Examining the clinical relevance of metformin as an antioxidant intervention
In physiological concentrations, reactive oxygen species play a vital role in regulating cell signaling and gene expression. Nevertheless, oxidative stress is implicated in the pathogenesis of numerous diseases and can inflict damage on diverse cell types and tissues. Thus, understanding the factors that mitigate the deleterious effects of oxidative stress is imperative for identifying new therapeutic targets. In light of the absence of direct treatment recommendations for reducing oxidative stress, there is a continuing need for fundamental research that utilizes innovative therapeutic approaches. Metformin, known for its multifaceted beneficial properties, is acknowledged for its ability to counteract the adverse effects of increased oxidative stress at both molecular and cellular levels. In this review, we delve into recent insights regarding metformin’s antioxidant attributes, aiming to expand its clinical applicability. Our review proposes that metformin holds promise as a potential adjunctive therapy for various diseases, given its modulation of oxidative stress characteristics and regulation of diverse metabolic pathways. These pathways include lipid metabolism, hormone synthesis, and immunological responses, all of which may experience dysregulation in disease states, contributing to increased oxidative stress. Furthermore, our review introduces potential novel metformin-based interventions that may merit consideration in future research. Nevertheless, the necessity for clinical trials involving this drug remains imperative, as they are essential for establishing therapeutic dosages and addressing challenges associated with dose-dependent effects
TLR4 ligation induces expression of APRIL molecule in human neutrophils — a preliminary study
In the present study we investigate the consequences of TLR4 activation by LPS for the synthesis of a proliferation-inducing ligand (APRIL) by human neutrophils (PMNs), and the possible role of the ERK1/2 kinases signaling pathway. In order to make a comparison, the same examinations were carried out on autologous peripheral blood mononuclear cells (PBMCs). The levels of mRNA for APRIL and TLR4 were measured using the real-time PCR method. Western blot analysis was used to assay the expressions of APRIL and ERK1/2 in cell lysates. We discovered an increased expression of APRIL accompanying the increased expression of TLR4 in the LPS-stimulated PMNs and PBMCs. Furthermore, stimulation with LPS triggered similar changes in phospho-ERK1/2 proteins expression in those cells. The present study suggests that LPS plays a role in TLR4-ligation in APRIL induction through ERK1/2 pathway activation in human neutrophils and mononuclear cells of peripheral blood. The association between TLR4 activation and APRIL expression in examined leukocytes might have important implications for the mmune response of the host exposed to TLR4 ligands such as LPS
Występowanie chorób autoimmunologicznych u pacjentów z późno ujawniającą się cukrzycą autoimmunologiczną u osób dorosłych — związek z genotypem HLA
Introduction: Latent autoimmune diabetes in adults (LADA) is a slow-developing form of autoimmune diabetes, characterised by the presence of type 1 diabetes-associated autoantibody and presentation at diagnosis similar to patients with type 2 diabetes. The aim of this study was to determine the prevalence of auto-antibodies related to endocrine autoimmune diseases in patients with LADA and to assess their association with HLA genotype.
Material and methods: We evaluated the presence of anti-thyroglobulin (ATG), anti-thyroid peroxidase (ATPO), anti-tissue transglutaminase IgA (ATTA), and anti 21 hydroxylase (A21H) in 70 patients with LADA, 69 with Type 2 diabetes, and in 50 healthy controls HLA genotype was assed in subpopulation of sluclied subjects.
Results: The presence of ATPO (28.6 vs. 10%); ATG (28.6 vs. 14%) was higher in patients with LADA in comparison to healthy controls and ATPO in comparison to patients with type 2 diabetes (38.6 vs. 17.4 %). In patients with LADA the presence of autoimmune thyroid autoantibodies was associated with newly diagnosed subclinical hypothyroidism; almost 7% of patients presented with high TSH. The presence of A21H (2.86 vs. 5.8 vs. 6.1%) and ATTA (2.86 vs. 4.3 vs. 6.0%) was not different between groups. Patients with high TSH level were positive for DQA1*0301 and DRB1*04 HLA genotype: DQB1*0201 and DQB1*02 were higher in patients positive for ATTA.
