    Tumor Angiogenesis in Pituitary Adenoma

    The role of angiogenesis in pituitary tumor development used to be questioned, since pituitary tumors have been usually found to be less vascularized than the normal pituitary tissue. Nevertheless, a significantly higher degree of vasculature has been shown in invasive or macropituitary prolactinomas when compared to noninvasive and microprolactinomas. We should know VEGF was found firstly in pituitary anterior lobe, then tumor angiogenesis must occur. Meanwhile the vascular arrangement raised by VEGF is irregular, that sometimes lead to pituitary apoplexy. In this chapter, hypoxia inducible factors (HIF), transcription factors regulating expression of several genes related to oxygen homeostasis are in response to hypoxic stress.     Resorption of apatite-wollastonite containing glass-ceramic and beta-tricalcium phosphate in vivo.

    Apatite-wollastonite containing glass ceramic is considered to be difficult to resorb, but we experienced the disappearance of the porous type of Apatite-wollastonite glass ceramic particles . In this study, the resorption of porous apatite-wollastonite glass-ceramic implanted in the femurs of rabbits was investigated, and the process was compared with beta-tricalcium phosphate, a resorbable ceramics. Porous apatite-wollastonite glass-ceramic (70, 80, and 90% porosity) and beta-tricalcium phosphate (75% porosity) were implanted in the femurs of Japanese white rabbits. Samples were harvested and examined 0, 4, 8, 12, 24 and 36 weeks after implantation. Quantitative analysis of the radiographic and histologic findings was performed with NIH Image software. Radiographic examination demonstrated that the radiopacity and size of the porous apatite-wollastonite glassceramic cylinders decreased gradually after implantation. Histologic examination revealed that the surface area of the apatite-wollastonite glass-ceramic cylinders decreased continuously, and approached 20% of the original area 36 weeks after implantation. However, the resorption rate of porous apatite-wollastonite glass-ceramic was slower than that of beta-tricalcium phosphate. Toluidine blue staining showed abundant new bone formation on the surface of the apatite-wollastonite glassceramic matrix. Considering its mechanical strength, gradual resorption characteristics, and good osteochonductive activity, porous apatite-wollastonite glass-ceramic appears to be a suitable artificial bone substitutes.</p

    Preparation of surfactant-free vinyl polymers by conventional emulsion polymerization using hydrolysable emulsifiers

    Emulsion polymerizations of several vinyl monomers, styrene, methyl methacrylate, butyl methacrylate, butyl acrylate, and vinyl acetate, in water using alkali-hydrolysable cationic surfactants with a betaine ester group (1-alkoxycarbonylmethyl)trimethylammonium chlorides, as emulsifiers were carried out and properties of the resulting latices and the polymers recovered by hydrolysis and salting out were investigated. There were little influences of the surfactants and monomers used here on the polymerizations, forming stable and monodisperse latices with a mean diameter of ca. 70 nm and giving a high molecular weight of polymers at high yields. All polymers were precipitated and recovered by adding a small amount of sodium hydroxide into the latex solutions contained little amount of ionic species. Solvent-cast films of the polymers were found to have surfaces as hydrophobic as those for the corresponding pure polymers prepared by bulk polymerization.ArticlePOLYMER BULLETIN. 67(8):1455-1462 (2011)journal articl

    A Case of Ovarian Actinomycosis

    Background: Pelvic actinomycosis is uncommon and often presents as a complication of an intrauterine device (IUD). A diagnosis of actinomycosis can be made from the finding of sulfur granules within inflammatory exudate on histologic examination after surgery. However, it may be possible to diagnose actinomycosis before surgery by finding Actinomyces-like organisms on Papanicolaou smears. Case: A 41-year-old woman had been diagnosed as having a pelvic abscess, and bilateral salpingo-oophorectomy was performed. She had been an IUD user for 6 years. Actinomyces-like organisms were detected in her previous Papanicolaou cervical smears. If the patient had been treated when the Actinomyces-like organisms were detected by Papanicolaou smears, the serious ovarian actinomycosis might have been avoided. Conclusion: We suggest that routine cervical examinations are important for women who are IUD users

    Preparation of Polyrotaxane Fibers. Part II: Tensile Properties of Polyrotaxane Fibers Treated with Two Cross-linking Reagents

    Polyrotaxane fibers prepared with wet spinning of polyrotaxane consisting of poly(ethylene glycol) and cyclodextrins were cross-linked with two different cross-linking reagents, i.e., divinyl sulfone (DVS) and ethylene glycol diglycidyl ether (EGDE), to improve the tensile properties of the fibers. By cross-linking with DVS, the values for the tenacity at break and the initial modulus were increased with cross-linking time, while the elongation at break was improved only moderately. On the other hand, drastic improvements in elongation at break were observed after EGDE cross-linking, up to 645% of its original length, although the tenacity at break and the initial modulus showed only slight improvements.     Efficacy and safety of alirocumab in patients with hypercholesterolemia not adequately controlled with non-statin lipid-lowering therapy or the lowest strength of statin : ODYSSEY NIPPON study design and rationale

