    Swirl coaxial injector element characterization for booster engines

    Recent hot fire testing at the Marshall Space Flight Center (MSFC) has indicated the swirl-coaxial element to be a viable candidate for the STBE injector. Plans are to test the current 40K lbf thrust injector at the higher chamber pressure and colder fuel temperature which are anticipated for STBE. A cold flow program to characterize the swirl coax element over a range of operating points was conducted. The results are presented and compared to the hot fire data. Predictions for compatibility, performance and stability are then presented for the uprated test conditions

    The appendices A-K to the user's manual for the rocket combustor interactive design (ROCCID) computer program are presented. This includes installation instructions, flow charts, subroutine model documentation, and sample output files. The ROCCID program, written in Fortran 77, provides a standardized methodology using state of the art codes and procedures for the analysis of a liquid rocket engine combustor's steady state combustion performance and combustion stability. The ROCCID is currently capable of analyzing mixed element injector patterns containing impinging like doublet or unlike triplet, showerhead, shear coaxial and swirl coaxial elements as long as only one element type exists in each injector core, baffle, or barrier zone. Real propellant properties of oxygen, hydrogen, methane, propane, and RP-1 are included in ROCCID. The properties of other propellants can be easily added. The analysis models in ROCCID can account for the influences of acoustic cavities, helmholtz resonators, and radial thrust chamber baffles on combustion stability. ROCCID also contains the logic to interactively create a combustor design which meets input performance and stability goals. A preliminary design results from the application of historical correlations to the input design requirements. The steady state performance and combustion stability of this design is evaluated using the analysis models, and ROCCID guides the user as to the design changes required to satisfy the user's performance and stability goals, including the design of stability aids.     Safety and Tolerability of PD‐1/PD‐L1 Inhibitors Compared with Chemotherapy in Patients with Advanced Cancer: A Meta‐Analysis

    BACKGROUND: Compared with chemotherapy, significant improvement in survival outcomes with the programmed death receptor-1 (PD-1) inhibitors nivolumab and pembrolizumab and the programmed death-ligand 1 (PD-L1) inhibitor atezolizumab has been shown in several types of advanced solid tumors. We conducted a systematic review and meta-analysis to compare safety and tolerability between PD-1/PD-L1 inhibitors and chemotherapy. METHODS: PubMed and American Society of Clinical Oncology (ASCO) databases were searched 1966 to September 2016. Eligible studies included randomized controlled trials (RCTs) comparing single-agent U.S. Food and Drug Administration-approved PD-1/PD-L1 inhibitors (nivolumab, pembrolizumab, or atezolizumab) with chemotherapy in cancer patients reporting any all-grade (1-4) or high-grade (3-4) adverse events (AEs), all- or high-grade treatment-related symptoms, hematologic toxicities and immune-related AEs, treatment discontinuation due to toxicities, or treatment-related deaths. The summary incidence, relative risk, and 95% confidence intervals were calculated. RESULTS: A total of 3,450 patients from 7 RCTs were included in the meta-analysis: 4 nivolumab, 2 pembrolizumab, and 1 atezolizumab trials. The underlying malignancies included were non-small cell lung cancer (4 trials) and melanoma (3 trials). Compared with chemotherapy, the PD-1/PD-L1 inhibitors had a significantly lower risk of all- and high-grade fatigue, sensory neuropathy, diarrhea and hematologic toxicities, all-grade anorexia, nausea, and constipation, any all- and high-grade AEs, and treatment discontinuation. There was an increased risk of all-grade rash, pruritus, colitis, aminotransferase elevations, hypothyroidism, and hyperthyroidism, and all- and high-grade pneumonitis with PD1/PD-L1 inhibitors. CONCLUSION: PD-1/PD-L1 inhibitors are overall better tolerated than chemotherapy. Our results provide further evidence supporting the favorable risk/benefit ratio for PD-1/PD-L1 inhibitors. The Oncologist 2017;22:470-479 IMPLICATIONS FOR PRACTICE: We conducted a systematic review and meta-analysis to compare summary toxicity endpoints and clinically relevant adverse events between programmed death receptor-1 (PD-1)/programmed death-ligand 1 (PD-L1) inhibitors and chemotherapy. PD1/PD-L1 inhibitors were associated with a lower risk of treatment-related symptoms (fatigue, anorexia, nausea, diarrhea, constipation, and sensory neuropathy) but a higher risk of immune-related adverse events (AEs). Summary toxicity endpoints favor PD1/PD-L1 inhibitors (any all- and high-grade AEs and treatment discontinuation). PD1/PD-L1 inhibitors are overall better tolerated than chemotherapy.     International audienceIn 2010 the Mimar Sinan University of Istanbul started a research project on the Ionian town of Colophon which is conducted in close collaboration with the University of Vienna [1]. It aims at the exploration of the ancient urban area as well as on the distribution of the necropoleis. For the campaign of the year 2013 the Austrian research team concentrated on the following topics [2]. 1. Survey in the area of the ancient city This year, the main focus of the investigations in the area within the fortification walls was laid on the exploration of the urbanistic organisation of the city by magnetic survey and by ground penetrating radar. For this purpose two areas were chosen. The first area consisted of fields in the plain between the brooks Kabaklidere and Kurudere in the north eastern quarters of the ancient city. Here the existence of houses has been proven by excavations by the museum of Izmir about 10–15 years ago. The remains of walls were also visible in several trenches opened by illegal diggers both on the plains and in the forested area near the slopes of the surrounding hills. The geophysical prospection confirmed the assumed existence of several buildings and streets in the area. In addition we conducted an extensive survey in adjacent fields and collected pottery from the holes dug by illegal diggers. The finds included roof tiles and a small amount of pottery with a chronological range from the 6 th to the 4 th c. BC. A detailed documentation of the present land-use and land-division of this area, already begun in 2012, was continued with the aim of a diachronical understanding of the landscape's historical development. This included the description of modern features in the fields as well as interviews with owners and tenants. The second area chosen for a geophysical prospection is situated to the south of the Halil Ağa Tepesi, where W. Höpfner proposed to locate the ancient agora [3]. Both magnetic survey and ground penetrating radar prospections were carried out here. Further examination of the data is currently being carried out.     Randomized Controlled Trial of a Home‐Based Walking Program to Reduce Moderate to Severe Aromatase Inhibitor‐Associated Arthralgia in Breast Cancer Survivors