Conclusions: Patients with LADA have higher prevalence of thyroid autoimmune diseases. In patients with LADA similarly to type 1 genotype DQA1*0301 seems to CONFER susceptibility to thyroid autoimmunity, and DQB1*0201 to celiac disease. (Endokrynol Pol 2016; 67 (2): 197–201)
Wstęp: Późno ujawniająca się cukrzyca autoimmunologiczną u osób dorosłych cechuje się obecnością przeciwciał skierowanych przeciwko antygenom wysp trzustkowych u pacjentów z kliniczną prezentacją cukrzycy typu 2. Celem pracy była ocena występowania chorób autoimmunologicznych u pacjentów z cukrzycą LADA oraz ocena związku z genotypem HLA.
Materiał i metody: W obecnej pracy oceniono występowanie przeciwciał przeciwko-tyreoglobulinie (ATG), przeciwko peroksydazie tarczycowej (ATPO), przeciwko transglutaminazie tkankowej IgA (ATTA) oraz przeciwko 21 hydroksylazie (A21H) u 70 pacjentów z cukrzycą LADA, 69 z cukrzycą typu 2 oraz 50 osób z grupy kontrolnej oraz genotyp HLA.
Wyniki: Częstość występowania przeciwciał ATPO (28,6 vs. 10%), ATG (28,6 vs. 14%) była wyższa w grupie pacjentów z cukrzycą typu LADA w porównaniu do grupy kontrolnej oraz ATPO (38,6 vs. 17,4%) w porównaniu z pacjentami z cukrzycą typu 2. W grupie pacjentów z cukrzycą typu LADA istotnie cześciej stwierdzono obecność przeciwciał ATPO oraz ATG, którym towarzyszyly nowowykryte zaburzenia kliniczne funkcji tarczycy. Częstość występowania przeciwciał przeciwko 21 OH hydroksylazie (2,86 vs. 5,8 vs. 6,1%) oraz przeciwko transglutaminazie w klasie IgA (2,86 vs. 4,3 vs. 6,0%) nie różniła się pomiędzy badanymi grupami. Genotyp HLA DQA1*0301 występował częściej u pacjentów z autoimmunologiczną niedoczynnością tarczycy, nastomiast DQB1*0201 u pacjentów z przeciwciałami ATTA będącymi markerem choroby trzewnej.
Wnioski: U pacjentów z cukrzycą LADA częściej występują przeciwciała skierowane przeciwko antygenom tarczycy współistniejące z subliniczną niedoczynością tarczycy. (Endokrynol Pol 2016; 67 (2): 197–201)
Assessment of the Immune Response in Patients with Insulin Resistance, Obesity, and Diabetes to COVID-19 Vaccination
The SARS-CoV-19 pandemic overwhelmed multiple healthcare systems across the world. Patients with underlying medical conditions such as obesity or diabetes were particularly vulnerable, had more severe symptoms, and were more frequently hospitalized. To date, there have been many studies on the severity of SARS-CoV-2 in patients with metabolic disorders, but data on the efficiency of vaccines against COVID-19 are still limited. This paper aims to provide a comprehensive overview of the effectiveness of COVID-19 vaccines in individuals with diabetes, insulin resistance, and obesity. A comparison is made between the immune response after vaccination in patients with and without metabolic comorbidities. Additionally, an attempt is made to highlight the mechanisms of immune stimulation affected by SARS-CoV-2 vaccines and how metabolic comorbidities modulate these mechanisms. The focus is on the most common COVID-19 vaccines, which include mRNA vaccines such as Pfizer-BioNTech and Moderna, as well as viral vector vaccines such as AstraZeneca and Johnson & Johnson. Furthermore, an effort is made to clarify how the functional differences between these vaccines may impact the response in individuals with metabolic disorders, drawing from available experimental data. Insulinooporność i adaptacja komórek beta trzustki podczas ciąży
