    Determinants of childhood obesity in representative sample of children in north east of iran

    Childhood obesity has become, a global public health problem, and epidemiological studies are important to identify its determinants in different populations. This study aimed to investigate factors associated with obesity in a representative sample of children in Neishabour, Iran. This study was conducted among 1500 randomly selected 6-12-year-old students from urban areas of Neishabour, northeast of Iran. Then, through a case-control study, 114 obese (BMI � 95 th percentile of Iranian reference) children were selected as the case group and were compared with 102 controls (15 th � BMI 85 th percentile). Factors suggested to be associated with weight status were investigated, for example, parental obesity, child physical activity levels, socio-economic status (SES), and so forth. The analysis was conducted using univariate and multivariate logistic regression (MLR) in SPSS version 16. In univariate logistic regression model, birth weight, birth order, family extension, TV watching, sleep duration, physical activity, parents job, parents education, parental obesity history, and SES were significantly associated with childrens obesity. After MLR analysis, physical activity and parental obesity history remained statistically significant in the model. Our findings showed that physical activity and parental obesity history are the most important determinants for childhood obesity in our population.     Dietary predictors of childhood obesity in a representative sample of children in North East of Iran

    Objective: The prevalence of obesity is increasing in Iranian youngsters. This study aimed to assess some dietary determinants of obesity in a representative sample of children in Neishabour, a city in northeastern, Iran. Methods: This case-control study was conducted among 114 school students, aged 6-12 years, with a body mass index (BMI) �95th (based on percentile of Iranian children) as the case group and 102 age- and gender-matched controls, who were selected from their non-obese classmates. Nutrient intake data were collected by trained nutritionists by using two 24-hour-dietary recalls through maternal interviews in the presence of their child. A food frequency questionnaire was used for detecting the snack consumption patterns. Statistical analysis was done using univariate and multivariate logistic regression (MLR) by SPSS version 16. Results: In univariate logistic regression, total energy, protein, carbohydrate, fat (including saturated, mono- and poly-unsaturated fat), and dietary fiber were the positive predictors of obesity in studied children. The estimated crude ORs for frequency of corn-based extruded snacks, carbonated beverages, potato chips, fast foods, and chocolate consumption were statistically significant. After MLR analysis, the association of obesity remained significant with energy intake (OR = 2.489, 95 CI: 1.667-3.716), frequency of corn-based extruded snacks (OR = 1.122, 95 CI; 1.007-1.250), and potato chips (OR = 1.143, 95 CI: 1.024-1.276). The MLR analysis showed that dietary fiber (OR = 0. 01, 95 CI; 0.368-0.983) and natural fruit juice intake (OR = 0.909, 95 CI; 0.835-0.988) were protective factors against obesity. Conclusions: The findings serve to confirm the role of an unhealthy diet, notably caloriedense snacks, in childhood obesity. Healthy dietary habits, such as the consumption of high-fiber foods, should be encouraged among children

    Rising rural body-mass index is the main driver of the global obesity epidemic in adults

    Body-mass index (BMI) has increased steadily in most countries in parallel with a rise in the proportion of the population who live in cities 1,2 . This has led to a widely reported view that urbanization is one of the most important drivers of the global rise in obesity 3�6 . Here we use 2,009 population-based studies, with measurements of height and weight in more than 112 million adults, to report national, regional and global trends in mean BMI segregated by place of residence (a rural or urban area) from 1985 to 2017. We show that, contrary to the dominant paradigm, more than 55 of the global rise in mean BMI from 1985 to 2017�and more than 80 in some low- and middle-income regions�was due to increases in BMI in rural areas. This large contribution stems from the fact that, with the exception of women in sub-Saharan Africa, BMI is increasing at the same rate or faster in rural areas than in cities in low- and middle-income regions. These trends have in turn resulted in a closing�and in some countries reversal�of the gap in BMI between urban and rural areas in low- and middle-income countries, especially for women. In high-income and industrialized countries, we noted a persistently higher rural BMI, especially for women. There is an urgent need for an integrated approach to rural nutrition that enhances financial and physical access to healthy foods, to avoid replacing the rural undernutrition disadvantage in poor countries with a more general malnutrition disadvantage that entails excessive consumption of low-quality calories. © 2019, The Author(s)

    Dietary Factors Associated To Obesity In Ahwaz Primary School Pupils

    Background: Increase in obesity prevalence in recent years are associated to genetics as well environmental and behavioral factors. Change in dietary patterns including fatty and high density energy foods consumption have been reported to be very important. This study aimed to determine dietary factors (daily energy and macronutrient intakes, energy percentage of macronutrient, energy and macronutrient intakes per kilogram body weight, frequency of cola, natural fruit juice drinking, dairy products except cheese, tomato chips, puff, chocolate and fast food consumption and eating speed) associated to obesity in Ahwaz primary school pupils. Materials and Methods: Using two stage cluster sampling from 35 Ahwaz primary schools, all 10-11y students who had a BMI 95th percentile of Hosseini et al. (1999) reference, were identified as obese (n=150) and 150 same age and gender pupils (having BMI0.05). macronutrient intakes per kilogram body weight were significantly lower in obese group (p0.05). Obese students used to eat faster (p<0.05). Conclusion: In conclusion, high intakes of energy, protein, carbohydrate, tomato chips and puff and high eating speed were associated to obesity in Ahwaz primary school pupils

