Peculiarities of educational environment at mining and metallurgical schools of the Urals
The relevancy of the issue under study stems from the contradiction between the need for a modern interpretation of the activities of mining and metallurgical schools, which had played a prominent role in the institutionalization of the vocational education environment as well as from moral βobsolescenceβ of research work results in this field. The goal of the article is the development of environmental indices of mining and metallurgical education of the Urals. The principal concept of research of this problem is the system approach, which allows considering the main peculiarities of the educational environment at mining and metallurgical schools of the Urals comprehensively. This article includes structurally significant characteristics of the educational environment at mining and metallurgical schools of the Urals. The contents of the article may be used for the purpose of modeling, designing and construction of an educational establishment (system) meeting the demands of the new communicational reality within the system of relations between education, industry, and social environment. The article contains the results of a study of two different methods for calculating the spectral parameters of laser Doppler flowmetry in patients with diabetic microangiopathy caused by type 2 diabetes mellitus (main group) and those with excluded diabetes mellitus (control group). Spectral indices were calculated using either average or maximum amplitudes of the frequency ranges. When comparing the contribution of respiratory and pulse fluxmotions using average amplitudes, there were significant (p < 0.05) differences between the main and control groups. On the contrary, when using the maximum amplitudes, no significant differences were noted (p > 0.05). Also, significant correlations were found between the contributions of respiratory and pulse fluxmotions and the estimated glomerular filtration rate in the main group, using both calculation methods. Comprehensive analysis of the results of surgical treatment of patients with degenerative diseases of the lumbar spine using rigid fixation systems
Introduction. According to the literature, the development of adjacent-level syndrome 10 years after posterior rigid stabilization is noted in 6.7β80 % of patients, of which 24 % require revision surgery.The aim: to conduct a comprehensive analysis of the results of surgical treatment of patients with degenerative diseases of the lumbar spine using rigid fixation systems.Materials and methods. A multicenter retrospective study of the results of surgical treatment of 268 patients with degenerative-dystrophic diseases of the lumbar spine, who underwent mono- or bisegmental decompression-stabilizing intervention with the implementation of the TLIF (Transforaminal Lumbar Interbody Fusion) technique and open transpedicular rigid fixation, was conducted. The study included radiography, MRI and CT (in 2-energy mode) of intervertebral discs and isolated facet degeneration of the upper adjacent level.Results and discussion. The combination of the initial degeneration of the adjacent spinal motion segment in the form of disc degeneration of grade III and higher according to Pfirrmann with threshold values of the density of the outer cartilaginous plate from 161.7 Β± 18.8 to 164.8 Β± 14.2 HU, the density of the outer facet β from 702.43 Β± 12.3 to 713.65 Β± 13.6 HU and the density of the inner facet β from 580.5 Β± 11.6 to 582.1 Β± 15.1 HU, as well as with signs of segmental instability of the adjacent segment can be considered as risk factors for the development of its degeneration. With the progression of degeneration of the upper intervertebral disc, revision interventions were performed within 12β60 months after surgery.Conclusion. Bisegmental stabilization with signs of initial degeneration in the intervertebral disc and facet joints in the upper segment has proven its clinical and instrumental effectiveness in the period of 36 months and reduces the risks of developing degenerative disease of the adjacent disc
Morphofunctional reorganization of plantar aponeurosis in experimental modeling of fasciopathy by synthetic analogue of prostaglandin E1
Foundation. Chronic plantar fasciopathic pain syndrome is a pathology that significantly affects the quality of life of patients of all age categories. Insufficient knowledge of the etiological and pathogenetic factors in the development of fasciopathies explains the multiplicity, and sometimes inconsistency, of conservative and surgical treatment regimens. The choice of the optimal variant of therapeutic or surgical intervention may be associated with experimental modeling of fasciopathies and the study of the dynamics of the pathological process.The aim. To study the morphological changes in structures identical to the human plantar aponeurosis in experimental modeling of fasciopathy in animals.Research methods. The material for the study was fragments of the tendonaponeurotic complex of the foot of laboratory animals (control