Insulinooporność jest to zmniejszona wrażliwość tkanek na insulinę. U kobiet w trakcie ciąży w kolejnych trymestrach dochodzi do wzrostu insulinooporności pod wpływem hormonów produkowanych przez łożysko oraz wielu różnych nie do końca jeszcze poznanych czynników. W związku z narastającą insulinoopornością zwiększają się ilość i masa komórek beta oraz następuje wzrost wydzielania insuliny, dzięki czemu utrzymane zostają homeostaza glukozy i prawidłowy rozwój płodu. W przypadku, gdy dochodzi do zbyt dużego wzrostu insulinooporności, niewystarczającej kompensacji komórek beta trzustki lub obniżenia ich funkcji, zwiększa się glikemia i rozwija się cukrzyca ciążowa. Celem niniejszej pracy jest analiza czynników wpływających na insulinooporność i adaptację komórek beta trzustki podczas ciąży oraz metod oceny insulinooporności
Risk factors for the onset of gestational diabetes mellitus
WSTĘP. Cukrzyca ciężarnych (GDM, gestational diabetes mellitus) to różnego
stopnia zaburzenia tolerancji węglowodanów wykryte podczas ciąży. W Polsce częstość
GDM szacuje się na około 4%. Celem badań była identyfikacja czynników ryzyka ujawnienia
się GDM, które wskazują na konieczność wykonywania badań przesiewowych w grupie
kobiet ciężarnych.
MATERIAŁ I METODY. W częściowo retrospektywnym badaniu uczestniczyły 283
kobiety. Badania przeprowadzono 8-12 tygodni po porodzie u 136 pacjentek z rozpoznaną
GDM klasy G1 oraz u 147 kobiet, które w ciąży miały prawidłową tolerancję węglowodanów.
W pobranej krwi żylnej oznaczano wybrane parametry gospodarki węglowodanowej i
tłuszczowej. Ponadto, analizie statystycznej poddano dane antropometryczne pacjentek.
WYNIKI. Znaczna część pacjentek z GDM miała nadwagę lub otyłość i w ich
rodzinach częściej występowała cukrzyca niż w rodzinach chorych z grupy kontrolnej.
Ponadto średni wiek metrykalny był wyższy niż u kobiet z prawidłową tolerancją
węglowodanów. Urodzeniowa masa ciała noworodków matek z GDM była wyższa niż w
grupie kontrolnej. Stężenia HbA1c, triglicerydów i cholesterolu były wyższe w
grupie kobiet z GDM.
WNIOSKI. Cukrzyca ciężarnych częściej ujawnia się u kobiet starszych, z
nadwagą lub otyłością. Liczba porodów oraz występowanie cukrzycy w rodzinie zwiększa
ryzyko GDM. Wśród tych kobiet należy bezwzględnie przeprowadzić badania przesiewowe
w kierunku GDM.INTRODUCTION. Gestational diabetes mellitus (GDM) is defined as carbohydrate
intolerance of variable severity with onset or first recognition during pregnancy.
MATERIAL AND METHODS. For this partly retrospective study 283 women were
selected. Of these 136 were diagnosed with class G1 GDM (treated by diet only).
The remaining 147 with negative screenings for GDM in both, previous and ongoing
pregnancies and otherwise uneventful course of gestation, formed the control
group. Laboratory blood testing including fasting glycemia, lipid profile and
HbA1c level was carried out 8 to 12 weeks post partum. Partly retrospectively
and also on the day of biochemical lab tests the basic anthropometry was obtained.
RESULTS. Our data highlighted increased BMI and raised WHR, gestational
diabetes mellitus in previous pregnancy, family history of diabetes mellitus
and advanced age as maternal factors predisposing for GDM onset.
