Efficacy of Topical Corticosteroid Monotherapy in Inducing and Maintaining Clinical and Histologic Remission in Adolescent and Adult Patients with Eosinophilic Esophagitis: a Systematic Review and Meta-Analysis
Background: Eosinophilic esophagitis (EoE) is the second most common cause of esophagitis. Topical steroids represent a promising group of drugs for inducing and maintaining clinical and histological remission in these patients.Objective. To evaluate the effectiveness of topical steroids in inducing and maintaining clinical and histological remission in adolescent and adult patients with EoE.Methods. A systematic literature search using defined keywords was performed up to March 20, 2021 in the MEDLINE / PubMed, EMBASE (Excerpta Medica), and Cochrane Central Register of Controlled Trials, ClinicalTrial.gov databases.Results. 390 patients from 5 studies were included in this systematic review with meta-analysis. The meta-analysis showed that topical steroids, compared with placebo, was more effective in inducing (odds ratio (OR) 75.77; 95 % confidence interval (CI): (21.8; 263.41), p < 0.001) and maintaining complete histological remission (OR 103.65; 95 % CI: (36.05; 298.01), p < 0.001) in patients with EoE. Also, topical steroids significantly relieved disease symptoms compared with placebo in inducing and maintaining clinical remission (OR 4.86; 95 % CI: (1.4; 16.86), p = 0.01) and (OR 11.06; 95 % CI: (4.62; 26.45), p < 0.001) respectively.Conclusions. Topical steroids represent an effective group of drugs for inducing and maintaining histologic and clinical remission in adolescent and adult patients with EoE
Severe Diverticulitis Associated to <i>Clostridioides difficile</i> Infection in a 91 Year Old Patient (Clinical Case)
Aim. To present a clinical case of a 91-year-old patient with a severe course of diverticulitis combined with the development of Clostridioides difficile-associated disease.Key points. On admission the patient complained of pain in the left iliac region, increased body temperature, constipation and bloating. The medical history showed that constipation increased on the background of prolonged bed rest and discontinuation of psyllium. According to the laboratory and instrumental examinations, the patient had signs of acute diverticulitis, antibacterial therapy was corrected twice, positive dynamics of the condition was noted. However, a few days later, the patient developed a clinic of C. difficile-associated disease, which required the prescription of anticlostridial therapy (vancomycin), until the laboratory confirmation of the accession of this infection was obtained. Combined therapy of exacerbation of diverticular disease and C. difficile-associated disease made it possible to achieve a steady improvement of the condition.Conclusion. The exclusion of possible development of C. difficile-associated disease on the background or prior antibiotic therapy is an important condition for correct and adequate management of a patient with exacerbation of diverticular disease. If the patient develops a clinical picture of C. difficile-associated disease, treatment may be initiated before laboratory confirmation

New Parameters for Impedance–pH Monitoring in Diagnosing Gastroesophageal Reflux Disease and Predicting its Severity
Aim. The present articleidentifies possible correlations between new parameters for impedance–pH monitoring, such as mean nocturnal baseline impedance (MNBI); post-reflux swallow-induced peristaltic wave index (PSPW); and the main parameter, acid exposure time (AET), as well as esophageal motor function. The authors set out to assess the values of MNBI and the PSPW index as additional criteria improving the diagnostic efficacy of impedance-pH monitoring, the completeness of clinical phenotyping of gastroesophageal reflux disease (GERD), as well as determining the probability of a more severe disease course. Materials and methods. A total of 60 patients aged 19 to 71 (mean age 44.7 years) participated in the study: 30 patients with erosive reflux disease (ERD), 30 with non-erosive reflux disease (NERD) and 20 healthy volunteers aged 26 to 65 (mean age 45.2 years). All of them underwent 24-hour combined esophageal impedance–pH monitoring (Gastroscan-IAM, JSC RPE Istok-Sistema, Fryazino) and high-resolution esophageal