    BACKGROUND: In postmenopausal women diagnosed with breast cancer (BC), most BC tumors are hormone receptor positive and guidelines recommend adjuvant endocrine therapy that includes an aromatase inhibitor (AI). This study investigates the impact of a 6-week, home-based, self-directed walking program on the commonly reported side effect of AI-associated arthralgia (AIAA). MATERIALS AND METHODS: In this phase II trial, consented BC patients were randomized to walking Intervention (n = 31) or Wait List Control (WLC; n = 31). Eligibility criteria included: stage 0-III BC, on AI for at least 4 weeks, ≥3 on a 5-point scale inquiring about joint symptom intensity "at its worst," and exercising ≤150 minutes per week. Outcomes were self-reported joint symptoms and psychosocial measures. Analyses comparing Intervention and WLC groups were conducted on an intention-to-treat basis to assess intervention impact at 6 weeks (postintervention) and at 6-months follow-up. Adjusted means were calculated to assess differences in two groups. RESULTS: In our final sample (n = 62), mean age was 64 years, 74% were white, and 63% had a body mass index of 30 or higher. At postintervention, Intervention group participants reported significantly increased walking minutes per week, reduced stiffness, less difficulty with activities of daily living (ADL), and less perceived helplessness in managing joint symptoms. At 6-months follow-up (postwalking period in both Intervention and WLC), walking minutes per week had decreased significantly; however, improvements in stiffness and difficulty with ADLs were maintained. CONCLUSION: This study adds to the growing evidence base suggesting exercise as a safe alternative or adjunct to medications for the management of AIAA.     Advancing the State-of-the-Practice for Liquid Rocket Engine Injector Design

    Current shortcomings in both the overall injector design process and its underlying combustion stability assessment methodology are rooted in the use of empirically based or low fidelity representations of complex physical phenomena and geometry details that have first order effects on performance, thermal environments and combustion stability. The result is a design and analysis capability that is often inadequate to reliably arrive at a suitable injector design in an efficient manner. Specifically, combustion instability has been particularly difficult to predict and mitigate. Large hydrocarbon-fueled booster engines have been especially problematic in this regard. Where combustion instability has been a problem, costly and time-consuming redesign efforts have often been an unfortunate consequence. This paper presents an overview of a recently completed effort at NASA Marshall Space Flight Center to advance the state-of-the-practice for liquid rocket engine injector design. Multiple perturbations of a gas-centered swirl coaxial (GCSC) element that burned gaseous oxygen and RP-1 were designed, assessed for combustion stability, and tested. Three designs, one stable, one marginally unstable and one unstable, were used to demonstrate both an enhanced overall injector design process and an improved combustion stability assessment process. High-fidelity results from state-of-the-art computational fluid dynamics CFD simulations were used to substantially augment and improve the injector design methodology. The CFD results were used to inform and guide the overall injector design process. They were also used to upgrade selected empirical or low-dimensional quantities in the ROCket Combustor Interactive Design (ROCCID) stability assessment tool. Hot fire single element injector testing was used to verify both the overall injector designs and the stability assessments. Testing was conducted at the Air Force Research Laboratory and at Purdue University.     Older-Patient-Specific Cancer Trials: A Pooled Analysis of 2,277 Patients (A151715).

    BACKGROUND: Less than 3% of older patients with cancer are enrolled in clinical trials. To reverse this underrepresentation, we compared older patients enrolled with older-patient-specific trials, defined as those designed for older patients with cancer, with those enrolled in age-unspecified trials. MATERIALS AND METHODS: We focused on individual patient data from those ≥65 years (younger patients excluded) and included all Alliance phase III adjuvant breast cancer trials from 1985-2012. RESULTS: Among 2,277 patients, 1,014 had been enrolled to older-patient-specific and 1,263 to age-unspecified trials. The median age (range) in the older-patient-specific trials was 72 (65-89) years compared with 68 (65-84) years in the cohort of older patients in age-unspecified trials; CONCLUSION: Older-patient-specific trials appear to address this underrepresentation of older patients with ostensibly comparable outcomes
