated with cognitive deficits. At the same time, no significant associations were found between global cognition and clinical symptoms, autonomy, or depression (p > 0.05).ConclusionThe COVID-19 pandemic has significantly impacted various cognitive functions, such as verbal memory, working memory, and global cognition. Insomnia has been identified as the predominant determinant of cognitive impairment, alongside the confirmation of a COVID-19 diagnosis. Additional research is imperative to elucidate the diversification of cognitive functionality observed in individuals diagnosed with schizophrenia who have acquired COVID-19

    An innovative state-of-the-art health storytelling technique for better management of type 2 diabetes

    Background: Type 2 diabetes (T2D) is a chronic lifelong disease that requires long-term prevention and management strategies in a community setting. A health story is a novel technique that may be used as an effective tool for better prevention and management of T2D. Objective: The main objective of this study is to develop a story to be used as a social health technique based on contemporary scientific knowledge that may be used at a community level for better communication and management of T2D. Methods: A community–academic partnership was formed with a not-for-profit Nutrition Education, Awareness, and Training (NEAT) organization in Khyber Pakhtunkhwa, Pakistan. We agreed to develop a story that may be used as a health and nutrition education communication tool for better management of patients with T2D. The following phases were followed during the story creation process: (1) the theory phase, (2) the modeling phase, and (3) the evaluation phase. Raters evaluated the story to determine its literary and scientific quality, comprehensiveness, and T2D specificity. Results: The title of the story translated into English is “The Story of Diabetes—The Story of Success.” It is text based and contains 86 pages in the local language, “Pashto,” with an English translation. The story is divided into five chapters and describes the initial diagnosis, fear associated with the disease, issues related to referral to certified practitioners, the importance of a balanced diet, and related lifestyle habits. After story evaluation, the raters suggested its literary and scientific quality, comprehensiveness, and T2D specificity (Pearson correlation scores of \u3e0.8). Conclusion: This unique story was created for T2D and found to be of significant quality in terms of its literary and scientific quality, as well as its comprehensiveness and diabetes specificity. As a result, it may be suggested that it can be used in subsequent studies to improve T2D management among adult patients

    Ramadan-specific nutrition education improves cardio-metabolic health and inflammation—a prospective nutrition intervention study from Pakistan

    There are recent reports that Ramadan fasting (RF) results in weight gain instead of weight loss. In addition, the data on the efficacy of brief nutrition education on healthy eating practices in Ramadan for better health are scarce. Therefore, a study was conducted to investigate the effects of brief nutrition education before the start of RF on healthy eating practices during RF. For this purpose, a prospective observational study focused on “Dietary Education and Awareness for Ramadan (DEAR)” as an intervention was carried out. The participants (n = 74) were recruited and divided into two groups, i.e., intervention and control groups (n = 37 each). As an intervention, nutrition education lessons were given before and during RF month. The control group did not attend these nutrition education lessons. Data on anthropometrics, dietary intake, and other parameters were collected at three time points: before, in the end, and 4 weeks after RF. Weight was measured in kg; height, waist circumference (WC), and hip circumference (HC) were measured in cm; and body mass index (BMI) was calculated. Waist-to-hip ratio (WHR) was calculated by dividing the waist value by the hip value. Body composition analysis was performed by the body composition analyzer (BF-907). Blood pressure (BP) was measured using a validated automated blood pressure. A 3–5 ml of venous blood was collected, and plasma and serum were separated. Serum and plasma samples were processed for general blood chemistry (blood lipid profile, glucose, and CRP) within 2 h. CRP was determined by the immunoturbidimetry method using an auto-analyzer. An enzyme-linked immunosorbent assay (ELISA) was used to determine cytokine/chemokines. Adherence to nutrition education (intervention) was assessed. The results show that nutrition education has positive effects on overall nutrition. Significant improvement in dietary adherence to dietary advice in the intervention group was noted. Significant BW loss (mean loss: 1.21 kg) in the intervention group was observed. The majority (63.3%) had lost BW ≥ 1.0 kg. Other changes observed as a result of the intervention included improvements in blood glucose, cholesterol, CRP levels, and systolic and diastolic BP. There was a notable shift in pro- and anti-inflammatory cytokine concentrations: IL-7, IL-4, and TGF-α decreased, while IL-2, TNF-α and resistin, IL-1 RA, IL-17 A, and sCD40 increased. In conclusion, RF resulted in a loss in mean BW and an improvement in related blood chemistry and cytokine profiles. Furthermore, nutrition education before RF resulted in better nutrition practices during RF and a desirable healthy BW, blood lipid, and cytokine profiles

