    A simple synthetic strategy is described for synthesis of the hitherto unreported 5-arylazo-1,3-diphenyl-6-substituted-1H-pyrazolo[3,4-d]pyrimido[1,6-b][1,2,4]triazines 5a-n. The spectral data indicated that the studied compounds exist predominantly in the hydrazone tautomeric form 5A. The site-selectivity and mechanism of the studied reactions are discussed

    C-reactive protein as a screening test for gestational diabetes mellitus in first-trimester of pregnancy: a prospective cohort study

    Background: The current study aims to determine if increased inflammation identified by increased C-reactive protein (CRP) level is associated with the subsequent development of gestational diabetes mellitus (GDM).Methods: A prospective cohort study conducted at a tertiary University Hospital included 496 booking for their antenatal care visit between 10-12 weeks of gestation. They were tested for presence of CRP in blood. All women without a history of antenatal type 1 or type 2 diabetes undergo routine GDM screening with the 50-g oral glucose-loading test (GLT) between 24 and 28 weeks 'gestation. Women whose 1-h post-loading plasma glucose level >7.8 mmol/l (>140 mg/dl) undergo a diagnostic, fasting, 100-g, 3-h oral glucose tolerance test (OGTT) within 1-2 weeks following the GLT.Results: Among these 496 women, a total of 27 women (5.4%) were diagnosed as GDM and 10 women (2.0%) were diagnosed as IGT at week 24-28. The CRP was positive and higher in the GDM diagnosed group (p=0.000) compared to those who did not develop GDM. CRP was positively correlated with diagnosis of GDM (r=0.438, p=0.012).Conclusions: There is an association between first-trimester inflammation, marked by increased CRP levels and subsequent risk of GDM

    Effect of excess boron on growth, membrane stability, and functional groups of tomato seedlings

    With the scarcity of good quality water, plants like tomatoes will be more susceptible to excess boron (EB) in Mediterranean regions. The effects of EB on the growth, free, semi-bound, and bound boron (B) concentrations, and macromolecules of the Solanum lycopersicum L. cultivar Castle Rock, were investigated in this study. Seedlings were exposed to four levels of EB using boric acid. The results manifested that EB inhibited tomato growth, total water content, and photosynthetic pigments. EB harmed the membrane stability, as seen by increased potassium (K) leakage, UV absorbance metabolites, and electrolyte conductivity (EC) in leaf disc solution. EB raised concentrations of free, semi-bound, and bound forms of B in seedlings. Fourier transform infrared spectroscopy (FTIR) data revealed that EB induced uneven wax deposition, altered the shape of cell walls, and lowered cellulose synthesis in seedlings. EB affected the amide I and amide II indicating damage to the protein pools. These results provide new insights into understanding the specific effects of EB on the functional groups of different macromolecules of tomato seedlings

    A COMPARISON BETWEEN THE EFFECT OF DIFFERENT PRESSURES OF AIR PARTICLE ABRASION BEFORE AND AFTER SINTERING WITH AND WITHOUT ZIRCONIA LINER ON BOND STRENGTH OF RESIN TO ZIRCONIA SURFACE

    This study aimed to compare the effect of different pressures of air particle abrasion 2 and 4 bars before and after sintering with and without zirconia liner on SBS of resin to zirconia Surface. Materials and Methods: 54 zirconia cuboids 8×8×3 mm were milled and divided into 6 equal groups (N=9) according to surface treatment; sintered with ceramic liner application and 2 bars pressure (SL2) and with 4 bars pressure (SL4), sintered with no ceramic liner and 2 bars pressure (SN2) and 4 bars pressure (SN4), unsintered with no ceramic liner and 2 bars pressure (UN2) and 4 bars pressure (UN4). Specimens underwent thermocycling (1000 cycles) between 5 ̊ and 55 ̊ Celsius with dwell time of 10 seconds, and submitted to SBS test. Data was statistically analysed. Results: All 4 bars APA groups showed higher SBS than 2 bars groups with significant difference between SL4 group (4.7±2.07) and SL2 group (2.58±1.94). Groups with APA before sintering (12.42±2.56) showed significantly higher SBS than APA after sintering (6.35±3.7). SL2 group showed least SBS with significant difference (2.58±1.94) compared to UN2 (11.83±3.35) and SN2 (7.87±2.12). SL4 group (4.7±2.07) showed the least SBS compared to the UN4 (13±1.38), and SN4 (10.24±2.94) groups. Conclusion: It was found that APA of zirconia surface before sintering yielded superior performance than APA after sintering, also 4 bars APA pressure application showed higher SBS than 2 bars pressure, application and firing of ceramic liner onto zirconia surface didn’t enhance SBS

