    Healthcare costs associated with progressive diabetic retinopathy among National Health Insurance enrollees in Taiwan, 2000-2004

    <p>Abstract</p> <p>Background</p> <p>Diabetic retinopathy is one of the most common microvascular complications of diabetes and one of the major causes of adult visual impairment in national surveys in Taiwan. This study aimed to identify the healthcare costs of Taiwan's National Health Insurance program on behalf of diabetic patients with stable or progressive retinopathy.</p> <p>Methods</p> <p>A retrospective cohort study was conducted with 4,988 medication-using diabetic retinopathy subjects ≥ 40 years of age under National Health Insurance Program coverage between 2000 and 2004. Study cohort subjects were recorded as having diabetic retinopathy according to ICD-9-CM codes. States of diabetic retinopathy were strategically divided into stable and progressive categories according to subjects' conditions at follow-up in 2004. Expenditures were calculated and compared for the years 2000 and 2004.</p> <p>Results</p> <p>During the 4-year follow-up (2000 through 2004), 4,116 subjects (82.5%) of 4,988 diabetic subjects were in the stable category, and 872 (17.5%) were in the progressive category. Average costs of those in the normal category increased by US 48fromUS48 from US 1921 in 2000 to US 1969in2004(p=0.594),whereascostsforthoseprogressingfromnormaltononproliferativediabeticretinopathy(NPDR)orproliferativediabeticretinopathy(PDR)increasedbyUS1969 in 2004 (p = 0.594), whereas costs for those progressing from normal to non-proliferative diabetic retinopathy (NPDR) or proliferative diabetic retinopathy (PDR) increased by US 1760, from US 1566in2000toUS1566 in 2000 to US 3326 in 2004 (p < 0.001). The PDR category had the highest average costs at US 3632in2000.TheNPDRtoPDRcategoryexperiencedthegreatestincreaseincostsatUS3632 in 2000. The NPDR-to-PDR category experienced the greatest increase in costs at US 3482, from US 2723in2000toUS2723 in 2000 to US 6204 in 2004 (p = 0.042), and the greatest percentage of increase at 2.3% (2.2% when adjusted by comparing to normal category).</p> <p>Conclusions</p> <p>This large-scale longitudinal study provides evidence that increased healthcare costs are associated with progressive diabetic retinopathy among diabetic NHI enrollees in Taiwan.</p

    Rare cases of mucopolysaccharidosis type I in children with Hurler and Hurler-Scheie syndromes

    Two rare cases of mucopolysaccharidosis (MPS) I in children are presented. In the first case, the ophthalmologist suspected a Hurler syndrome in a child aged 1 year with corneal opacities in both eyes, taking into account the unusual appearance of the child, which gradually formed over the year after birth, retarded psychic and speech development and the presence of hypoxic ischemic central nervous system injury (CNS) with hypertensive syndrome, hepatomegalia, and mitral valve prolapse. The diagnosis was confirmed by the results of a biochemical blood test, which showed a decrease in the activity of the enzyme of alpha-L-iduronidase. Genetic testing was recommended for the determination of the proband genotype. The second case was a boy S., aged 3 years and 7 months, with Hurler - Scheie syndrome. Numerous comorbidities (CNS, hypertensive syndrome, hip dysplasia, kyphosis, ambilateral inguinal hernia, etc.) combined with gradual formation of grotesque facial features by the age of eighteen months suggested that genetic testing should be taken during the second year of life. As a result, Q 70X mutation was identified in the heterozygous state and replacement therapy with Aldurazyme was prescribed. Clinical polymorphism and different severity of symptoms combined with rare occurrence was the reason why difficulties were experienced in the early identification of MPS I. Early diagnosis is very important so that the children could be timely referred to experts of the interdisciplinary center with an experience in the specific treatment, which is the most effective in the early stages of the disease.     Study of qualitative and quantitative indicators of the spermatogenesis for determination of the effectiveness of cryopreservation

    Semen or spermatozoa cryopreservation (commonly called sperm banking) is a procedure to preserve sperm cells. Semen can be used successfully indefinitely after cryopreservation and might be stored successfully over 20 years. It can be used for sperm donation where the recipient wants the treatment in a different time or place, or as a means of preserving fertility for men undergoing vasectomy or treatments that may compromise their fertility, such as chemotherapy, radiation therapy or surgery. However, before the cryopreservation, it is necessary to check the parameters of the ejaculate and treat the sperm in a special container (a small in diameter plastic box with information about the patient) to increase the concentration of spermatozoa. Put the container for storage into Dewar tube. In some cases, cryopreservation might worsen the quality of the ejaculate. After freezing, the mobility and morphology of the sperm can deteriorate. Thus, it is desirable to carry out freezing in test conditions to determine the need for the frozen material. Especially when spouses plan to use additional fertilization programs, this approach should be taken into account, since cryopreservation might cause a spontaneous change in the program of intracytoplasmic vaccination of the spouse. One of the methodological problems of cryopreservation is poor quality of the ejaculate, i.e. when freezing spermatozoa, their activity and function deteriorate from the norm. This reduces the possibility of using them in the future for artificial insemination. Results of the experiment show that cryof preservation is possible only in specialized centers with the highest professional standards
