    Controlling Rate and Rhythm Increases Feasiblity of CT Angiography in Atrial Fibrillation

    A woman, aged 48 years, with severe rheumatic mitral stenosis and uncontrolled permanent atrial fibrillation (AF) underwent preoperative assessment of coronary arteries. Invasive coronary angiography was not possible because of occluded common iliac artery and bilateral radial spasm. Transesophageal echocardiogram showed a very large mobile left atrial appendage clot, precluding cardioversion. The severe motion artifacts during cardiac CT angiography (64 slices) due to atrial fibrillation were overcome by controlling rhythm and rate through insertion of a temporary pacemaker via right femoral vein, and slowing heart rate below 65 beats per minute by intravenous metoprolol (25 mg) and verapamil (5 mg). Clear pictures of all coronary arteries as well as the left atrial appendage clot were obtained. The temporary pacemaker was removed after eight hours. Uneventful mechanical mitral valve replacement and maze procedure were performed and the patient was discharged in a stable condition

    Objectives: to evaluate the health care services provided for older adults by primary health care centers (PHCCs) in Riyadh, Kingdom of Saudi Arabia (KSA), and the ease of use of these centers by older adults. Methods: between October 2013 and January 2014, we conducted a descriptive cross-sectional study of 15 randomly selected PHCCs in Riyadh City, KSA. The evaluation focused on basic indicators of clinical services offered and factors indicative of the ease of use of the centers by older adults. Evaluations were based upon the age-friendly PHCCs toolkit of the World Health Organization. Results: coverage of basic health assessments (such as blood pressure, diabetes, and blood cholesterol) was generally good. However, fewer than half of the PHCCs offered annual comprehensive screening for the common age-related conditions. There was no screening for cancer. Counseling on improving lifestyle was provided by most centers. However, there was no standard protocol for counseling. Coverage of common vaccinations was poor. The layout of most PHCCs and their signage were good, except for lack of Braille signage. There may be issues of access of older adults to PHCCs through lack of public transport, limited parking opportunities, the presence of steps, ramps, and internal stairs, and the lack of handrails.     Identification and characterization of a novel antimicrobial peptide compound produced by Bacillus megaterium strain isolated from oral microflora

    In recent years, the decreased efficacy of existing antibiotics toward management of emergent drug-resistant strains has necessitated the search for novel antibiotics from natural products. In this regard, Bacillus sp is well known for producing variety of secondary metabolites of potential use. Therefore, we performed an investigation to isolate and identify Bacillus sp from oral cavity for production of novel antimicrobial compounds. We extracted, purified, and identified a novel bioactive compound by B. megaterium (KC246043.1). The optimal production of compound was observed on de Man Rogosa and Sharpe broth by incubating at 37 °C, and pH 7.0 for 4 days. The bioactive compound was extracted by using n-butanol (2:1 v/v), purified on TLC plates with detection at Rf 7.8 cm; further characterized and identified as a cyclic ploypeptide sharing structural similarity with bacitracin. Minimum inhibitory concentration of bioactive compound was found to be 0.25, 0.5, 1.0, 3.125 and 6.25â€ŻÎŒg/ml against Micrococcus luteus ATCC10240, Salmonella typhi ATCC19430, Escherichia coli ATCC35218. Pseudomonas aeruginosa ATCC27853 and Staphylococcus aureus ATCC25923 respectively, with no activity against Candida albicans ATCC10231. Our findings have revealed a novel cyclic peptide compound from B. megaterium with broad spectrum antimicrobial activity against both Gram positive and Gram negative bacteria.     Synthesis of novel Cu/Fe based benzene Dicarboxylate (BDC) metal organic frameworks and investigations into their optical and electrochemical properties

    In the recent past Metal-organic frameworks (MOFs) based thin films have demonstrated superior performance in various technological applications such as optical and optoelectronic devices, electrochemical energy storage, catalysis, and sensing. Herein we report tuning the optical performance of stable complexes using Cu and Fe metal ions with carboxylate benzene dicarboxylic (BDC), leading toward the formation of novel MOF structures. The formation of Cu-BDC and Fe-BDC were confirmed by XRD and SEM studies. The thermal stability of two MOFs was investigated, indicating that, the Cu-BDC is more stable than Fe-BDC. Further, the optical properties were investigated in the wavelength range 325–1100 nm, and the Fe-BDC exhibited greater optical transmission properties than Cu-BDC by 33 %, as investigated by Wemple-DiDomenico and Tauc models. The dispersion parameters related to optical studies for Cu-BDC were better in comparison to Fe-BDC, which could be attributed to the increase in Cu valence electrons due to an increase in the number of cations. The electrochemical behavior in terms of CV measurements shows the presence of pseudo capacitance in both Fe-BDC and Cu-BDC MOFs. The improved CV performance of Cu-BDC MOF suggests that it could be used as a storage material. This work successfully demonstrates the tailoring of optical properties related to MOF thin films through the formation of stable complexes using BDC as a potential material for the fabrication of OLED's and Solar cells. The improved CV performance suggests that these MOF based materials could be used as anodes in fabrication of batteries or supercapacitors
