    The Role of Perfusion Computed Tomography in the Prediction of Cerebral Hyperperfusion Syndrome

    Hyperperfusion syndrome (HPS) following carotid angioplasty with stenting (CAS) is associated with significant morbidity and mortality. At present, there are no reliable parameters to predict HPS. The aim of this study was to clarify whether perfusion computed tomography (CT) is a feasible and reliable tool in predicting HPS after CAS.We performed a retrospective case-control study of 54 patients (11 HPS patients and 43 non-HPS) with unilateral severe stenosis of the carotid artery who underwent CAS. We compared the prevalence of vascular risk factors and perfusion CT parameters including regional cerebral blood volume (rCBV), regional cerebral blood flow (rCBF), and time to peak (TTP) within seven days prior to CAS. Demographic information, risk factors for atherosclerosis, and perfusion CT parameters were evaluated by multivariable logistic regression analysis. The rCBV index was calculated as [(ipsilateral rCBV - contralateral rCBV)/contralateral rCBV], and indices of rCBF and TTP were similarly calculated. We found that eleven patients had HPS, including five with intracranial hemorrhages (ICHs) of whom three died. After a comparison with non-HPS control subjects, independent predictors of HPS included the severity of ipsilateral carotid artery stenosis, 3-hour mean systolic blood pressure (3 h SBP) after CAS, pre-stenting rCBV index >0.15 and TTP index >0.22.The combination of severe ipsilateral carotid stenosis, 3 h SBP after CAS, rCBV index and TTP index provides a potential screening tool for predicting HPS in patients with unilateral carotid stenosis receiving CAS. In addition, adequate management of post-stenting blood pressure is the most important treatable factor in preventing HPS in these high risk patients

    Metalenses : advances and applications

    Metasurfaces, the 2D counterpart of artificial metamaterials, have attracted much attention because of their exceptional ability to manipulate the electromagnetic wave such as amplitude, phase, polarization, propagation direction, and so on. Different from conventional lenses, metalenses based on the metasurface optics are truly flat and compact and exhibit superior performance. In this report, recent progress in the development of metalenses is explored. First, the working principle and characteristics of metalenses are discussed. Then, it is described how the dispersion aberration in metalenses can be eliminated to make them suitable for being employed in a range of applications that are difficult or impossible for traditional lenses. In addition, various metalens‐based applications are introduced, including imaging, high spectral resolution spectroscopy, and multiplex color routing. Furthermore, a survey of reconfigurable and tunable metalenses is conducted. Finally, the report concludes by addressing future prospects of metalenses

    The association between alcohol, betel nut, and cigarette use with hepatitis C virus infection in Taiwan

    Abstract Hepatitis C virus (HCV) infection may cause chronic liver disease, liver cirrhosis, and liver cancer. It has been reported to associate with habits including alcohol, betel nut and cigarette use. We aimed to investigate the association between alcohol, betel nut, and cigarette use with HCV infection in Taiwan and to explore their effects. A total of 121,421 participants were enrolled from the Taiwan Biobank. They were stratified into two groups according to whether they had (n = 2750; 2.3%) or did not have (n = 118,671; 97.7%) HCV infection. All participants were also classified into four groups according to the number of habits, including a history of alcohol drinking, betel nut chewing, and cigarette smoking. There were 85,406 (no habit), 24,299 (one habit), 8659 (two habits), and 3057 (three habits) participants in the four groups, respectively. Multivariable analysis showed that the participants who had an alcohol drinking history (odds ratio [OR] 1.568; 95% confidence interval [CI] 1.388–1.773; p < 0.001), betel nut chewing history (OR 1.664; 95% CI 1.445–1.917; p < 0.001), cigarette smoking history (OR 1.387; 95% CI 1.254–1.535; p < 0.001), were significantly associated with HCV infection. Furthermore, the participants were classified into four groups according to the number of habits as follows: 85,406 (no habit), 24,299 (one habit), 8659 (two habits), and 3057 (three habits). The HCV infection rates in these four groups were 2.11%, 2.14%, 3.23%, and 4.78%, respectively. Compared to the participants with no or one habit, those with two habits had a higher HCV infection rate (all p < 0.001). In addition, compared to the participants who had no, one or two habits, those who had three habits also had higher HCV infection rates (all p < 0.001). The participants who had three habits had the highest prevalence of HCV infection. In an era when most HCV can be cured, understanding the epidemiology link between habits and HCV may help the case finding

    In recent years, there has been a significant increase in collaboration between medical imaging and artificial intelligence technology. The use of automated techniques for detecting medical symptoms has become increasingly prevalent. However, there has been a lack of research on the relationship between impacted teeth and the inferior alveolar nerve (IAN) in DPR images. The severe compression of teeth against the IAN may necessitate the requirement for nerve canal treatment. To reduce the occurrence of such events, this study aims to develop an auxiliary detection system capable of precisely locating the relative positions of the IAN and impacted teeth through object detection and image enhancement. This system is designed to shorten the duration of examinations for dentists while concurrently mitigating the chances of diagnostic errors. The innovations in this research are as follows: (1) using YOLO_v4 to identify impacted teeth and the IAN in DPR images achieves an accuracy of 88%. However, the developed algorithm in this study achieves an accuracy of 93%. (2) Image enhancement is utilized in this study to expand the dataset, with an accuracy of up to 2~3% enhancement in detecting diseases. (3) The segmentation technique proposed in this study surpasses previous methods by achieving 6% higher accuracy in dental diagnosis

    Lung cancer has the highest global mortality rate of any cancer. Although targeted therapeutic drugs are commercially available, the common drug resistance and insensitivity to cisplatin-based chemotherapy, a common clinical treatment for lung cancer, have prompted active research on alternative lung cancer therapies and methods for mitigating cisplatin-related complications. In this study, we investigated the effect of WSG, a glucose-rich, water soluble polysaccharide derived from Ganoderma lucidum, on cisplatin-based treatment for lung cancer. Murine Lewis lung carcinoma (LLC1) cells were injected into C57BL/6 mice subcutaneously and through the tail vein. The combined administration of WSG and cisplatin effectively inhibited tumor growth and the formation of metastatic nodules in the lung tissue of the mice. Moreover, WSG increased the survival rate of mice receiving cisplatin. Co-treatment with WSG and cisplatin induced a synergistic inhibitory effect on the growth of lung cancer cells, enhancing the apoptotic responses mediated by cisplatin. WSG also reduced the cytotoxic effect of cisplatin in both macrophages and normal lung fibroblasts. Our findings suggest that WSG can increase the therapeutic effectiveness of cisplatin. In clinical settings, WSG may be used as an adjuvant or supplementary agent