CONCLUSIONS. We believe that identification of women with carbohydrate
intolerance, along with establishing more risk factors for the onset of gestational
diabetes mellitus would be crucial for better monitoring the outcomes and sequels
of GDM. GDM women represent a public health concern because of increased maternal
and neonatal morbidity. This population study on the sample of Polish women
enabled to present pregnant individuals at risk. Hopefully it would serve to
promote international consensus on the controversial issue of the screening,
diagnosis and management of GDM
Compliance in diabetes — target or way to success?
In medicine, the term "compliance" is associated with medical practice and guidance, especially in relation to chronic diseases. Its definition has evolved with the development of modern medicine. In the last decade, the relationship between patient and doctor has changed significantly. The increasing importance of therapeutic success is attributed to the personalization of medicine and the consideration of patients’ perspectives and preferences. The purpose of this manuscript is to find a modern definition of the term compliance in relation to diabetes and to determine what elements of personalized medicine can be incorporated into patient care to improve overall clinical success. Postprandial metabolic and hormonal response to meals with varying carbohydrate content (preliminary examination)
WSTĘP. Zaburzenia homeostazy energetycznej organizmu zależną
od czynników genetycznych, hormonalnych/metabolicznych i środowiskowych
oraz ich wzajemnych interakcji. W wielu badaniach
potwierdzono wpływ diety o różnej zawartości podstawowych
składników odżywczych na centralne i obwodowe mechanizmy
regulacji łaknienia. Celem pracy była ocena zmian metabolicznych
i hormonalnych po posiłkach o różnej zawartości węglowodanów.
MATERIAŁ I METODY. Badaniem objęto 20 zdrowych mężczyzn
w wieku 28–52 lat, ze wskaźnikiem masy ciała (BMI) 19,6–43,0 kg/m2
(średnie BMI 28,0 ± 5,8 kg/m2). Badani w 2-tygodniowym odstępie
czasowym otrzymali standaryzowane izokaloryczne (450 kcal)
posiłki: Nutridrink Fat Free, w którym 89% energii pochodziło z węglowodanów,
oraz Cubitan, w którym 45% energii pochodziło
z węglowodanów. Na czczo, 30, 60, 120, 180 i 240 minut po posiłku
oznaczano stężenia glukozy, insuliny, triacylogliceroli, leptyny
i greliny całkowitej oraz, metodą kalorymetrii pośredniej, oceniono
oksydację substratów energetycznych.
WYNIKI. Po posiłku, w którym 89% energii pochodziło z węglowodanów,
wykazano wyższe stężenia glukozy (p < 0,01) i insuliny
(p < 0,01). W 180. minucie po posiłku wysokowęglowodanowym
u 15% (3 osób) oraz w 240. minucie u 25% (5 osób) badanych odnotowano
glikemię poniżej 60 mg/dl (hipoglikemie). Po posiłku, w którym
45% energii pochodziło z węglowodanów, wykazano wyższe stężenia
triacylogliceroli (p = 0,02) i jednoczesne zwiększenie oksydacji kwasów
tłuszczowych (p < 0,01). Obserwowano też tendencję (p =0,12)
do zmniejszenia stężenia greliny po posiłku, w którym 45% energii
pochodziło z węglowodanów, czego nie stwierdzono po posiłku wysokowęglowodanowym.
Odnotowano różne stężenie leptyny
(p = 0,03), której stężenia wzrastały istotnie (p = 0,01), począwszy od
60. minuty po spożyciu posiłku o mniejszej zawartości węglowodanów.
WNIOSKI. Uzyskane wyniki wskazują, że następstwa metaboliczne
i hormonalne po spożyciu posiłku wysokowęglowodanowego mogą zaburzać równowagę między czynnikami biorącymi udział w regulacji łaknienia. Zaobserwowane zmiany mogą sprzyjać występowaniu
otyłości.INTROCUCTION. Disorders of energy homeostasis depend on interactions
between genetic, hormonal/metabolic and environmental
factors. Several studies have confirmed the influence of diet with
different nutrients content on central and peripheral mechanisms
that regulate energy balance.The aim of the study was to evaluate
metabolic and hormonal response to meals with different carbohydrate
content.