    Prevalence of cesarean section on demand in Assiut Governorate, Egypt

    Background: The current study aims to evaluate the prevalence of CS on demand in Women's health hospital, Assiut University and Abnob Central Hospital in Assiut Governorate, Egypt.Methods: A cross sectional study conducted in Assiut Women Health Hospital and Abnob central hospital from January 2017 to December 2017. The total number of cesarean section done was 180 cases and the number of CS on demand was 64 (35.6%). The demographic data were collected by one of the study investigators. Women were asked about the causes of requesting CS before surgery.Results: The study group was 64 women with age ranging from 18-40 years old, 40 primipara and 24 multipara. Of those 24 women, 21 of them previously delivered vaginally and only 3 women delivered by emergency CS. Twenty- six women had a history of previous abortion. Fear of pain was the main cause for CS on demand in the whole study participants (57.8%). In primipara, the main cause for requesting CS is fear of pain in 62.5% of participants followed by fear on the baby in 45 % of women. On the other hand, in multipara, the main cause for CS on demand was bad history of previous experience (60%) followed by fear of pain in 50% of cases. There was statistical significant difference between both groups in only two causes; fear of pelvic floor injuries (50% in multipara vs. 20% in primipara, p=0.02) and bad history of previous experience (60% in multipara vs. 0% in primipara, p=0.001). Other causes were not statistically different.Conclusions: The incidence of cesarean sections performed on request without medical indications is rising. The reasons for this are not only for perceived medical benefit, but also due to social, cultural, and psychological factors

    STRESS ANALYSIS COMPARING EFFECT OF TWO DIFFERENT CAD-CAM IMPLANT SUPERSTRUCTURE MATERIALS (IN-VITRO STUDY)

    Purpose: This study was conducted to evaluate the micro-strain around dental implant using two different CAD/CAM crown fabricated materials through strain gauge analysis. Materials and Methods: Five dental implants were fixed in a 5 previously drilled solid rigid polyurethane test blocks in the edentulous area with neighboring abutments mesial and distal printed out using special 3D Dental printer, Cyanoacrylate adhesive was then used to fix the printed-out part on to the polyurethane test blocks, creating the bounded saddle replicas. Ten CAD/CAM screw retained crowns were fabricated; five Enamic crowns from vita enamic blocks, and five zirconia crowns from presintered katana zirconium blocks. Each crown was cemented to abutment and screwed over the implant fixture. Two strain gauges were installed on their corresponding prepared sites to measure the micro-strains in the medium surrounding the implant. For each tested implant, loads were applied by a universal testing machine, micro-strains were recorded with the strain gauges and stress distribution around the implant was statistically evaluated. Results: Micro-strain recording revealed a statistically significant difference in mean micro-strain recording applied in central fossa between Zirconia and Enamic, for both the buccal and lingual measurements, Enamic was significantly lower than Zirconia. Conclusion: The modulus of elasticity of restorative materials has a meaningful effect on forces applied to dental implant and transmitted to the supporting bone

    BIOPROCESIRANJE PRIRODNE FOSFATNE RUDE BAKTERIJOM STAPHYLOCOCCUS AUREUS

    Phosphate ores are in high demand around the world because they are the primary raw materials used in the manufacturing of phosphatic fertilizers and other chemicals. Since the grade of the ore is gradually declining, it is becoming economically viable to mine and beneficiate numerous lower-grade deposits, and a significant number of precious minerals are discarded due to the inadequacy of new technological advances. Thus, biological processes are becoming more appealing in mineral processing due to their lower operating costs and potential applications to beneficiate low-grade complex ores through the interaction of bacteria and mineral surfaces, resulting in surface modification and mineral separation via bio-flotation. Staphylococcus aureus was supplied by the mineral bioprocessing lab, CMRDI. Bacterial adhesion measurements revealed a higher affinity for apatite than quartz. A binary mixture containing 12.5% P2O5 and 42.5% SiO2 yielded a concentrate containing 20.15% P2O5 and 33.5% SiO2. Using the bio-flotation process with Staphylococcus aureus and the optimal conditions on natural phosphate ore, a concentrate with 30.25% P2O5 was obtained from a feed containing 21.89% P2O5.Diljem svijeta velika je potražnja za fosfatnom rudom s obzirom na to da je riječ o primarnoj sirovini koja se koristi u proizvodnji fosfatnih gnojiva i drugih kemikalija. Budući da se koncentracija rude postupno smanjuje, ekonomski je sve isplativije eksploatirati i oplemenjivati brojna ležišta niže koncentracije s obzirom na to da se znatne količine korisne mineralne sirovine nedovoljno iskorištavaju zbog neadekvatnosti novih tehnologija. Stoga biološki procesi postaju sve zanimljiviji u oplemenjivanju SiO2 mineralnih sirovina zbog nižih operativnih troškova i potencijalne primjene za obogaćivanje kompleksnih ruda niskoga stupnja koncentracije, i to interakcijom bakterija i površine minerala, što rezultira površinskom modifikacijom i odvajanjem minerala putem bioflotacije. Bakteriju Staphylococcus aureus isporučio je laboratorij za bioprocesiranje minerala, CMRDI. Mjerenja bakterijske adhezije pokazala su veći afinitet za apatit nego za kvarc. Binarna smjesa koja je sadržavala 12,5 % P2O5 i 42,5 % SiO2 dala je koncentrat koji je sadržavao 20,15 % P2O5 i 33,5 % SiO2. Postupkom bioflotacije bakterijom Staphylococcus aureus pri optimalnim uvjetima na prirodnoj fosfatnoj rudi dobiven je koncentrat s 30,25 % P2O5 iz sirovine koja je sadržavala 21,89 % P2O5

