    Abstrak: Arus globalisasi saat ini tidak dapat dipungkiri terdapat fasilitas-fasilitas bagus dan modern yang sangat diminati oleh khalayak. Sehingga mampu menggeser segala sesuatu yang dianggap ketinggalan jaman dengan lebih memilih dan mengedepankan hal-hal yang dianggap lebih kekinian. Misalnya di daerah-daerah yang saat ini berlomba-lomba membangun pariwisata yang menghadirkan unsur modern dengan segala fasilitas kekinian yang dapat dinikmati oleh para wisatawan. Pembangunan pariwisata sangat menjadi prioritas utama sebagai upaya untuk kemajuan dan peningkatan terhadap potensi yang dimiliki, baik dari masyarakat maupun daerah. Hadirnya destinasi wisata modern yang juga mengusung pasar tradisional papringan dengan memasukkan budaya Jawa sebagai upaya untuk melestarikan serta mengenalkan kepada para pengunjung, menjadi tujuan utama masyarakat yang ada di Desa Jambu. Segala wujud tindakan yang dilakukan oleh masyarakat menurut Parson memiliki tujuan yang ingin dicapai, sehingga dengan memanfaatkan dan mengembangkan segala potensi yang dimiliki, kini desa tersebut menjadi destinasi wisata yang banyak dikunjungi oleh wisatawan baik dari dalam maupun luar kota. Adanya pasar tradisiona,l juga memberikan kesempatan bagi para wanita yang notabennya adalah ibu rumah tangga untuk bisa lebih mandiri dalam memenuhi kebutuhan keluarga. Pada era modernisasi saat ini dianggap memberikan kemudahan untuk dapat mempromosikan segala keunikan dan keistimewaan yang dimiliki oleh desa wisata tersebut. So as to be able to shift everything that is considered outdated by preferring and prioritizing things that are considered more contemporary. For example in areas that are currently competing to build a tourism business that presents a modern element with all the current facilities that can be enjoyed by tourists. Tourism development is a top priority as an effort to progress and increase the potential of the community and region. The presence of modern tourist destinations that carry Papringan traditional markets by incorporating Javanese culture as an effort to preserve and introduce to visitors, became the main destination of activists in Jambu Village. All forms of actions taken by the community according to Parson have goals to be achieved. So that by utilizing and developing all the potential that is owned, the village has now become a tourist destination that is visited by many tourists both from within and outside the city. The existence of traditional markets also provides opportunities for women who incidentally are housewives to be more independent in meeting family needs. In the current era of modernization, it is considered to be easy to be able to promote all the uniqueness and privileges that are owned by the tourist village.     Differences in delivery of prehospital ambulance care comparing non-white versus white patients with suspected cardiac chest pain: cross-sectional study

    Introduction Quality implies equitable care irrespective of ethnicity. There have been few previous studies investigating quality of prehospital cardiac care by ethnicity. We aimed to investigate whether prehospital care for suspected cardiac pain varied by ethnicity. Methods We conducted a cross-sectional analysis of retrospective electronic clinical data for patients with chest pain over one year (August 2011 to July 2012) extracted from a single regional ambulance service. This included patient demographic data (ethnicity, age, sex, deprivation), clinical measurements (blood pressure, respiratory rate, pain assessment, temperature, blood glucose, oxygen saturation), drugs (aspirin, nitroglycerin, Entonox, morphine) and outcomes such as transportation to hospital or referral to primary care. We used multivariate regression to investigate differences in care by ethnicity comparing non-white with white patients. Results There were 7046 patients with suspected cardiac chest pain, with 4825 who had ethnicity recorded including 4661 (96.6%) white, 164 (3.4%) non-white (2221 patients had missing data for ethnicity). Non-white patients were similar in sex (p=0.63) and socioeconomic group (p=0.07) but significantly younger in age (p<0.001) than white patients. After correcting for age, sex, socioeconomic status and whether transported to hospital, non-white patients were similar to white patients in recording of blood pressure, pain score or electrocardiogram but significantly more likely to have temperature (77.4 vs.69.8%), blood glucose (78.7 vs. 69.4%), and oxygen saturation (85.4 vs. 80.7%) recorded. There were no differences in aspirin, nitroglycerin, Entonox or morphine treatment but non-white patients were less likely than white patients to be transported to hospital (93.3 vs. 94.4, p=0.02). Conclusion We found differences in prehospital ambulance care for non-white compared with white patients with cardiac pain that could be due to recording bias, varying clinical condition or provider management. Further analysis should involve larger and more complete datasets to explore ethnic differences in greater detail