MATERIALS AND METHODS. Twenty healthy men aged 28–52
years and body mass index (BMI) 19.6–43.0 kg/m2 (average BMI
28.0 ± 5.8 kg/m2) were examined. Subjects received (in two weeks
interval) standardized (450 kcal) meals: Nutridrink Fat Free (89%
of energy from carbohydrate) and Cubitan (45% of energy from
carbohydrate). Plasma glucose, insulin, triacyloglicerol, leptin and
total ghrelin levels were measured at 0’, 30’, 60’, 120’, 180’ and
240’ min after meal ingestion. Substrate oxidation was determined
by indirect calorimetry.
RESULTS. After the meal containing 89% of energy from carbohydrates
we found higher glucose (p < 0.01) and insulin (p < 0.01)
concentrations. At 180 min after the high-carbohydrate meal in 15%
(3 men), and at 240 minutes in 25% (5 men) of subjects glucose
levels < 60 mg/dl (hypoglycemia) were noted. After the meal
containing 45% of energy from carbohydrates, triacyloglicerols
levels were higher (p = 0.02) and lipid oxidation was increased
(p < 0.01). We observed tendency (p = 0.12) to reduce plasma
ghrelin levels after meal with 45% of total energy as carbohydrates,
which was not observed after high-carbohydrate meal. Moreover,
varying secretion of leptin (p = 0.03) was noted. Concentrations of
leptin increased significantly (p = 0.01) from 60 minutes after meal
with lower carbohydrate content.
CONCLUSIONS. The results indicate that metabolic and hormonal
response after a high-carbohydrate meal ingestion may affect
unfavourably the balance between factors involved in appetite regulation.
Observed changes may favor obesity
Stężenie interleukiny-6, receptora dla interleukiny-6 i glikoproteiny 130 oraz cytokin zależnych od limfocytów Th17 u pacjentek z cukrzycą ciążową
Introduction: Interleukin-6 (IL-6) is a pleiotropic cytokine which signals through a cell surface receptor complex consisting of a cognatereceptor subunit (IL-6R) and glycoprotein 130 (gp130), which is considered an antagonist to the IL-6R/IL-6 pathway. The aim of the present study was to assess IL-6/IL-6R/gp130 system and Th17 associated cytokines in different time points during and after pregnancy in women with gestational diabetes mellitus (GDM) and normal glucose tolerance (NGT).Material and methods: Serum levels of IL-6, sIL6R, sgp130, IL-17 and IL-23 were measured in 91 women divided into three groups: GDMin the 24th–28th week of gestation (visit 1), NGT at the 1st visit and GDM in the 29th-32nd week, and NGT at both visits.Results: The patients with GDM recognised at the 1st visit had significantly higher IL-6 (p = 0.02) and sgp130 (p = 0.03) concentrations than had the women with NGT, whereas the women with GDM diagnosed at the 2nd visit had elevated sIL-6R concentrations (p = 0.03). The patients with low sIL-6R but high sgp130 concentration had significantly higher glucose levels (p = 0.04) and lower IL-6 values (p = 0.04) than had the patients with low sIL-6R and sgp130 concentrations. IL-17 and IL-23 were detected in approximately one-third of the population studied. A trend towards higher IL-17 levels was observed in the subjects with GDM, but the differences were not significant.Conclusions: Our results suggest that an increased serum sgp130 concentration in the patients with GDM might represent a compensatory mechanism, controlling intracellular IL-6 signalling and preventing the activation of the IL-6/IL-6R pathway.Wstęp: Interleukina-6 (IL-6) jest plejotropową cytokiną, która przekazuje sygnał do wnętrza komórki za pośrednictwem receptora błonowego złożonego z właściwej podjednostki receptorowej (Il-6R) i glikoproteiny 130 (gp130), uważanej za antagonistę kompleksu IL-6R//IL-6. Celem pracy była analiza szlaku IL-6/IL-6R/gp130 i cytokin zależnych od limfocytów Th17 w II i III trymestrze ciąży oraz po porodzie u pacjentek z cukrzycą ciążową i prawidłową tolerancją glukozy.Materiał i metody: Dokonano pomiaru stężenia IL-6, IL6R, gp130, IL-17 i IL-23 w surowicy 91 kobiet podzielonych na 3 podgrupy: pacjentki z cukrzycą ciążową zdiagnozowaną w 24.