manometry using a 22-channel water-perfusion catheter (Solar GI MMS, The Netherlands). The authors studied such parameters as AET, GER number, MNBI level, PSPW index, distal contractile integral (DCI), resting pressure in the lower esophageal sphincter (LES), peristaltic break. Statistical processing was performed using Statistica for Windows 10.0 (StatSoft Inc.) and Prism 8 (GraphPad).Results. The examination of patients revealed that MNBI, the PSPW index and DCI significantly decrease with the development of more severe GERD (r = -0.79; p = 0.0000, r = -0.4; p = 0.0002, r = -0.49; p = 0.0000, respectively). A negative correlation was found between AET and the PSPW index (r = -0.38; p = 0.0003) and the MNBI level (r = -0.59; p = 0.0000). A correlation was determined between the value of MNBI and the following parameters: LES resting pressure (r = 0.26; p = 0.0006), DCI (r = 0.35; p = 0.00004), peristaltic break (r = -0.21; p = 0.007), the PSPW index (r = 0.41; p = 0.0000), and GER number (r = -0.59; p = 0.0). A correlation between the PSPW index and DCI (r = 0.22; p = 0.001) was found as well.In the group of ERD patients, the median values of PSPW and MNBI constitute 0.23 [0.17; 0.33] and 1.13 [0.63; 1.53], respectively. Also, a correlation between AET and the MNBI level was found in this group of patients (r=-0.53; p=0.000036). In turn, MNBI correlated with LES resting pressure (r = 0.46; p = 0.0004), DCI (r = 0.36; p = 0.005), peristaltic break (r = -0.37; p = 0.004), and GER number (r = -0,42; p = 0,0000).In the group of NERD patients, the median values of PSPW and MNBI constitute 0.56 [0.51; 0.75] and 3.3 [2.57; 4.8], respectively. A correlation was also found between the MNBI level and AET (r = -0.35; p = 0.005), GER number (r = -0.39; p = 0.00005), as well as between AET and the PSPW index (r = -0.26; p = 0.0000).In the control group, the median values of PSPW and MNBI constitute 0.42 [0.3; 0.5] and 5.83 [5.21; 6.48], respectively. A statistically significant difference (p = 0.02) was found between the median values of MNBI in patients with ERD, NERD, and the control group. A statistically significant difference (p = 0.0) between the median values of the PSPW index in patients with ERD, NERD and the control group was found as well.Conclusions. The revealed correlations between AET, esophageal motor function and new parameters for impedance-pH monitoring allow them to be used as additional criteria improving the diagnostic efficacy of impedance-pH monitoring and the completeness of GERD clinical phenotyping. A decrease in the level of these parameters in patients reflects the probability of a more severe disease course

Clinical Recommendations of the Russian Gastroenterological Association on Diagnosis and Treatment of Infectious Esophagitis
Aim. Current clinical recommendations aim to provide gastroenterologists, general practitioners (family doctors), endoscopists and infectionists with modern methods for diagnosis and treatment of infectious esophagitis.Key points. Infectious esophagitis is an esophageal disease of fungal, viral, bacterial or parasitic origin. Esophagus may be affected per se or conjointly in a common gastrointestinal infection. All patients suspected for infectious esophagitis, unless contraindicated, are advised an esophagogastroduodenoscopy with biopsy to confirm diagnosis. Selected incidents of infectious esophagitis require a histological, immunohistochemical examination or polymerase chain reaction-based diagnosis to verify the infectious agent. Uncomplicated infectious esophagitis requires a conservative therapy, mostly on an outpatient basis. However, cases of severe odynophagia, dysphagia, severe pain syndrome, high complication risks (e.g., esophageal ulcer bleeding in thrombocytopenia), severe immunodeficiency, generalised forms of disease and severe concomitant disorders are considered for hospital care. The clinical recommendations outline criteria for the medical care quality assessment and provide relevant information to the patient.Conclusion. Diagnosis of infectious esophagitis capitalises on the clinical picture (odynophagia, dysphagia), presence of immunosuppression, endoscopic and histological evidence. All patients with infectious esophagitis of verified origin are recommended a suitable etiotropic therapy
