Makrolidy — nie tylko działanie przeciwbakteryjne
The article discusses the distribution, application, mechanism of action and the differencesbetween individual molecules of antibiotics belonging to the macrolides. Indicated theirdifferent antimicrobial activity and the precautions necessary for their application. Quoted a study evaluating the use of macrolides (mainly azithromycin) in chronic inflammatorypulmonary diseases to prevent exacerbations. In one of them on chronic obstructive pulmonarydisease (COPD) receiving azithromycin for one year at a dose of 1 × 250 mg was associated with a lower incidence of exacerbations, longer time to first exacerbation and improved quality of life. Also in patients with cystic fibrosis, as well as bronchiectasis, shown above effects (erythromycin and azithromycin). Attention was also noted that the standard applied a three-day treatment with azithromycin, however, requires time extension.W artykule omówiono podział, zastosowanie, mechanizm działania oraz różnice międzyposzczególnymi cząsteczkami antybiotyków zaliczanych do grupy makrolidów. Wskazano ichróżną aktywność przeciwbakteryjną oraz środki ostrożności konieczne przy ich stosowaniu.Przytoczono też badania oceniające zastosowanie makrolidów (głównie azytromycyny)w przewlekłych chorobach zapalnych płuc celem zapobiegania zaostrzeniom. W jednymz nich dotyczącym przewlekłej obturacyjnej choroby płuc (POChP) przyjmowanie przez rokazytromycyny w dawce 1 × 250 mg wiązało się z mniejszą częstością zaostrzeń, dłuższymczasem do wystąpienia pierwszego zaostrzenia i poprawą jakości życia. Także u pacjentówz mukowiscydozą, jak również z rozstrzeniami oskrzeli, wykazano powyższe efekty (erytromycyna i azytromycyna). The impact of comorbidities on tuberculosis treatment outcomes in Poland: a national cohort study
BackgroundTuberculosis (TB) is a complex disease associated with other medical conditions, that may affect disease severity. This study aimed to investigate the impact of comorbidities on treatment outcomes and mortality rates in patients with TB in Poland.MethodsWe analyzed a national cohort of 19,217 adult TB patients diagnosed between 2011 and 2016 in Poland. We compared treatment success rates and mortality rates in patients with comorbidities and those without to assess the impact of various comorbidities on these outcomes. Odds ratios (OR) were calculated to quantify the association between comorbidities and TB treatment outcomes.ResultsPatients with comorbidities had lower treatment success rates and higher mortality rates. Diabetes was identified as a significant risk factor for increased TB mortality (OR = 1.9) and mortality from all other causes (OR = 4.5). Similar associations were found for alcoholism (OR = 8.3 and OR = 7.1), immunosuppressive therapy (OR = 5.7 and OR = 5.9), and cancer (OR = 3.4 and OR = 15.4). HIV and tobacco use were associated with an increased risk of mortality from causes other than TB, with odds ratios of 28.6 and 2.2, respectively. The overall treatment success rate in the study population was 88.0%, with 9.2% of patients failing to achieve treatment success and 2.8% dying. Comorbidities such as diabetes, alcoholism, substance addiction, immunosuppressive therapy, cancer, and tobacco use increased the risk of tuberculosis treatment failure.ConclusionPatients with comorbidities face a higher risk of unsuccessful treatment outcomes and increased mortality. It is essential to implement integrated management strategies that address both TB and comorbid conditions to improve treatment success rates and reduce mortality
Impact of mild anaemia on dyspnoea during exertion and exercise tolerance in patients with acute exacerbation of chronic obstructive pulmonary disease
WSTĘP: Duszność i zmniejszenie tolerancji wysiłku są objawami zaostrzenia przewlekłej obturacyjnej choroby płuc (POChP). Niedokrwistość stanowi czynnik ryzyka obniżonej tolerancji wysiłku i duszności w stabilnej POChP. Mało jest danych na temat wpływu niedokrwistości na tolerancję wysiłku i duszność u chorych z zaostrzeniem POChP. Celem pracy była ocena wpływu obniżonego stężenia hemoglobiny we krwi na wyniki 6-minutowego testu chodu (6MWT) u pacjentów w trakcie zaostrzenia POChP.MATERIAŁ I METODY: Retrospektywna analiza danych zebranych w trakcie badania u chorych z zaostrzeniem POChP. Do oceny wykorzystano pierwsze dostępne oznaczenia stężenia hemoglobiny, wykonane w trakcie hospitalizacji. 