    Transcendence over Diversity: black women in the academy

    Universities, like many major public institutions have embraced the notion of ‘diversity’ virtually uncritically- it is seen as a moral ‘good in itself’. But what happens to those who come to represent ‘diversity’- the black and minority ethnic groups targeted to increase the institutions thirst for global markets and aversion to accusations of institutional racism? Drawing on existing literature which analyses the process of marginalization in higher education, this paper explores the individual costs to black and female academic staff regardless of the discourse on diversity. However despite the exclusion of staff, black and minority ethnic women are also entering higher education in relatively large numbers as students. Such ‘grassroots’ educational urgency transcends the dominant discourse on diversity and challenges presumptions inherent in top down initiatives such as ‘widening participation’. Such a collective movement from the bottom up shows the importance of understanding black female agency when unpacking the complex dynamics of gendered and racialised exclusion. Black women’s desire for education and learning makes possible a reclaiming of higher education from creeping instrumentalism and reinstates it as a radical site of resistance and refutation

    Patient’s lack of understanding of disease hinders in proper control of disease thus increasing the morbidity and mortality. Review of the studies done in this region, dose not provide a detail account of patients understanding about the disease. Majority of patients when interviewed in clinics do not know the right answer to cause, level of desired sugar, possible complication. Material and methods: Hospital-based cross-sectional KAP study was conducted on diabetic patients visiting the diabetic clinic in two tertiary care Hospitals and a diabetic poly clinic in different areas of Karachi during the month of September 2012. Results: The mean age was 56 years, with 122 male and 118 female, 40.4 % patients were uneducated, 22.9% metric, 12.9% intermediate and 22.9% were graduate. Considering the knowledge, as to what is diabetes? only 46% answered correctly. What could cause the disease? 35% thought there was no reason for sugar. Regarding most frequent symptom, 27% thought there were no specific symptoms, 24.2% weight loss and 23.8% excessive urine and 21.7% thought not healing of wound was first indicator and 2.1 % thought that increase appetite is the cause. The desired levels of random sugar only 24.2% thought it be around 180 mg /dl while 32.5% had no idea about the value. Desired fasting sugar was correctly known by 34.6% as less than 100 mg/dl and 22.9% as less than 120 mg/dl. , Duration of treatment was believed to be life long treatment by 85%.Oral hypoglycemic were consumed by 68%, 24% were on insulin. And 6% were using both. 75% patients did not do exercise regularly. Regarding the harms created by diabetes, 51.3 % knew that heart or kidneys may get damaged. Regular doctor visits were done by 80% , and 85 % were satisfied with the level of care. The average sugar check cost was less than 100 rupees for 62.1 % . Conclusion: Lack of proper concepts regarding the disease, desired level of sugars, possible complications could be a big hinder in achieving good diabetic control. Thus by addressing the specific deficiencies in the knowledge and practices of patients, a better outcome may be achievable

    High CD44 immunoexpression correlates with poor overall survival: Assessing the role of cancer stem cell markers in oral squamous cell carcinoma patients from the high-risk population of Pakistan

    Oral squamous cell carcinoma (OSCC) is a top-ranked cancer in the Pakistani population, and patient survival has remained unchanged at ∌50% for several decades. Recent advances have claimed that a subset of tumour cells, called cancer stem cells (CSCs), are responsible for tumour progression, treatment resistance, and metastasis, which leads to a poor prognosis. This study investigated the impact of CSC markers expression on overall survival (OS) and disease-free survival (DFS) of OSCC patients. Materials and Methods. Immunohistochemistry was used to evaluate CD44, CD133, L1CAM, and SOX2 expression in a well-characterized cohort of 100 Pakistani patients with primary treatment naĂŻve OSCC. The immunoreactivity for each marker was correlated with patient clinicopathologic characteristics, oral cancer risk chewing habits, and survival. The minimum follow-up time for all patients was five years, and survival estimates were calculated using the Kaplan-Meier method and Cox proportional hazards model. Results. In this cohort of 100 patients, there were 57 males and 43 females. The median OS and DFS time durations observed were 64 and 52.5 months, respectively. Positive expression for CD44, CD133, L1CAM, and SOX2 was observed in 33%, 23%, 41%, and 63% of patients. High CD44 expression correlated with decreased OS (P=0.047) but did not influence DFS. However, CD133, L1CAM, and SOX2 had no effect on either OS or DFS. Tonsils, nodal involvement, and AJCC stage were independent predictors of worse OS and DFS both. Conclusion. Of the CSC markers investigated here, only CD44 was a predictor for poor OS. CD44 was also associated with advanced AJCC and T stages. Interestingly, CD133 was significantly lower in patients who habitually consumed oral cancer risk factors

