Avances en la inmunosupresión para el trasplante renal. Nuevas estrategias para preservar la función renal y reducir el riesgo cardiovascular
The development of new immunosuppressants for renal transplantation is aimed not only at improving short-term outcomes, but also at achieving better safety, cardiovascular, and metabolic profiles and at decreasing nephrotoxicity. Belatacept is a fusion protein that inhibits T cell activation by binding to CD80 and CD86 antigens. Clinical trials, particularly the BENEFIT and BENEFIT-EXT studies, have shown that belatacept preserves function and structure in renal grafts. The effects of belatacept provide long-term, sustained results, and the safety and efficacy of this drug have been demonstrated in cases of renal transplantation from expanded criteria donors. Compared to calcineurin inhibitors, belatacept is associated with a lower incidence of chronic allograft nephropathy and a more favourable cardiovascular and metabolic profile
Effectiveness of individualized inhaler technique training on low adherence (LowAd) in ambulatory patients with COPD and asthma

This document is the published version of a published work that appeared in final form in npj primary care respiratory medicineTo analyze whether there is improvement in adherence to inhaled treatment in patients with COPD and asthma after an educational intervention based on the teach-to-goal method. This is a prospective, non-randomized, single-group study, with intervention and before-after evaluation. The study population included 120 patients (67 females and 53 males) diagnosed with asthma (70.8%) and COPD (29.1%). The level of adherence (low and optimal) and the noncompliance behavior pattern (erratic, deliberate and unwitting) were determined by the Test of the adherence to Inhalers (TAI). This questionnaire allows you to determine the level of adherence and the types of noncompliance. Low Adherence (LowAd) was defined as a score less than 49 points. All patients received individualized educational inhaler technique intervention (IEITI). Before the IEITI, 67.5% of the patients had LowAd. Following IEITI, on week 24, LowAd was 55% (p = 0.024). Each patient can present one or more types of noncompliance. The most frequent type was forgetting to use the inhaler (erratic), 65.8%. The other types were deliberate: 43.3%, and unwitting: 57.5%. All of them had decreased on the final visit: 51.7% (p = 0.009), 25.8% (p = 0.002), 39.2% (p = 0.002). There were no significant differences in adherence between asthma and COPD patients at the start of the study. The only predicting factor of LowAd was the female gender. An individualized educational intervention, in ambulatory patients with COPD and asthma, in real-world clinical practice conditions, improves adherence to the inhaled treatment

El libro reúne trabajos en los que se exponen resultados de investigaciones presentadas por investigadores de Argentina, Chile, Brasil, España, Italia y Alemania en el XII Congreso de la Sociedad Argentina de Lingüística (SAL), Bicentenario: la renovación de la palabra, realizado en Mendoza, Argentina, entre el 6 y el 9 de abril de 2010. Las temáticas abordadas en los 167 capítulos muestran las grandes líneas de investigación que se desarrollan fundamentalmente en nuestro país, pero también en los otros países mencionados arriba, y señalan además las áreas que recién se inician, con poca tradición en nuestro país y que deberían fomentarse. Rapid recurrence of ANCA-MPO positive vasculitis after non-heart beating donor renal transplantation
We report a patient with end stage renal disease with lesions compatibles with renal vasculitis antineutrophil cytoplasmic autoantibody (ANCA)-associated in phase of sclerosis that underwent renal transplantation from a non-heart beating donor after one year of haemodialysis treatment, without evidence of active vasculitis. Post-transplantation management was performed according to our protocol in this kind of donors with immunosuppressive treatment based on daclizumab, half-doses of tacrolimus, mycophenolate mofetil and steroids. In the third week the renal biopsy showed an acute necrotizing vasculitis associated with crescent glomerulonephritis. The patient was initially diagnosed of acute vascular rejection and initiated treatment with 6-metilprednisolone and anti-CD3 monoclonal anti-bodies. Two days later he developed a cutaneous purpura and the skin biopsy showed an acute necrotizing vasculitis. The determination of circulating ANCA-anti-myeloperoxidase (MPO) was positive. We initiated treatment with oral cyclophosphamide plus mycophenolate mofetil discontinuation with rapid improvement of cutaneous lesions and initiation of renal function recovery