group: animals with the introduction of physiological sodium chloride solution (n = 12); main group: animals with the introduction of alprostadil (n = 12)). The methods of light microscopy (staining with alcian and toluidine blue, according to Van Gieson, Weigert β Van Gieson and Picro-Mallory) and morphometry were used.Results and discussion. As a result of the study, it was found that the four-fold administration of alprostadil had a significant effect on the structure of the dense fibrous connective tissue of the plantar foot of laboratory animals. The mechanisms of damage (edema, microhemorrhages, infiltration by lymphocytes, plasmocytes and leukocytes, dystrophy by the type of mucoid and fibrinoid swelling, delamination and rupture of collagen fibers), adaptation and regeneration (the appearance of a large number of activated fibrocytes, fibroblasts, microvessels, neoplasm of collagen fibers) were activated. All this together led to spatial focal histotopographic changes, consisting in an increase in the cellular composition of connective tissue structures against the background of a noticeable violation of their spatial orientation.Conclusion. Modeling of fasciopathy using alprostadil was accompanied by the appearance of mosaic reversible and irreversible heteromorphic and heterochronous changes in all connective tissue aponeurotic structures. Such histotopographic changes should be considered as one of the reasons for the clinical manifestations of plantar fasciopathies, causing functional insufficiency and explaining the clinical recurrent nature of the pathological process

This article provides data from the current Russian (National guidelines for the management of patients with diseases of the brachiocephalic arteries of 2013; Recommendations βBlockage and stenosis of the carotid arteryβ of the Ministry of Health of the Russian Federation, 2016) and foreign (European Society of Cardiology / European Society of Vascular Surgeons for Diagnosis and Treatment Peripheral Artery Diseases 2017; Recommendations for myocardial revascularization of the European Society of Cardiology and the European Association of Cardio-Thoracic Surgeons 2018) recommendations regarding the choice of a revascularization strategy for combined coronary and carotid artery disease. Conclusions are drawn about the unresolved issue. A literature review of the largest series of Russian articles by one institution devoted to this topic was carried out. Hospital and long-term outcomes have been demonstrated, as well as predictors of complications for various revascularization strategies. The stages of creation and the results of approbation of a new computer program for risk stratiο¬ cation, which makes it possible to determine the mathematical probability of the development of unfavorable cardiovascular events during the implementation of various surgical tactics, taking into account the individual characteristics of the patient. Influence of a high-frequency pulsed nanosecond diffusion discharge in the nitrogen atmosphere on the electrical characteristics of a CdHgTe epitaxial films
The effect of a high-frequency nanosecond volume discharge forming in an inhomogeneous electrical field at atmospheric pressure on the CdHgTe (CMT) epitaxial films is studied. The measurement of the electrophysical parameters of the CMT specimens upon irradiation shows that that the action of pulses of nanosecond volume discharge leads to changes in the electrophysical properties of CMT epitaxial films due to formation of a near-surface high-conductivity layer of the n-type conduction. The preliminary results show that it is possible to use such actions in the development of technologies for the controlled change of the properties of CMT narrow-band solid solutions and production of structures heterogeneous with respect to conduction
Prognosis model and rationalization of infusing-transfusing therapy of massive obstetric bleeding in the perinatal center
Aim: to perform multifactor retrospective analysis of the reasons of maternal mortality because of massive bleedings in obstetrics in order to work out the rules of prognosis and definition of the quality and quantity of infusing-transfusing therapy in the perinatal center. This research is based on the data of obstetrical institutions of Sverdlovsk region. Material and methods: using total choice method in the period 2002- 2006yy the retrospective analysis of 88 cases of labor has been performed. All patients were divided on two main and one control groups. The main groups consisted of the patients with massive bleeding in labor: the first group (21 women β groupA) with lethal outcome and the second group with beneficial outcome (37women β group B). 40 patients with physiological blood loss were included into the control group (group C). Results: The blood loss in the group A was reliably higher (3580+1097 ml) in comparison with group Π (2123+808) due to late surgical hemostasis. Infusing-transfusing therapy was different in groups A and B: volume of blood transfusion was lower in group A and quantity of colloids was higher.