    Prognosis of Liver Disorders in Dna Positive Hbv Patients Based on Fuzzy Soft Sets

    Liver disease and disorders are serious public health burdens because of the high prevalence among populations worldwide and poor long-term clinical outcome. The outcomes of the disease include deaths from liver decompensation, cirrhosis and HCC. Many liver diseases, including chronic HBV and HCV infection, ALD, NAFLD, autoimmune liver disease and drug-induced liver disease (DILI), potentially threaten a large proportion of the global population. The fuzzy soft set principle theory has been used for the developing of a diagnostic system in medicine and devise a prediction system named as fuzzy soft expert system which is a rule-based system uses fuzzy set and fuzzy soft set. There are five main components included in the basic structure, they are: (1) A fuzzication that translates the inputs (real-values) into fuzzy values, (2) obtaining fuzzy sets, (3) changing in to fuzzy soft sets, (4) reduction of normal parameter of fuzzy soft sets, (5) output data by algorithm. Fifty two individuals suspected and managed as HBV patients were involved in this study.All of them were attending liver diseases unit at Azadi teaching hospital in Duhok, Kurdistan Region-Iraq. They were being managed by the herpetology specialist as HBV infected patients. Their parameters (Alanine Aminotransferase (ALT), Aspartate aminotransferase (AST), Total Serum Albumin (Alb.), and Total Serum Bilirubin (T.S.Bil.)), were used as input data and the score of each patient was calculated. The developed fuzzy soft expert system was used to obtain the score for each as prognostic model for liver disorders. The score of 10 of those patients are selected and compared with the clinical status of each base on signs and symptoms of the HBV infection. Score more than 101.844 was considered to be highly linked with HBV infection. Scores less than 101.844 was considered to be not related to HBV infection

    Microstructure, tensile and fatigue properties of ultrasonic spot welded aluminum to galvanized high-strength-low-alloy and low-carbon steel sheets

    The microstructure evolution, tensile lap shear strength and fatigue properties of dissimilar ultrasonic spot welded (USWed) joints of aluminum to two commercial steel sheets at different welding energies were investigated. The main intermetallics at the weld interface were Ξ (FeAl3) in both joints along with eutectic Al-Zn in Al-to-galvanized high-strength-low-alloy (HSLA) steel joints and Fe3Al in Al-to-ASTM A36 steel joints. The welding strengths of both joints were higher than those of other dissimilar joints reported in the literature. With increasing welding energy, the maximum tensile lap shear strength increased in the Al-to-galvanized HSLA steel joints, while the lap shear strength increased up to a peak value and then decreased in the Al-to-ASTM A36 steel joints. Both the average peak welding strength and fracture energy of the Al-to-galvanized HSLA steel joints were higher than those of the Al-to-ASTM A36 steel joints. The fatigue lives of both welded joints were in agreement with or somewhat longer than other Al-to-steel USWed joints in the literature. The fatigue fracture mode changed with increasing cyclic loads in both welded joints. Fatigue crack growth was mainly characterized by the formation of fatigue striations perpendicular to the fatigue crack propagation direction

    Effect of welding energy on microstructure and strength of ultrasonic spot welded dissimilar joints of aluminum to steel sheets

    Two dissimilar ultrasonic spot welded joints of aluminum to commercial steel sheets at different levels of welding energy were investigated. The tensile lap shear tests were conducted to evaluate the failure strength in relation to microstructural changes. The main intermetallics at the weld interface in both joints was ξ (FeAl3), along with ɳ (Fe2Al5) phase in Al-to-AISI 304 stainless steel joint and Fe3Al phase in Al-to-ASTM A36 steel joint, respectively. The welding strength of Al-to-AISI 304 stainless steel weld samples was slightly higher than Al-to-ASTM A36 steel weld samples, whereas the fracture energies of Al-to-AISI 304 stainless steel weld samples were significantly higher as compared with Al-to-ASTM A36 steel weld samples. The welding strength of both Al-to-Steel welds were higher than other reported dissimilar USW joints in literature. The fracture surfaces of both weld joints exhibits the growth of IMC layer with increasing welding energy or time, whose inherent brittleness compromises the integrity of joints. In both cases, the lap shear tensile fracture occurred from the Al/Fe interface at lower energy inputs and the failure mode at higher welding energy inputs became the “transverse through-thickness crack growth” at the edge of the nugget zone on the softer Al side

