    Introducción. Este es un trabajo descriptivo retrospectivo, en el cual se analizaron las historias clínicas de los pacientes que ingresaron al hospital Susana López de Valencia, Popayán, con diagnostico de intento suicida durante el año 2005. Objetivo: Determinar los factores de riesgo asociados al intento suicida Métodos: Se trabajó finalmente con 158 casos analizándose variables socio demográficas, datos relacionados con el intento, evolución, seguimiento y calidad de la atención. Resultados: Encontramos que las variables asociadas con peor pronóstico para intento suicida fueron: ideación suicida persistente luego de la atención inicial y la existencia de previos intentos suicidas, estas dos variables representaron un riesgo relativo de complicaciones entre 3 y 9 veces mayor. Conclusiones: Debe realizarse un formato para consignar en este, de manera sistemática, las variables que determinen incremento del riesgo suicida como ideas suicidas, premeditación del intento e intentos suicidas previos

    Patient safety is an essential component of quality in health care. After understanding and awareness of the medical act is subject to the occurrence of errors, in recent decades, more attention has been paid to the processes related to the culture of patient safety, particularly in sensitive areas as perioperative care. Implementing measures of improvement begins with a self-assessment of "safety climate" which it represents the measurable component of the "safety culture". Analysis can identify strengths and weaknesses that shape the way health care professionals think, act and focus their attention. Different instruments assess the safety climate, each one with strengths and limitations. In this reflection, the basics of perioperative safety and instruments commonly used for analysis are reviewed, while the situation of health care system in Colombia is approached

Tras la comprensión y conciencia de que el acto médico está sujeto a la ocurrencia de errores, en las últimas décadas se ha prestado mayor atención a los procesos relacionados a la cultura de seguridad del paciente, principalmente en áreas susceptibles como el cuidado perioperatorio. La implementación de medidas de mejoramiento empieza con una autoevaluación del “clima de seguridad”, el cual representa el componente medible de  la “cultura de seguridad”. Su análisis puede identificar fortalezas y debilidades que configuran la forma en la que los profesionales de la salud piensan, actúan y enfocan su atención. Existen diferentes instrumentos que evalúan el clima de seguridad, cada uno con limitaciones y fortalezas. En esta reflexión, se revisan los conceptos básicos de seguridad perioperatoria y los instrumentos utilizados comúnmente para su análisis, al tiempo que se aborda la situación de seguridad en salud en Colombia