    Background: Statins are generally well-tolerated and serious side effects are infrequent, but some patients experience adverse events and reduce their statin dose or discontinue treatment altogether. Alirocumab is a highly specific, fully human monoclonal antibody to proprotein convertase subtilisin/kexin type 9 (PCSK9), which can produce substantial and sustained reductions of low-density lipoprotein cholesterol (LDL-C). Methods: The randomized, double-blind, placebo-controlled, parallel-group, phase 3 ODYSSEY NIPPON study will explore alirocumab 150 mg every 4 weeks (Q4W) in 163 Japanese patients with hypercholesterolemia who are on the lowest-strength dose of atorvastatin (5 mg/day) or are receiving a non-statin lipid-lowering therapy (LLT) (fenofibrate, bezafibrate, ezetimibe, or diet therapy alone). Hypercholesterolemia is defined as LDL-C ≥ 100 mg/dL (2.6 mmol/L) in patients with heterozygous familial hypercholesterolemia or non-familial hypercholesterolemia with a history of documented coronary heart disease, or ≥120 mg/dL (3.1 mmol/L) in patients with non-familial hypercholesterolemia classified as primary prevention category III (i.e. high-risk patients). During the 12-week double-blind treatment period, patients will be randomized (1:1:1) to receive alirocumab subcutaneously (SC) 150 mg Q4W alternating with placebo for alirocumab Q4W, or alirocumab 150 mg SC every 2 weeks (Q2W), or SC placebo Q2W. The primary efficacy endpoint is the percentage change in calculated LDL-C from baseline to week 12. The long-term safety and tolerability of alirocumab will also be investigated. Discussion: The ODYSSEY NIPPON study will provide insights into the efficacy and safety of alirocumab 150 mg Q4W or 150 mg Q2W among Japanese patients with hypercholesterolemia who are on the lowest-strength dose of atorvastatin, or are receiving a non-statin LLT (including diet therapy alone)

    Intraluminal implantation of rectal carcinoma successfully resected by endoscopy.

    A 55-year-old Japanese woman presented at our hospital complaining of hematochezia 4 months after surgery for a rectal carcinoma. A proctoscopy revealed 2 protuberant lesions in the rectum, 5 mm anally from the anastomotic suture line. The diagnosis of adenocarcinoma was confirmed by biopsy. It was considered that these lesions were caused by intraluminal implantation from the primary rectal carcinoma. The patient underwent an endoscopic resection for these recurrent lesions and has remained stable, with neither recurrence nor metastasis, in the 7 years since the resection. For rectal carcinoma, we propose early follow-up by proctoscopy, namely within 4 months after surgery.</p

    Prevalence of the metabolic syndrome in elderly and middle-aged Japanese

    AbstractBackground/PurposeDiagnosis and management of the metabolic syndrome (MetS) are beneficial for successful aging. In spite of several criteria for MetS, there is little information on cardiometabolic risk clustering in elderly Japanese. The purpose of this study was, therefore, to determine the relationship between age-associated changes in obesity and metabolic components in the Japanese.MethodsWe analyzed data from the nationwide survey conducted in 2000. Using Adult Treatment Panel III (ATP III) and Japanese diagnostic criteria for MetS, we analyzed 2366 people aged from 40 to 79 years (men, 1425 and women, 941) from the total participants.ResultsThe prevalence of MetS was almost three fold higher by modified ATP III, International Diabetes Federation, and Japanese criteria, in elderly women than in middle-aged women, whereas no difference was found between middle-aged and elderly men by the three criteria. A marked increase in the prevalence of MetS was found by modified ATP III and International Diabetes Federation criteria compared with that by the Japanese criteria in women. Among the risk factors, the prevalence of central obesity and dyslipidemia increased only in women and that of high fasting glucose and high blood pressure increased in both genders with aging. Among the MetS subjects who fulfilled the modified ATP III criteria, more clustering of risk was observed in elderly than in middle-aged subjects, especially in women. Blood pressure increased and triglyceride decreased in both genders, and non-high-density-lipoprotein cholesterol decreased in elderly men. The prevalence of dyslipidemia decreased in elderly men.ConclusionAging is an important factor that affects the metabolic abnormality, and aging of the population would lead to increase in the prevalence of MetS. Therefore, the development of better approaches to the prevention and management of MetS is necessary for successful aging in our society

    Impact of functional studies on exome sequence variant interpretation in early-onset cardiac conduction system diseases

    Aims The genetic cause of cardiac conduction system disease (CCSD) has not been fully elucidated. Whole-exome sequencing (WES) can detect various genetic variants; however, the identification of pathogenic variants remains a challenge. We aimed to identify pathogenic or likely pathogenic variants in CCSD patients by using WES and 2015 American College of Medical Genetics and Genomics (ACMG) standards and guidelines as well as evaluating the usefulness of functional studies for determining them. Methods and Results We performed WES of 23 probands diagnosed with early-onset (&amp;lt;65 years) CCSD and analyzed 117 genes linked to arrhythmogenic diseases or cardiomyopathies. We focused on rare variants (minor allele frequency &amp;lt; 0.1%) that were absent from population databases. Five probands had protein truncating variants in EMD and LMNA which were classified as “pathogenic” by 2015 ACMG standards and guidelines. To evaluate the functional changes brought about by these variants, we generated a knock-out zebrafish with CRISPR-mediated insertions or deletions of the EMD or LMNA homologs in zebrafish. The mean heart rate and conduction velocities in the CRISPR/Cas9-injected embryos and F2 generation embryos with homozygous deletions were significantly decreased. Twenty-one variants of uncertain significance were identified in 11 probands. Cellular electrophysiological study and in vivo zebrafish cardiac assay showed that 2 variants in KCNH2 and SCN5A, 4 variants in SCN10A, and 1 variant in MYH6 damaged each gene, which resulted in the change of the clinical significance of them from “Uncertain significance” to “Likely pathogenic” in 6 probands. Conclusions Of 23 CCSD probands, we successfully identified pathogenic or likely pathogenic variants in 11 probands (48%). Functional analyses of a cellular electrophysiological study and in vivo zebrafish cardiac assay might be useful for determining the pathogenicity of rare variants in patients with CCSD. SCN10A may be one of the major genes responsible for CCSD.