    Outcome of tetanus patients admitted in Rangpur Medical College Hospital

    Background: Tetanus remains one of the major public health hazards of the developing world. Mortality is much lower in the developed world because of the availability of facilities, unlike in most developing countries' Objectives: This study was aimed to determine the outcome of tetanus patients admitted in infectious disease unit of Rangpur Medical College Hospital. Methods: A total of 91 cases of tetanus patients were seen in the 18 month period with a mean of 5 cases per month; they are diagnosed and managed for tetanus in the medical wards from January 2011 to June 2012. The data were retrieved from their case records and analyzed. Results: There were thirty one deaths, accounting for an overall mortality of 34.16o.Total fifty six patients were cured among which sixteen (17.67%) were cured with complication and four (4.4%) patient were absconded. Mortality was high 45.84% (11 patients out of 24) ?40 years age, whereas low 29.85 (20 patient out of 87) in <40 yea15 age (45.84% vs 29.85%). Mortality rate was also higher among female than male patients (34.66% vs 33.85%). Farmers experienced more death than non-farmer (4l.l8% vs 29-82%, P<0.05). Mortality was higher in patients who had not received any medical treatment for their wound than in patients who had received it for their wound (52.78% vs 21.81%, p<0.05). Patients with short incubation period of less than one week had higher mortality in comparison with those who had incubation period more than one week (53.33% vs 23.25%, P<0'05). Of the 91 patients, fifty six (64.34%) were alive, though four remained in a persistent vegetative state due to tetanus toxin-induced brain damage and another two required a below knee amputation of the left leg. Hence, 40 were discharged well and 16 were discharged with permanent disabilities. Conclusion: The case fatality rate of tetanus has remained consistently high in the medical college. Factors that were significantly associated with high mortality included older age, and incubation period of less than 7 days. It is recommended that preventive immunization against tetanus be given to all Bangladeshis with secondary vaccination at adulthood

    A comprehensive first principles calculations on (Ba0.82K0.18)(Bi0.53Pb0.47)O3 single-cubic-perovskite superconductor

    In this present study, the pseudopotential plane-wave (PP-PW) pathway in the scheme of density functional theory (DFT) is utilized to investigate the various physical properties on (Ba0.82K0.18)(Bi0.53Pb0.47)O3 (BKBPO) single perovskite superconductor. We have analyzed elastic constants and moduli at zero and elevated pressures (up to 25 GPa) as well. We also have investigated the anisotropic nature incorporating both the theoretical indices and graphical representations in 2D and 3D dimensions, which reveals a high level of anisotropy. The flatness of the energy bands near EF is a sign of Van-Hf singularity that might increase the electron pairing and origination of high-TC superconductivity. The computed band structure exhibits its metallic characteristics is confirmed by band overlapping. A band of DOS is formed for the strong hybridization of the constituent elements. The orbital electrons of O-2p contribute most dominantly at EF in contrast to all orbital electrons. The orbital electrons at the EF are higher from both the partial density of states and charge density mapping investigation. The coexistence of the electron and hole-like Fermi sheets exhibits the multi-band nature of BKBPO. On the other hand, Fermi surfaces with flat faces promote transport features and Fermi surface nesting as well. The calculated value of the electron-phonon coupling constant ({\lambda} = 1.46) is slightly lower than the isostructural superconductor, which indicates that the studied BKBPO can be treated as a strongly coupled superconductor similar to the reported isostructural perovskite superconductors. Furthermore, the thermodynamic properties have been evaluated and analyzed at elevated temperature and pressure by using harmonic Debye approximation (QHDA).Comment: 20 pages, 7 figures, 6 table

    Religion and religious education : comparing and contrasting pupils’ and teachers’ views in an English school

    This publication builds on and develops the English findings of the qualitative study of European teenagers’ perspectives on religion and religious education (Knauth et al. 2008), part of ‘Religion in Education: A contribution to dialogue or a factor of conflict in transforming societies of European countries?’ (REDCo) project. It uses data gathered from 27 pupils, aged 15-16, from a school in a multicultural Northern town in England and compares those findings with data gathered from ten teachers in the humanities faculty of the same school, collected during research for the Warwick REDCo Community of Practice. Comparisons are drawn between the teachers’ and their pupils’ attitudes and values using the same structure as the European study: personal views and experiences of religion, the social dimension of religion, and religious education in school. The discussion offers an analysis of the similarities and differences in worldviews and beliefs which emerged. These include religious commitment/observance differences between the mainly Muslim-heritage pupils and their mainly non-practising Christian-heritage teachers. The research should inform the ways in which the statutory duties to promote community cohesion and equalities can be implemented in schools. It should also facilitate intercultural and interreligious understanding between teachers and the pupils from different ethnic and religious backgrounds