6MWT wykonano po uzyskaniu poprawy klinicznej pacjenta. Nasilenie duszności oceniano przed oraz po 6MWT. W trakcie 6MWT mierzono saturację krwi tlenem (SpO2).WYNIKI: Zbadano 402 pacjentów z zaostrzeniem POChP (średnie stadium wg PTChP 3,5 ± 0,6). Chorzy z niedokrwistością (26% zbadanych, wiek średni 74,5 ± 8,2 roku) w teście chodu pokonali 258,1 ± 125,1 m, z desaturacją wysiłkową na poziomie 2,9 ± 2,6%. Chorzy bez niedokrwistości (74% zbadanych, wiek średni 70,2 ± 8,7 roku) w teście chodu pokonali 271 ± 136 m, z desaturacją wysiłkową na poziomie 3,8 ± 3,7%. Stężenie hemoglobiny nie korelowało z 6MWT, dusznością po 6MWT, saturacją w trakcie wysiłku i desaturacją krwi po wysiłku.WNIOSKI: Łagodnie obniżone stężenie hemoglobiny we krwi nie wpływało na wyniki 6MWT u pacjentów z zaostrzeniem POChP. INTRODUCTION: Dyspnoea and decreased exercise tolerance are symptoms of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Anaemia is a risk factor for reduced functional capacity and dyspnoea in stable COPD. There is limited information about the impact of anaemia on functional capacity and dyspnoea of patients during AECOPD. The aim of this study was to evaluate the impact of decreased blood haemoglobin concentration on the results of six-minute walking test (6MWT) in patients during AECOPD.MATERIAL AND METHODS: A post hoc analysis of data collected from prospective long-term studies on AECOPD. Haemoglobin concentration from the first obtainable hospital measurement were included in the assessment. 6MWT was performed after clinical improvement of the patient. Dyspnoea at baseline and after exercise and oxygen saturation (SpO2) during exercise was measured.RESULTS: (presented as means ± SD): 402 patients with exacerbation of COPD (COPD stage 3.5 ± 0.6) were examined. Patients with anaemia (26% of those studied, age 74.5 ± 8.2 years) achieved 258.1 ± 125.1 m during 6MWT, with exertional desaturation of 2.9 ± 2.6%. Patients without anaemia (74% of those studied, age 70.2 ± 8.7 years) achieved 271 ± 136.0 m during 6MWT with exertional desaturation of 3.8 ± 3.7%. The haemoglobin concentration did not correlate with 6MWT, dyspnoea during 6MWT, or exercise oxygenation and blood desaturation during exercise.CONCLUSION: Mildly decreased blood haemoglobin concentration did not influence the results of 6MWT in patients with AECOPD.
Does bronchiectasis affect COPD comorbidities?
Introduction: COPD and bronchiectasis, chronic inflammation disorders of the bronchial tree through the mechanism of 'spill-over' of inflammatory mediators, may lead to systemic manifestations of illness of the respiratory system and comorbidities. The aim of the study was to evaluate the frequency of coexisting chronic obstructive pulmonary disease and bronchiectasis and influence of bronchiectasis on COPD comorbid diseases. Material and methods: A post-hoc cross-sectional analysis of cohort study of 288 consecutive patients hospitalized due to acute exacerbation of COPD was performed. Results: 177 males (61.5%) and 111 females (38.5%) with mean age = 71.0 8 ± 8.9 yrs, FEV1 % pred. = 34.6 ± 16.8 with COPD diagnosis were studied. In this group, 29 (10.1%) patients presented with bronchiectasis confirmed by HRCT scan. COPD patients with and without bronchiectasis had similar Charlson index results (2.5 vs 2.1, p=0.05). COPD patients with bronchiectasis required longer hospitalization during exacerbation. COPD patients with bronchiectasis significantly more often than patients without this comorbidity revealed the features of colonization with P. aeruginosa (OR = 4.17, p = 0.02). Conclusions: Bronchiectasis is a relatively common comorbidity in COPD patients. COPD patients with bronchiectasis are more frequently colonized with P. aruginosa comparing to non-bronchiectasis COPD patients. Influence of treatment with continuous positive airway pressure on respiratory muscle function and physical fitness in patients with obstructive sleep apnoea and overlap syndrome
Wstęp: Celem badania była ocena wpływu leczenia dodatnim ciśnieniem w drogach oddechowych (CPAP) na czynność
mięśni oddechowych oraz wydolność fizyczną u chorych na obturacyjny bezdech senny (OBS) i zespół nakładania (ZN) (OBS
i POChP). Materiał i metody: Zbadano 9 chorych z OBS oraz 9 pacjentów z ZN. Przed zastosowaniem leczenia za pomocą CPAP oraz
po 6 miesiącach terapii wykonywano badania maksymalnych ciśnień oddechowych, badania czynnościowe układu oddechowego
oraz oceniano wydolność fizyczną pacjentów.