    Insulin sensitizing agent improves clinical pregnancy rate and insulin resistant parameters in polycystic ovarian syndrome patients with acanthosis nigricans: a randomized controlled study

    Objective: To investigate the effect of adding metformin to clomiphene citrate (CC) in polycystic ovarian syndrome (PCOS) patients with acanthosis nigricans (AN) who were previously not responding to CC. Material and Methods: A double blinded randomized controlled trial (NCT02562664) included 66 PCOS women with acanthosis nigricans who were CC resistant (at least 3 months). Day 3 follicle stimulating hormone (FSH) level, fasting insulin, fasting glucose and homeostatic model assessment were used to quantify insulin resistance. Participants were randomly assigned to either group I (CC with placebo tablets) or group II (CC with metformin) for three cycles. Insulin resistance parameters as well as clinical pregnancy rate had been evaluated in both groups. The statistical analysis was done using Chi- square and Fischer exact tests. Results: The demographic data was comparable in both groups, however; there was higher cumulative pregnancy rate after three cycles of stimulation in group II (18/33) (54.5%) in comparison with group I (7/33) (21.1%) (P=0.03). There was a significant improvement in the insulin resistance parameters after three months of combining clomiphene citrate with metformin as compared with CC alone. Conclusion: Adding metformin to CC in clomiphene citrate resistant PCOS patients who have acanthosis nigricans improves the pregnancy rate and insulin resistant parameters

    A comparison of periodontal disease among rural Amish and non-Amish adults

    Periodontal disease can be more efficiently studied within a homogeneous population where genetic influences and lifestyles are similar enough to negate their effect on the disease process. This study focuses on an Amish population in southern Michigan who isolate themselves from outside influences and their non-Amish neighbors. A total of 425 Amish and 290 non-Amish were contacted resulting in 330 Amish and 215 non-Amish who were examined in their homes giving a participation rate of 76.2%. Ages ranged from 18 to 82 years. Prevalence of periodontal disease tended to be higher among males and increased with age. There were slightly more Amish females (52%) than non-Amish (49%). Means of periodontal conditions for Amish were 1.35 mm for attachment loss, 2.59 mm for pocket depth, 0.24 for calculus, 0.77 for plaque and 0.74 for gingivitis. For non-Amish, the means were 1.03 mm for attachment loss, 2.38 mm for pocket depth, 0.40 for calculus, 0.95 for plaque and 0.87 for gingivitis. It is of interest that the Amish do not practice routine oral hygiene. Only 36.8% of Amish reported brushing at least 1 × a day compared to 84.6% of non-Amish.     Population-based study of acquired cerebellar ataxia in Al-Kharga district, New Valley, Egypt

A door-to-door survey was used to identify cases of acquired cerebellar ataxia. Patients with acquired cerebellar ataxia at any age and of both genders were included. Cases of known inherited cerebellar ataxia, acquired neurological disorders with ataxia as a minor feature, or pure acquired sensory ataxia, were excluded.Results: We identified 17 cases of acquired ataxia, of which eight were vascular, six were an ataxic cerebral palsy subtype, and three involved postencephalitic ataxia. The crude prevalence rate for acquired ataxia was 27.16/100,000 (95% confidence interval [CI]: 14.3–40.1). The mean age of the patients at interview was 31.8 (range 4–72) years, with a male to female ratio of 2.1:1. The most frequent presenting complaint was disturbance of gait (90.7%). The majority (92%) were ambulatory, but only 9.3% were independently self-caring.Conclusion: This population-based study provides an insight into acquired cerebellar ataxia within a defined region, and may inform decisions about the rational use of health care resources for patients with acquired cerebellar ataxia.
