Placenta accreta diagnosed 15 days following primary cesarean section
Placenta accreta is a life-threatening obstetric complication with an ever-increasing incidence. Between 1982 and 2002, the reported incidence of placenta accreta was 1/533 pregnancies, nearly 4 times its incidence in the 1980s and 8 times its incidence in the 1970s. As Cesarean sections (C/S) become more common, so does placenta accreta, as prior C/S is a risk factor. Placenta accreta requires emergent treatment and unique anesthetic considerations. However, little research discusses the anesthetic protocol to treat this condition. This report details the anesthetic procedure used to successfully treat a patient with placenta accrete diagnosed 15 days post-C/S
Haemorrhage during cesarean section for parturient with antiphospholipid syndrome
This case describes a 39-year-old G3P2 parturient with a history of the antiphospholipid syndrome (APS), who experienced severe hemorrhage during her cesarean section (CS) delivery of twins. At 36 weeks gestation, the patient was being treated prophylactically with Lovenox and acetylsalicyclic acid. In preparation for delivery, her medications were discontinued 24 h prior to admission. Due to breech presentation, the patient required delivery by CS. The patient received epidural anesthesia and successfully delivered two healthy babies. Following delivery, the patient became hypotensive and unresponsive and experienced uterine atony with profuse bleeding. Based on the patient′s clinical symptoms and history of APS, hemorrhage was suspected. Airway patency was immediately established using rapid sequence intubation, and the patient was placed under general anesthesia for removal of her atonic uterus. Following massive fluid resuscitation and correction of her coagulopathy, the patient stabilized and was transferred to the surgical intensive care unit. Four days later, she was discharged from the hospital without further complications
Scale and Drivers of Female Agricultural Labor: Evidence from Pakistan
Agricultural labor is largely informal, particularly for female agricultural labor in developing countries. Despite significant participation in the agricultural labor force in Pakistan, women’s contribution is not properly acknowledged and rewarded. The issue is further aggravated by the dearth of literature on gender–labor relations in cropping and livestock activities. Considering this gap in the literature, the current study was conducted with the specific objective of exploring the labor composition of different agricultural activities in different farm size categories in general and, particularly, female agricultural labor (family and hired labor) participation and its determinants in the rice–wheat cropping system of the Punjab province, Pakistan. The data were collected from 300 households across four districts of the province. Labor participation was calculated on an official farm size classification basis, i.e., small (<12.5 acres), medium (12.6–25 acres) and large (>25 acres) farms. The findings show that female labor is predominantly demanded in the manual harvesting of wheat, rice nursery transplantation and harvesting, and the majority of the livestock-related activities. The regression model results showed that family female labor and hired female labor participation significantly depend on the landholding status of farmers, household size, family type and level of education. The interviews also illustrated that labor relations are rapidly changing—ongoing mechanization threatens conventional female labor activities due to the lack of machinery operation skills among females, caused by informal state policies and cultural barriers. The findings of the study have important policy implications for mainstreaming gender status in agricultural policy and rural development and contribute directly to the Sustainable Development Goals on Gender Equality (SDG#5) and Decent Work and Economic Growth (SDG#8), and indirectly to No Poverty (SDG#1), Zero Hunger (SDG#2), Responsible Consumption and Production (SDG#12) and Climate Action (SDG#13)
Experience with the quantitative lytA gene real-time polymerase chain reaction for the detection of Streptococcus pneumoniae from pediatric whole blood in Pakistan
Background: We present our experience with optimization and diagnostic use of quantitative real-time polymerase chain reaction (PCR) targeting the lytA gene of Streptococcus pneumoniae for the detection of S. pneumoniae in whole blood of children <5 years of age. The assay was optimized to detect ≥5 CFU/10 μl or 1 copy of DNA/2 μl of blood. Methods: This assay was applied on 1912 whole blood specimens collected from children <5 years of age with pneumonia, of which 35 specimens were lytA positive. The bacterial loads were determined through categorization of load into five different categories, i.e., very high load, high load, moderate load, low load, and very low load. Results: Of the 35 lytA-positive samples, 9 (25.71%), 4 (11.42%), 1 (2.85%), 13 (37.14%), and 8 (22.85%) were categorized as very high load, high load, moderate load, low load, and very low load, respectively. Extracted samples were also subjected to serotyping by the Centers for Disease Control and Prevention PCR scheme. Positive samples with very high load and high load category were serotyped successfully in all instances. A high proportion of samples with low and very low load (61.53% and 75%, respectively) remained untypeable by the currently proposed schemes. Conclusions: LytA PCR assay in whole blood provides rapid and sensitive results for the diagnosis of invasive S. pneumoniae disease in a resource-limited setting, while also being amenable to quantitation and serotyping
Ropivacaine 0.025% mixed with fentanyl 3.0 μg/ml and epinephrine 0.5 μg/ml is effective for epidural patient-controlled analgesia after cesarean section
Background and Aims: We aimed to determine the ropivacaine concentration that provided adequate analgesia with early ambulation and minimal urinary retention or other side-effects when used with fentanyl and epinephrine for patient-controlled epidural analgesia (PCEA) after elective cesarean section.
Material and Methods: Forty-eight patients were randomized to four groups in a double-blinded fashion. All groups received an initial 10 ml/h of epidural study solution for 24 h. The solution contained: 0.2, 0.1, 0.05, or 0.025% ropivacaine for Groups I-IV, respectively, with fentanyl 3.0 μg/ml and epinephrine 0.5 μg/ml. Patients could administer additional PCEA doses of 4 ml of their study solution with a lock-out time of 10 min. Overall satisfaction, side-effects, motor block, neurologic function, and pain using Visual Analog Scale were assessed.
Results: Patients in all groups showed no difference in sedation, pruritus, nausea, vomiting, and uterine cramps. Pain scores at rest were lower for Group IV than Groups I-III (P < 0.001). Twelve, five, one, and zero patients could not ambulate in Groups I-IV, respectively. Nine, nine, two, and zero (III <I and II, P = 0.02; IV P = 0.001) patients reported urinary retention in Groups I-IV, respectively. Overall satisfaction scores were high for all groups. Neonatal behavior score was similar and high in all groups.
Conclusion: 0.025% ropivacaine PCEA combined with fentanyl and epinephrine provided effective pain relief after cesarean section with early ambulation and without sensory loss, urinary retention, or increase of side-effects