Wyniki: W grupie OBS nie stwierdzono poprawy tolerancji wysiłku pod wpływem terapii. Dystans w 6MWD wynosił
571,8 ± 76,6 m przed i 554,0 ± 125,5 m po leczeniu, a Wmax 142 ± 41 W i 139 ± 38 W odpowiednio przed i po leczeniu.
Maksymalne ciśnienie wdechowe (PImax) w grupie OBS nie zmieniło się istotnie: 140,4 ± 32,0 cm H2O przed i 155,9 ± 31,5
po leczeniu (p = 0,14). Maksymalne ciśnienie wydechowe (PEmax) wynosiło 170,5 ± 49,2 cm H2O przed i 199,9 ± 27,6 cm
H2O po terapii (p = 0,067). Siła uścisku mięśni rąk u chorych z grupy OBS wzrosła z 50,5 ± 16,5 kg przed do 61,0 ± 17,0 kg
po leczeniu (lewa ręka) (p = 0,05) i z 53,3 ± 14,2 do 58,9 ± 15,9 (prawa ręka) (p < 0,05). W grupie ZN leczenie poprawiło
tolerancję wysiłku o 17%. Wmax zwiększyło się z 81 ± 33 W przed do 95 ± 38 W po leczeniu. Nie zmienił się natomiast
dystans w 6MWD (504 ± 144 m przed i 492 ± 108 m po leczeniu). Siła mięśni oddechowych u chorych z grupy ZN po
leczeniu CPAP wykazywała tendencję do poprawy. PImax wzrosło z 89,2 ± 35,7 cm H2O przed do 106,3 ± 31,4 cm H2O po
terapii (p < 0,05). PEmax wynosiło 159,9 ± 45,8 cm H2O przed i 184,2 ± 45,0 cm H2O po leczeniu (NS). Siła uścisku prawej
ręki w grupie ZN wynosiła 44,5 ± 17,7 kg przed i 47,9 ± 10,4 kg po leczeniu (NS), natomiast lewej - 38,1 ± 15,9 kg przed
i 46,9 ± 11,1 kg po terapii (p < 0,05). W łącznej analizie obu grup wykazano tendencję do poprawy siły mięśni oddechowych
po leczeniu CPAP (wzrost PImax z 123 do 133 cm H2O; p = 0,006 i PEmax z 168,1 do 192 cm H2O).
Wnioski: Wyniki te wskazują, że leczenie za pomocą CPAP poprawia siłę mięśni oddechowych i szkieletowych u chorych
z OBS i ZN oraz poprawia wytrzymałość i tolerancję wysiłku u chorych z ZN.Introduction: The aim of this study was to evaluate the effect of CPAP treatment on respiratory muscle strength and
exercise tolerance in patients with obstructive sleep apnoea (OSA) and overlap syndrome (OS).
Material and methods: 9 patients with OSA and 9 patients with OS were studied. Respiratory muscle assessment,
6 minute walking distance (6MWD) and cycloergometry exercise test were performed before and after six month period of
CPAP treatment.