    Impact of COVID-19 pandemic on obesity among adults in Jordan

    COVID-19 is a severe acute respiratory syndrome that mainly affects the human respiratory system. Unhealthy nutritional habits and obesity are expected as consequences of protective measures including quarantine. Obesity, in its growing prevalence, is a worldwide health issue associated with worsening health conditions. This is a cross-sectional study to assess the impact of the COVID-19 pandemic on obesity among Jordanian adults and across epidemiological statuses. Participants were randomly selected, and the survey was distributed on social media networking sites. A total of 672 subjects were surveyed and participated in the study between March and June 2021 via Google Form questionnaire. The results indicated that 74.4% of participants reported that they did not do any physical activity, and 43.5% changed their lifestyle and eating habits for the worse. During the COVID-19 pandemic, almost half of the participants reported an increase in hunger, consuming 3–4 meals/day, and consuming < 1 liter of water/day. Additionally, more than half of the participants reported no change in fat, cereals, and protein consumption, 46.4 % had no change in fruit and vegetable consumption, and 50.6% increased their consumption of sweets. Our results showed a significant increase in the self-reported BMI categories during the COVID-19 pandemic for all ages (p < 0.001). Change in weight and BMI was significantly associated with marital status, education level, living place, family size, family working members, and working status. Participants across all epidemiological statuses displayed a statistically significant increase in BMI. This study was conducted to observe the impacts of the COVID-19 pandemic on health behaviors and obesity among Jordanian adults and across epidemiological statuses. We found that there were significant negative changes in the lifestyle (physical activity) and eating behaviors of Jordanians during the COVID-19 quarantine which in turn increased their body weight and changed the obesity rate

    Dietetic management of obesity in Saudi Arabia: towards evidence based clinical practice guidelines

    Obesity is one of the most common disorders encountered in clinical practice and has major public health implications. It is also one of the most difficult and frustrating disorders to manage successfully. The prevalence of overweight and obesity in Saudi Arabia is high and increasing over recent years. Management of obesity should be based on the best available scientific evidence. At present, there are no national clinical practice guidelines of use by dietitians and other health practitioners for the management of obesity. Since dietetics is a relatively new profession in Saudi Arabia there is little published data available in this area. This thesis aimed to describe the current dietetic practices of obesity in Saudi Arabia and to develop a draft set of national clinical practice guidelines for obesity management. The present thesis includes three main projects. Based on the outcomes of these projects, a draft of evidence-based practice guidelines for the nutritional management of obesity in Saudi Arabia was prepared. The first project (Chapter 3) involved dietitians to investigate the context and better understand the range of current practices in obesity management in Saudi Arabia, demand for and level of service, and barriers to obesity management. Analysis of the study showed that Saudi Arabian dietetic practice for the management of obesity does incorporate most practice recommendations, but some specific elements are rarely used. The most common assessment approaches were assessment of BMI, exercise habits and weight history while the most common strategies for obesity management were dietary total fat reduction and increased incidental daily activity. The major barriers for establishment of a weight management clinic were inadequate resources and administration and referral issues. None of the participants used local obesity guidelines but 61% of participants relied on international guidelines. The second project included two studies focused on the validity of the most important practical tools used for the classification of obesity (Chapter 4) and the assessment of energy requirements (Chapter 5) since research has been lacking in this area in the Saudi population. The first study examined the use of different BMI cut-off points for obesity classification. Results indicated that the diagnostic usefulness of BMI alone in defining obesity is limited in the Saudi adult population, for both men and women. It seems likely that limiting management of obesity only to those individuals with a BMI ≥ 30, as defined by the WHO, may mean that many Saudis at risk of serious co-morbidities could be missing necessary interventions. The second study assessed the accuracy of prediction equations and a popular hand-held calorimeter (BodyGem) for assessment of resting energy expenditure (REE). Based on the findings of this study it was concluded that the Harris-Benedict, Schofield and WHO equations tend to predict REE more accurately than the BodyGem device. However, their accuracy was not clinically acceptable on an individual level. Therefore, the value of the use of both BodyGem devices and predictive equations is still uncertain for Saudi population and more research is needed in this area. The third project (Chapter 6) focused on the development of draft clinical practice guidelines, based on a review of existing international guidelines, supplemented with systematic literature reviews, and refined through the use of consultation workshops and Delphi technique consultations with Saudi experts and practitioners. Findings from the systematic mini reviews provided low to medium level evidence for the use of some novel dietary interventions such as the high intake of calcium, PUFA or fiber to assist with weight loss or maintenance. There was also similar evidence for the use of a low glycemic index diet. Higher eating frequency, not exceeding 6 meals per day, may also help in weight reduction. Regular breakfast intake also appears to be associated with lower body weight. Consultations workshops and Delphi consultations indicated that there are cultural differences between Saudi Arabian population and other Western populations. Therefore, specific consensus statements were developed to cover practice areas such as behavioral modifications, dietary counselling strategies, physical activity and obesity management in Ramadan. In summary, this thesis has provided clinical practice guidelines for obesity management in Saudi Arabia. The application of these guidelines will improve nutritional management of obesity and enable dietitians and other health professionals to use approaches based on the best available evidence