    Food insecurity and primary school girl students� intelligence quotients: A case-control study

    Background: Food insecurity (FI) refers to the lack of sufficient and safe availability of the food. Accumulating studies have suggested associations between dietary intake and Intelligence quotient (IQ). Accordingly, we aimed to examine the association between FI and IQ. Study design: Case-control study. Methods: In this case-control study, 222 girl students aged 9 to 11 years old were randomly chosen from Bandar-Anzali, Iran in 2013. Students with low and moderate IQ were considered as case (n=111) and control (n=111) group, respectively. General and demographic characteristics were collected using interview. United States Department of Agriculture (USDA) household FI questionnaires were also completed. To determine IQ, Wechsler�s revised intelligence test was used. Results: FI was observed in 51 of study participants. The prevalence of FI in case and control group was 58.6 and 22.5, respectively. The mean IQ was 77.97 ±5.56 in case and 94.6 ±5.47 for control group. It was found that there was an inverse association between FI and low IQ. The results of the multiple variable logistic regression analysis (odds ratio and its 95 confidence interval) showed that, FI 3.46 (1.85, 6.50; P>0.001), natural type of delivery 2.45 (1.30, 4.62; P=0.006), and father�s low education level 2.97 (1.43, 6.19; P=0.004) were the risky factors leading to low IQ. Conclusions: There was an inverse link between FI and IQ. Therefore, it is necessary to pay more attention to FI and its consequences, particularly in mental health of children. © 2018, Health Hamadan University of Medical Sciences. All rights reserved

    Effects of EPA and vitamin E on serum enzymatic antioxidants and peroxidation indices in patients with type II diabetes mellitus

    Background: Diabetes mellitus is associated with chronic changes in peripheral arteries because of oxidative stress and insufficient antioxidative defense mechanism. Omega-3 fatty acid supplementation could be effective in some diabetes complications; however, polyunsaturated fatty acids may increase lipid peroxidation. This study aimed to determine whether eicosapentaenoic acid alone or in conjunction with vitamin E had differential effects on serum antioxidants and peroxidation indices. Methods: This double-blind, placebo-controlled trial was carried out on 136 patients with type II diabetic mellitus (age 48.8±4.4 yr, BMI 27.8±1.7 kg/m2). The four groups of the study either received two grams of omega-3 fatty acids, 400 IU of vitamin E, a combination of the two or placebo for three months. Their serum total antioxidant capacity, enzymatic antioxidants and peroxidation indices were assessed. Result: Fasting serum TAC increased in EPA+E (10.7, P< 0.001) and E groups (7.5, P< 0.05). SOD, G-PX and G-RD increased in EPA group (7.3, 5.1, and 8.4, P< 0.05, respectively). MDA and protein carbonyl decreased in EPA and E groups (respectively, 12.5, 7.6 P< 0.05, P< 0.05; 13, 15.3 P< 0.001, P< 0.05). After adjustment for baseline values, age, sex, BMI and duration of diagnosed diabetes, protein carbonyl decreased in EPA+E and E group (30.7, 15.3; P< 0.05 respectively) relative to the placebo group. Conclusion: EPA, by itself has a statistically significant effect on serum total antioxidant capacity, enzymatic antioxidants and peroxidation indices in diabetic patients compared to EPA+E or E alone

    Comparison of waist circumference percentiles versus body mass index percentiles for diagnosis of excessive fatness in a large cohort of children

    Context. Waist circumference may offer improved diagnosis of obesity in youth compared with body mass index (BMI), but empirical evidence is limited. Objective. To compare the ability of BMI percentile using UK reference data and waist circumference percentile using UK reference data to diagnose high fat mass in English children. Design and Methods. In 7 722 9–10-year-olds (3 809 boys, 3 913 girls) sensitivity and specificity were calculated and receiver operator characteristic (ROC) analyses undertaken to determine the diagnostic accuracy of BMI and waist circumference z-scores to define high fat mass measured by dual energy x-ray absorptiometry (DXA). High fat mass was defined as being in the top decile of fatness for each sex (359 boys and 367 girls). Results. The area under the ROC curve was slightly higher for BMI percentile (0.92 in boys, 95% CI: 0.91 –0.93; 0.94 in girls, 95% CI: 0.93–0.95) than waist circumference percentile (0.89 in boys, 95% CI: 0.86–0.91; 0.81 in girls, 95% CI: 0.73–0.90). Specificity of BMI percentile was slightly but significantly higher than that of waist circumference percentile for both sexes (p<0.05 in each case). Conclusions. The present study suggests that waist circumference percentile has no advantage over BMI percentile for the diagnosis of high fat mass in children
