    Adolescent Violence: Proneness Factors of Victims

    Introduction: Approximately three-quarters of adolescents experience some type of victimization, either in school or as a whole in society. Their mental acumen is not strong enough due to various reasons to resolve this age-related crisis and pushing them in a web of stress. The attitude of victim is influenced by his self-social image, personality and behavior which provide the space to the perpetrator of the offence. Materials and Methods: A cross-sectional survey, using a pretested self-report questionnaire was conducted on 960 students aged 10–18 years of urban schools regarding the indulgence in violence as victims during the last one year.Observations: The prevalence of victimization was 18.33% while in male and female it was 59.09% and 40.90% in their respective groups. Parents possessing education less than high school produce the highest numbers of victims. 65.34% victims were staying with both parents and 19.88%, 63.63%, 55.11% and 35.79% were in habit of smoking, alcohol and tobacco use respectively.Discussion and Conclusion: Research studies have shown that the behavior can be modified and the proneness factors can be reduced or eliminated. Every child’s individuality should be appreciated for the value, rather than suppressed to reduce the risk of victimization. Not all children are able to alter their personal characteristics that may place them at increased risk. To benefit the society, the segregation and counselling of such students at school level is the only answer of this problem

    Genexpert test and endometrial histological findings in infertile women

    Background: Infertility is a worldwide health problem with very definite physiological, psychological and social implications. According to studies, about 30% of infertility cases are due to the male factors, 40 to 50% have the origin of female factors and 20 to 30% of cases are caused by both male and female factors. Genital tuberculosis is among the main causes of female infertility. Due to its high prevalence, especially in developing countries, work-up of an infertile couple should include evaluation for tuberculosis irrespective of history or exposure. The objective of the present study is to find out the prevalence of genital tuberculosis in all females presenting with infertility in a tertiary care hospital over a given period of time, and diagnostic comparison of endometrial tuberculosis by GeneXpert and Histopathological examination (HPE).Methods: The prospective observational study was conducted from January 2016-June 2017.A total of 81 endometrial tissue samples were collected from the women, satisfying the inclusion and exclusion criteria.Results: GeneXpert scored negative in our study. On HPE, out of 81 patients, proliferative endometrium (anovulatory) was found in 34 cases (41.9%), non-specific endometritis in 1 case (1.23%) and 1 case (1.23%) was found positive for tubercular endometritis. Secretory endometrium was found in 45 cases (55.55%).Conclusions: Endometrial biopsy shows not only the tuberculous endometritis, but also gives additional information about hormone response of endometrium, local factors of endometrium concerning non-specific and specific infections and anovulatory cycles. Gene Expert if positive on endometrial biopsy is a reliable test for FGTB and treatment can be started on its basis

    Identification of factors determining the initiation of breastfeeding in postnatal women in a tertiary care centre: an observational study

    Background: The WHO recommends that mothers worldwide to initiate breastfeeding within an hour of delivery to achieve optimal growth, development and health but the breastfeeding rates remain far below the recommended standards. The aim of this study was to find the prevalence of delay in initiation of breast feeding, to identify various factors which determine the initiation of breastfeeding and to correlate their association with delayed onset of breast feeding.Methods: Hundred and thirty six postnatal women, who delivered in Santosh Medical College and Hospital, Ghaziabad, were recruited in the study. They were interviewed after their informed consent using a pretested structured questionnaire. Socio-demographic and obstetric factors were compared between subjects who initiated breast feeding in less than and more than 1 hours. Frequencies were calculated for different variables. Data was statistically analyzed and p value of <0.05 was taken significant.Results: The prevalence of delay in initiation of breast feeding in our study was found to be 74.2% (n=101).The mean time of initiation of breast feeding was 18.43 hours. Significant association (p <0.05) was found in various sociodemographic, obstetrical and social factors.Conclusions: Identifying mothers at risk of delayed breastfeeding initiation should be the target for breastfeeding promotion during prenatal, antenatal as well as postnatal period. Public health officials and health care providers should consider interventions to promote and support early initiation of breast feeding

