Can care of elderly be measured? A method for estimating the individual care of recipients in community health care
<p>Abstract</p> <p>Background</p> <p>Almost every country in the Western world has great difficulties allocating enough financial resources to meet the needs in the care of the increasing elderly population. The main problem is common to all countries and concerns the efforts to meet elderly persons' needs on an individual level while still maintaining society's responsibility for distributing justice. The aim of this study is to elaborate an instrument for measuring the quality of individual care and staff's working time in order to allocate public resources fairly. The present study gives an account of a new classification system named TiC (Time in Care), indicating how it can be used most effectively and also investigating the validity and reliability of the system.</p> <p>Methods</p> <p>All recipients in 13 sheltered homes for elderly care (n = 505) in a Swedish municipality were surveyed regarding the care they needed, in dimensions of General Care, Medical Care, Cognitive Dysfunction and Rehabilitation, and the time required. Construct validity was assessed by means of factor analysis. The inter-rater agreement of two raters concerning 79 recipients was measured using weighted Kappa. The stability of the instrument and its sensitivity to change were investigated through test-retest reliability measurements, conducted once a month during a six-month period. The content validity of the instrument was also assessed.</p> <p>Results</p> <p>Factor analysis resulted in a reduction of the number of items from 25 to 16 in three dimensions: General Care, Medical Care and Cognitive Dysfunction. The Kappa analysis showed satisfactory to excellent inter-rater agreement. The care need scores were basically stable but showed sensitivity to change in health status.</p> <p>Conclusion</p> <p>The instrument was found to be useful and reliable for assessing individual needs in community health care.</p
Low-Prandtl-number B\'enard-Marangoni convection in a vertical magnetic field
The effect of a homogeneous magnetic field on surface-tension-driven
B\'{e}nard convection is studied by means of direct numerical simulations. The
flow is computed in a rectangular domain with periodic horizontal boundary
conditions and the free-slip condition on the bottom wall using a
pseudospectral Fourier-Chebyshev discretization. Deformations of the free
surface are neglected. Two- and three-dimensional flows are computed for either
vanishing or small Prandtl number, which are typical of liquid metals. The main
focus of the paper is on a qualitative comparison of the flow states with the
non-magnetic case, and on the effects associated with the possible
near-cancellation of the nonlinear and pressure terms in the momentum equations
for two-dimensional rolls. In the three-dimensional case, the transition from a
stationary hexagonal pattern at the onset of convection to three-dimensional
time-dependent convection is explored by a series of simulations at zero

Use of patientsâ classification instruments: analysis of the brazilian production of knowledge
Objective To analyze the production of scientific knowledge about the use of patientsâ classification instruments in care and management practice in Brazil. Method Integrative literature review with databases search in: Latin American and Caribbean Literature on Health Sciences (LILACS), Medical Literature Analysis and Retrieval System on-line (MEDLINE), Cumulative Index to Nursing and Allied Health Literature (CINAHL) and SCOPUS, between January 2002 through December 2013. Results 1,194 studies were found, 31 met the inclusion criteria. We observed a higher number of studies in the category care plans and workload (n=15), followed by the category evaluation of psychometric properties (n=14). Conclusion Brazilian knowledge production has not yet investigated some purposes of using instruments for classifying patients in professional nursing practice. The identification of unexplored areas can guide future research on the topic