    Charging induced asymmetry in molecular conductors

    We investigate the origin of asymmetry in various measured current-voltage (I-V) characteristics of molecules with no inherent spatial asymmetry, with particular focus on a recent break junction measurement. We argue that such asymmetry arises due to unequal coupling with the contacts and a consequent difference in charging effects, which can only be captured in a self-consistent model for molecular conduction. The direction of the asymmetry depends on the sign of the majority carriers in the molecule. For conduction through highest occupied molecular orbitals (i.e. HOMO or p-type conduction), the current is smaller for positive voltage on the stronger contact, while for conduction through lowest unoccupied molecular orbitals (i.e. LUMO or n-type conduction), the sense of the asymmetry is reversed. Within an extended Huckel description of the molecular chemistry and the contact microstructure (with two adjustable parameters, the position of the Fermi energy and the sulphur-gold bond length), an appropriate description of Poisson's equation, and a self-consistently coupled non-equilibrium Green's function (NEGF) description of transport, we achieve good agreement between theoretical and experimental I-V characteristics, both in shape as well as overall magnitude.Comment: length of the paper has been extended (4 pages to 6 pages), two new figures have been added (3 figures to 5 figures), has been accepted for PR

    Optimal Imperfection of Reactive Powder Concrete Slabs under Impact Load

    Optimization of imperfect reactive powder concrete slabs under impact load is the aim of the current study, the study adopts to investigate the optimal imperfection to decrease the maximum dynamic deflection which affects negatively on structures. Optimization by using Modified Hooke-Jeevs method of imperfect slabs has been adopted in three cases according to the required variables, optimal thickness, optimal thickness-length and optimal thickness-length-imperfection. The results show that the optimal ratio of imperfect reactive powder concrete slab thickness to the one of its dimension is equal to (0.049) whereas the optimal ratio of imperfection to the thickness of slab is equal to (0.785) . The current study adopted preparing designable table which is considered informative for any future designs

    Thermoelectric properties of Bi2Te3 atomic quintuple thin films

    Motivated by recent experimental realizations of quintuple atomic layer films of Bi2Te3,the thermoelectric figure of merit, ZT, of the quintuple layer is calculated and found to increase by a factor of 10 (ZT = 7.2) compared to that of the bulk at room temperature. The large enhancement in ZT results from the change in the distribution of the valence band density of modes brought about by the quantum confinement in the thin film. The theoretical model uses ab initio electronic structure calculations (VASP) with full quantum-mechanical structure relaxation combined with a Landauer formalism for the linear-response transport coefficients.Comment: 4 figures, submitted to AP

    Leadership, innovation and engagement in quality improvement in the Ambulance Services Cardiovascular Quality Initiative: cross sectional study

    Introduction: Clinical leadership and organisational culture are important contextual factors for successful Quality Improvement (QI) programmes. The relationship between these and with organisational performance is complex and poorly understood. We aimed to explore the relationship between leadership, culture of innovation, and clinical engagement in QI for organisations participating in a large-scale national ambulance Quality Improvement Collaborative (QIC). Methods: We used a cross sectional survey design. An online questionnaire was distributed to 22,117 frontline ambulance staff across all 12 ambulance services in England. Scores (0-100%) were derived for each key aspect: clinical leadership; culture of innovation; use of QI methods; and effectiveness of QI methods. Responses to an open-ended question were analysed and complemented the quantitative findings. Results: There were 2,743 (12%) responses from 11 of 12 participating ambulance services. Despite only a small proportion of responders (3%) being directly involved with ASCQI, leadership behaviour was significantly higher for ASCQI members than for non-ASCQI members. Involvement in ASCQI was not significantly associated with responders’ perceptions of the culture of innovation of their organisation, which was generally considered to be poor. ASCQI members were significantly more likely to use QI methods but overall uptake of QI methods was low. The use of QI methods was also significantly associated with leadership behaviour and service tenure. Limitations: There was a low response rate, although sufficient responses to enable comparison of those who participated in ASCQI with those who did not. Conclusion and recommendations: Although participants reported a lack of organisational culture of innovation, considered a prerequisite for QI, the collaborative achieved significant wide-scale improvements in prehospital care for myocardial infarction and stroke. We postulate that improvement was mediated through a ‘QI subculture’ developed from ASCQI’s distributed leadership and network. Further research is needed to understand success factors for QI in different complex healthcare environments