How to Assess Diabetic Kidney Disease Progression? From Albuminuria to GFR
Diabetic kidney disease (DKD) is one of the most relevant complications of type 2 diabetes and dramatically increases the cardiovascular risk in these patients. Currently, DKD is severely infra-diagnosed, or its diagnosis is usually made at advanced stages of the disease. During the last decade, new drugs have demonstrated a beneficial effect in terms of cardiovascular and renal protection in type 2 diabetes, supporting the crucial role of an early DKD diagnosis to permit the use of new available therapeutic strategies. Moreover, cardiovascular and renal outcome trials, developed to study these new drugs, are based on diverse cardiovascular and renal simple and composite endpoints, which makes difficult their interpretation and the comparison between them. In this article, DKD diagnosis is reviewed, focusing on albuminuria and the recommendations for glomerular filtration rate measurement. Furthermore, cardiovascular and renal endpoints used in classical and recent cardiovascular outcome trials are assessed in a pragmatic way
La representación como estrategia de resolución de problemas
Elaborar materiales para el entrenamiento de los alumnos en estrategias de resolución de problemas. Establecer pautas que permitan utilizar estas estrategias como instrumento metodológico en el área de Matemáticas. Comprobar las posibles diferencias entre sexos. Demostrar la transferibilidad de estas estrategias a otras situaciones. Comprobar las posibles variaciones de CI en la población experimental. Hipótesis de trabajo: utilizar la estrategia de la representación mejora los resultados en la resolución de problemas. 206 sujetos alumnos de octavo de EGB y tercero de ESO perteneceientes a cinco centros del municipio de Cartagena: 4 públicos y 1 privado concertado localizados en zonas urbanas y semiurbanas; resultado de la aplicación de un procedimiento (Mean Matching) para igualar las distribuciones de las variables perturbadoras en los grupos experimental y de control. Varones 38 por ciento y mujeres 62 por ciento. Edad media: 13.4 años. Variables independientes: presencia o ausencia de tratamiento (dicotómica), sexo (dicotómica), grupo al que pertenece: experimental/control (dicotómica). Variable dependiente: resultados obtenidos por los alumnos en las pruebas de Matemáticas, Ciencias Sociales y Ciencias Naturales, teniendo en cuenta el sexo y la utilización o no de la estrategia (variable de frecuencia). Pruebas de Matemáticas, Ciencias Sociales y Ciencias Naturales utilizadas en fase pretest y posttest. Protocolo TEA-2 en las dos fases. Escala de actitud hacia las Matemáticas. Paquete estadísico SYSTAT TABLES. Análisis descriptivo de las variables sexo y grupo: histograma de frecuencias. Análisis de correlaciones utilizando el estadístico Mantel-Haenszel. Análisis de varianza. Análisis descriptivo de la escala de actitud. 1. El uso de estrategias de representación aumenta la capacidad de los alumnos para resolver problemas, sobre todo en el área de Matemáticas y tanto para hombres como para mujeres; 2. Existe transferencia de estrategias para la resolución de problemas a otras áreas y por tanto, una generalización; 3. El entrenamiento ha incidido de forma distinta en hombres y mujeres; 4. Aumento del CI general; 5. El cambio de actitud hacia las Matemáticas no ha podido ser demostrado. El estudio contiene un programa de instrucción que puede enmarcarse dentro del paradigma instruccional del aprendizaje ya que no pretende crear cambios de naturaleza cuantitativa o cualitativa en los sujetos, sino facilitar las condiciones para que se adquieran las estrategias típicas que un experto aplica cuando soluciona un problema.MurciaES