    Introduction: The shift from the physical to virtual communication has been emerged as a vital platform to interact through technology. Apart from the positive influences, it is also affecting the adolescents in negative manner which leads to various health hazards. All governments are committed to the ambitious sustainable development agenda and its goals, in particular SDG target 16.2 to end abuse, exploitation, trafficking and all forms of violence against and torture of children. Despite the alarming situation and commitment, there is apathy towards coining the specific laws against cybercrime amongst adolescents.Methodology: A questionnaire based cross-sectional survey method was used for data collection from the adolescents who are using mobile phones since last one year and presented in outdoor pediatric department. The study was also conducted in two schools from urban and rural locations at Lucknow district during study period.Results: In the studied population of 900 students, the maximum students 606(67.3%) were from 16-18 years of age and simple mobile users were more 505(56.1%) in number when compared. the prevalence of bullied students were females 269(29.8%) and males 381(42.3%). Maximum bullied students were of age 16-18 years. As per the mode of assault sending inappropriate SMS 378 (58.1%) on mobile was outrageous than any other method. The assault was shared mainly with friends.Conclusion: Our results reflect that the prevalence of violence amongst adolescents have reached up to a point of saturation where it is mandatory to correct their attitude to avoid any unwanted happening. Apart from the surveillance of the mobile use this is our prime duty to monitor the activities of teens on internet and develop safe cyber space

    Rasashastra (Vedic chemistry) is the root branch of the Ayurveda (the science of life) that deals with herbomineral preparations. Abhraka (mica) is a mineral that classified in Maharasa group of Rasashastra. It contains several elements such as Si, Fe, Al, Mg, Na, and K as main ingredients. Four types of abhrakas are described in the Classics of Rasashastra including pinaka. Property of Pinaka is told that when it is heated on fire, the layers get separates. Intake of its bhasma causes severe constipation. The characteristic effect of pinak abhraka can be correlated with muscovite – paragonite micas. Because their perfect basal cleavage allows them to be split into thin, flexible sheets. Muscovite–paragonite series group of mica having hydrous potassium, sodium, aluminum, and silicate minerals. In this hydrous group, aluminum hydroxide causes constipation. In this paper, an attempt will be made to explain how does pinaka abhrak causes severe constipation

    Pulmonary alveolar microlithiasis: Incidental finding - should we Ignore?

    Pulmonary alveolar microlithiasis (PAM) is a rare entity, presenting mostly as an incidental finding. This disease has an autosomal recessive inheritance with inactivating mutations in the gene “solute carrier family 34 member 2”. The present study was conducted to bring attention to this rare though preventable disease. The study was a cross-sectional descriptive study, conducted at the Department of Pathology, of a tertiary care hospital in New Dehli-India. PAMs were incidentally seen in two patients diagnosed with micronodular hepatic cirrhosis leading to reanalysis of 212 autopsies, retrospectively. Statistical analysis was done using Stata 14.0. We observed three forms (Type A, B and C) of round hyaline bodies measuring in 200-800µm diameter with thin delicate, radiating fibrils. These bodies were PAS positive, showed black discolouration of the pigment with von Kossa stain and birefringence on polarized microscopy using Congo red stain, however the refringence was light green as compared to apple green birefringence seen with amyloid deposition. PAM has a slow progressive course leading to a high rate of incidental detection. Drugs known to inhibit the micro-crystal growth of hydroxyapatite may slow the disease progression. The family members of patients with PAM may also be kept on follow up with regular imaging. Key messages: It is important to bring out the incidental finding as, seemingly innocuous observations may provide valuable insight into incurable diseases, especially rare diseases

    Antenatal counselling as a tool to increase acceptability of postpartum intrauterine contraceptive device insertion in a tertiary care hospital

    Background: Acceptance of postpartum intrauterine device (IUD) insertions in women in Uttar Pradesh is low. In response to the concept that a good postpartum programme should begin prenatally, this study was designed to determine whether the provision of expert contraceptive counselling during the antenatal period would have an impact on its uptake as compared to women who were counselled in post partum period.Methods: Expert contraceptive counselling was given to 264 and154 women during their antenatal visits and postpartum hospitalization, respectively. Acceptors received IUCDs within 48 h of delivery in the case of vaginal delivery or transcesarean insertion in case of abdominal delivery. The acceptance rate of PPIUCD and the percentage of actual insertions were recorded. The causes of refusal were also recorded.Results: Among the 264 couples counselled in antenatal period 116 (43.9%) had PPIUCD insertion. Among the 154 unbooked women who were counselled postpartum only PPIUCD was inserted in 36 (23.4%) women. PPIUCD insertion was significantly high in women receiving expert counselling during antenatal period as compared to the women who were counselled in postnatal period (p< 0.001). Family refusal, No knowledge of PPIUCD and preference for another contraceptive methods were the most common reasons for refusing the use of PPIUCD.Conclusion: Acceptability of PPIUCD insertion was high in women counselled in antenatal period. Hence, it is suggested that counselling for PPIUCD should start in antenatal period