Results: In OSA group exercise tolerance did not change after the treatment. Mean 6MWD was 571.8 ± 76.6 m before and
554.0 ± 125.5 m after treatment, mean Wmax was 142 ± 41 W before and 139 ± 38 W after treatment. PImax in OSA group
did not change significantly, 140.4 ± 32.0 cm H2O before and 155.9 ± 31.5 after treatment (p = 0.14). PEmax improved from
170.5 ± 49.2 cm H2O, to 199.9 ± 27.6 cm H2O (p = 0.067). Handgrip force in OSA group improved from 50.5 ± 16.5 kg to
61.0 ± 17.0 kg (left hand) (p = 0.05) and from 53.3 ± 14.2 to 58.9 ± 15.9 (right hand) (p < 0.05). In OS group exercise
tolerance improved by 17% after CPAP treatment from Wmax = 81 ± 33 W before to 95 ± 38 W after. Mean 6MWD was at
the same level before (504 ± 144 m) and after treatment (492 ± 108 m). PImax in OS group improved from 89.2 ± 35.7 cm
H2O to 106.3 ± 31.4 cm H2O (p < 0.05). PEmax in OS group did not change significantly, 159.9 ± 45.8 cm H2O before and
184.2 ± 45.0 cm H2O after treatment (NS). Handgrip force in OS group improved from 38.1 ± 15.9 kg to 46.9 ± 11.1 kg (left
hand) (p < 0.05) and did not change in right hand (44.5 ± 17.7 kg vs. 47.9 ± 10.4 kg) (NS). Data analysis of the whole group
(18 pts) showed clear tendency to improve strength of respiratory muscles in patients treated with CPAP. Mean PImax
improved from 123 to 133 cm H2O (p = 0.006) and PEmax improved from 168.1 to 192 cm H2O (p = 0.02).
Conclusions: CPAP treatment improved strentgh of respiratory and skeletal muscles in patients with OSA and OS and
improved exercise tolerance in patients with OS
Community-acquired pneumonia complications in a patient with hereditary glucose-6-phosphate dehydrogenase deficiency
Wrodzony niedobór dehydrogenazy glukozo-6-fosforanowej (G6PD) może prowadzić do ciężkich powikłań w przypadku
zakażenia dolnych dróg oddechowych.
Przedstawiamy przypadek 68-letniego mężczyzny z wrodzonym niedoborem G6PD, u którego obserwowano ciężkie powikłania
pozaszpitalnego zapalenia płuc. Niedobór G6PD u chorego rozpoznano w dzieciństwie, od wielu lat chory pozostawał bez
objawów choroby. W opisywanym okresie w przebiegu zapalenia płuc obserwowano ropniaka opłucnej, przełom hemolityczny,
ciężką niedokrwistość i niewydolność nerek wymagającą leczenia hemodializami. W celu wyleczenia ropnych
powikłań wykonano u chorego wideopleuroskopię i drenaż opłucnej.
Pozaszpitalne zapalenie płuc może prowadzić do powikłań hemolitycznych u chorych z niedoborem G6PD. Ciężkie ropne
powikłania zapalenia płuc u tych chorych mogą być związane z upośledzeniem funkcji granulocytów.Severe complications of lower respiratory tract infection in a patient with hereditary glucose-6-phosphate dehydrogenase
(G-6-PD) deficiency may occur.
The case of a 68-year-old man with hereditary glucose-6-phosphate dehydrogenase (G6PD) deficiency who developed
severe haemolysis after community-acquired pneumonia is presented. G6PD deficiency in our patient was diagnosed during
childhood. We observed complications of community-acquired pneumonia: empyema, haemolytic crisis and renal failure.
Videopleuroscopy and pleural drainage were successfully performed.
Community-acquired streptococcal pneumonia may also lead to haemolysis in G6PD deficient patients. Acute haemolysis,
severe anaemia and renal insufficiency secondary to haemoglobinuria can be observed. Severe purulent complications of
pneumonia in G6PD deficient patients may suggest granulocyte function impairment

Introduction: Office spirometry has been widely used in recent years by general practitioners in primary care setting, thus the need for stricter monitoring of the quality of spirometry has been recognized.