–28. tygodniu ciąży (wizyta 1), pacjentki z prawidłową tolerancją glukozy w trakcie 1 wizyty i cukrzycą ciążową rozpoznaną w 29.–32. tygodniu ciąży oraz pacjentki z prawidłową tolerancją glukozy w trakcie obu wizyt.Wyniki: U pacjentek z cukrzycą ciążową rozpoznaną w trakcie 1 wizyty wykazano istotnie wyższe stężenia IL-6 (p = 0,02) i gp130(p = 0,03) w porównaniu z kobietami z prawidłową tolerancją glukozy, podczas gdy pacjentki z cukrzycą ciążową rozpoznaną w trakcie 2 wizyty charakteryzowały się wyższym stężeniem IL-6R (p = 0,03). U pacjentek z niskim stężeniem IL-6R i wysokim stężeniem gp130 obserwowano znamiennie wyższe wartości glikemii (p = 0,04) i niższe stężenia IL-6 (p = 0,04) w porównaniu z kobietami z niskimi stężeniami IL-6R i sgp130. Obecność krążących IL-17 i IL-23 stwierdzono u około 1/3 spośród badanych kobiet. Wykazano również tendencję do wyższych stężeń IL-17 u pacjentek z cukrzycą ciążową, różnice nie były jednak istotne statystycznie.Wnioski: Wyniki sugerują, że podwyższone stężenie gp130, obserwowane u kobiet z cukrzycą ciążową, może stanowić mechanizm kompensacyjny, zapobiegający nadmiernej aktywacji szlaku IL-6/IL-6R
Long-term effects of COVID-19 on the endocrine system – a pilot case-control study
BackgroundCoronavirus disease 2019 (COVID-19) has permanently changed the world. Despite having been a pandemic for nearly 3 years, the mid- and long-term complications of this disease, including endocrine disorders, remain unclear. Our study aimed to evaluate the lasting effects of COVID-19 on the endocrine system 6 months after initial infection.MethodsWe compared patients who underwent COVID-19 to age- and sex-matched subjects from a population-based study conducted before the pandemic. We evaluated differences in multiple parameters related to metabolism and the endocrine system including fasting glucose, insulin, lipids, body composition, thyroid stimulating hormone (TSH), free thyroxine (fT4), free triiodothyronine (fT3), anti-thyroglobulin (aTG) and anti-thyroid peroxidase (aTPO) antibodies, prolactin, cortisol, testosterone, and estradiol.ResultsWe found significantly lower levels of fT3 and fT4, accompanied by higher levels of TSH and aTPO antibodies, in COVID-19 survivors. Moreover, we found that patients who underwent SARS-CoV2 infection had higher levels of prolactin and lower levels of testosterone than controls. Interestingly, differences in testosterone levels were observed only in male subjects. We did not detect significant differences in body composition or metabolic and glycemic parameters between cases and controls, except for significantly higher values of the HOMA2-B index in COVID-19 survivors.ConclusionOur study indicates that severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection might have long-term consequences on the endocrine system, including the suppressed function of the thyroid gland, prolactin, and male sex hormone secretion. Moreover, we showed that in a 6-month follow-up, COVID-19 had no consequences on glycemic parameters, lipid profiles, liver function, body composition, cortisol levels, and estradiol levels
