    Surgical Management of Lumbar Spine Fractures and Dislocations

    Background: Lumbar spine fractures and dislocations, which are part of the thoracolumbar region, are critical injuries with significant morbidity. The epidemiological shift in the median age of injury and the high prevalence of these injuries, particularly in the T10-L2 region, highlight the necessity for effective therapeutic interventions. With advancements in spine biomechanics, imaging technologies, and surgical techniques, there has been a paradigm shift from conservative to surgical management, though high-quality comparative studies remain limited. Objective: To synthesize recent data on the epidemiology, evaluation, and management of lumbar spine fractures and dislocations, and to elucidate the comparative efficacy of surgical interventions and conservative approaches in optimizing patient outcomes. Method: This paper conducts a comprehensive review of epidemiological data on thoracolumbar traumatic injuries, diagnostic techniques, and management strategies, especially focusing on surgical interventions. The review also details specific surgical techniques utilized for lumbar spine fractures and their underlying rationale. Findings and Conclusion: Thoracolumbar injuries primarily affect the transitional zone (T11-L2) and show a higher incidence in males aged between 20 and 40. Imaging, especially CT scans, offers a definitive diagnostic approach, with MRI providing insights on soft tissue interactions. While historically, conservative methods dominated therapeutic interventions, surgical techniques, including Posterior Instrumentation, Anterior Lumbar Interbody Fusion (ALIF), Transforaminal Lumbar Interbody Fusion (TLIF), and Posterior Lumbar Interbody Fusion (PLIF), are increasingly being utilized. Some specific fractures even warrant a combined posterior-anterior surgical approach. Notably, certain case studies highlight the potential for superior outcomes with surgical intervention, even in the absence of neurological deficits.     Personality profiles of cultures: aggregate personality traits

    Personality profiles of cultures can be operationalized as the mean trait levels of culture members. College students from 51 cultures rated an individual from their country whom they knew well (N = 12, 156). Aggregate scores on Revised NEO Personality Inventory scales generalized across age and gender groups, approximated the individual-level Five-Factor Model, and correlated with aggregate self-report personality scores and other culture-level variables. Results were not attributable to national differences in economic development or to acquiescence. Geographical differences in scale variances and mean levels were replicated, with Europeans and Americans generally scoring higher in Extraversion than Asians and Africans. Findings support the rough scalar equivalence of NEO-PI-R factors and facets across cultures, and suggest that aggregate personality profiles provide insight into cultural differences

    Saudi Expert Consensus-Based Autism Spectrum Disorder Statement: From Screening to Management

    Background: There is a large gap between the needs of individuals diagnosed with autism spectrum disorder (ASD) and the currently available services in Saudi Arabia. Services are often difficult to access, inconsistent in quality, incomplete, unsatisfactory, and costly. As such, there is a national need for expert consensus on the appropriate standards for the assessment and management of children on the autism spectrum. Methodology: A guideline development group (GDC) was formed by professionals representing all related specialties and institutions involved in the management of individuals on the autism spectrum in Saudi Arabia. They met on a regular basis over 21 months. The guideline development process consisted of five steps starting from reviewing existing guidelines and ending with discussing and writing this manuscript. A formal voting process was utilized and recommendations were discussed until a consensus was reached. Results: There was consensus on the following: A specialized diagnostic assessment needs to be carried out by an experienced multidisciplinary team for children referred to assess for ASD. They should be assessed for medical etiology, their behavioral history carefully reviewed, and symptoms directly observed. Longitudinal assessments are encouraged to reflect the effects of symptoms on the individual’s ability to function while with their family, among peers, and in school settings. An additional formal assessment of language, cognitive, and adaptive abilities as well as sensory status is essential to complete the diagnostic process. Interventions should be individualized, developmentally appropriate, and intensive, with performance data relevant to intervention goals to evaluate and adjust interventions. Target symptoms must be identified to address and develop monitoring systems to track change. Conclusion: ASD is a complex condition with widely varying clinical manifestations, thus requiring evaluation and intervention by a range of professionals working in coordination. Behavioral and environmental interventions are the key to optimal outcomes, in conjunction with medications when indicated for specific symptoms. Parental involvement in interventions is vital to sustaining therapeutic gains

    The strategic plan for combating antimicrobial resistance in Gulf Cooperation Council States

    Summary: The Gulf Cooperation Council Center for Infection Control (GCC-IC) has placed the emergence of antimicrobial resistance (AMR) on the top of its agenda for the past four years. The board members have developed the initial draft for the GCC strategic plan for combating AMR in 2014. The strategic plan stems from the WHO mandate to combat AMR at all levels. The need for engaging a large number of stakeholders has prompted the GCC-IC to engage a wider core of professionals in finalizing the plan. A multi-disciplinary group of more than 40 experts were then identified. And a workshop was conducted in Riyadh January 2015 and included, for the first time, representation of relevant ministries and agencies as well as international experts in the field. Participants worked over a period of two and a half days in different groups. International experts shared the global experiences and challenges in addressing human, food, animal, and environmental aspects of controlling AMR. Participants were then divided into 4 groups each to address the human, animal, microbiological and diagnostic, or the environmental aspect of AMR. At the end of the workshop, the strategic plan was revised and endorsed by all participants. The GCC-IC board members then approved it as the strategic plan for AMR. The document produced here is the first GCC strategic plan addressing AMR, which shall be adopted by GCC countries to develop country-based plans and related key performance indicators (KPIs). It is now the role of each country to identify the body that will be accountable for implementing the plan at the country level.