Material and methods: A spirometry counseling network of outpatients clinics was created in Poland using portable spirometer Spirotel. The spirometry data were transferred to counseling centre once a week. The tests sent to the counseling centre were analyzed by doctors experienced in the analysis of spirometric data. In justified cases they sent their remarks concerning performed tests to the centres via e-mail.
Results: We received 878 records of spirometry tests in total. Data transmission via the telephone was 100% effective. The quality of spirometry tests performed by outpatients clinics was variable.
Conclusions: The use of spirometers with data transfer for training purposes seems to be advisable. The impact of comorbidities on the length of hospital treatment in patients with chronic obstructive pulmonary disease
Introduction: The aim of this study was to assess relationships of chronic obstructive pulmonary disease (COPD)
comorbidities number, with the duration of hospital stay due to acute AE COPD in longitudinal prospective study.
Material and methods: We evaluated the number of re-hospitalizations, length of stay and number of comorbidities in 464
consecutive COPD patients admitted to the tertiary respiratory hospital due to AE COPD enrolled in longitudinal prospective
study from 2005 to 2009 year.
Results: GOLD II stage COPD patients had 4.1 ± 1.2 comorbidities (p = 0.002), stage III 3.4 ± 1.3 and stage IV had 3.6 ±
1.2 comorbidities. Duration of hospital stay (medians) was longer in more severe patients. Duration of hospitalization
correlated with urea level (r = 0.19 p < 0.001), pCO2 (r = 0.193, p = 0.0003), HCO3 (r = 0.25, p < 0.0001), haemoglobin
(r = –0.18, p < 0.001), and hematocrit (r = –0.13, p = 0.008). The patients with the risk of readmission had more severe
GOLD stage and were hypercapnic (pCO2 = 47.6 mmHg v. 43.9 mmHg in those without hospitalization).
Conclusions: The haemoglobin level, hypercapnia and renal function are predictors of prolonged hospitalization. Patients
with more severe airflow limitation and higher pCO2 have increased risk for readmission to the hospital. More severe disease
stage, clinical diagnosis of cor pulmonale or bronchiectasis was related to longer hospital stay.

przewlekłej obturacyjnej choroby płuc (POChP) oraz ich związku z długością hospitalizacji na referencyjnym oddziale
chorób płuc.
Materiał i metody: Badano chorych przyjmowanych kolejno do II Kliniki Chorób Płuc Instytutu Gruźlicy i Chorób Płuc
w Warszawie, którzy w latach 2005–2009 zostali włączeni do długookresowego badania prospektywnego obejmującego
chorych na POChP. Oceniano liczbę chorób współistniejących i odsetek ponownych hospitalizacji oraz długość pobytu
szpitalnego.
Wyniki: Do badania włączono 464 pacjentów. Chorzy w II stadium POChP mieli 4,1 ± 1,2 (p = 0,002) chorób współistniejących,
w stadium III — 3,4 ± 1,3, a w stadium IV — 3,6 ± 1,2. Mediana czasu leczenia szpitalnego wzrastała w miarę
ciężkości choroby. Długość pobytu szpitalnego korelowała ze stężeniem mocznika we krwi (r = 0,19; p < 0,001), pCO2 (r =
0,193; p = 0,0003), HCO3 (r = 0,25; p < 0,0001), stężeniem hemoglobiny (r = –0,18; p < 0,001) oraz z hematokrytem (r =
–0,13; p = 0,008). Chorzy ze zwiększonym ryzykiem ponownego przyjęcia do szpitala z powodu POChP charakteryzowali się
hiperkapnią (pCO2 = 47,6 mmHg v. 43,9 mmHg u chorych bez ponownych pobytów szpitalnych).
Wnioski: Obniżone stężenie hemoglobiny, hiperkapnia oraz parametry czynności nerek wpływają na ryzyko przedłużonego
pobytu szpitalnego. U chorych z bardziej nasiloną obturacją oskrzeli i hiperkapnią ryzyko ponownego przyjęcia do szpitala jest
podwyższone. Cięższe stadium choroby, kliniczne rozpoznanie serca płucnego oraz rozstrzeni oskrzeli wiązało się z dłuższą
hospitalizacją.
