    The molecular epidemiology of Stenotrophomonas maltophilia bacteraemia in a tertiary referral hospital in the United Arab Emirates 2000–2004

    BACKGROUND: Stenotrophomonas maltophilia is recognised as an important cause of nosocomial infection, especially in immunocompromised patients, resulting in significant morbidity and mortality. The treatment of S. maltophilia infection presents a therapeutic challenge. The precise modes of transmission of S. maltophilia in the hospital environment are not known and such knowledge is essential to target interventions to prevent spread. There are few published data on the patterns of nosocomial infection in the United Arab Emirates (UAE). A recent study showed that S. maltophilia is an established cause of bloodstream infection in Tawam Hospital in the UAE. Little is known about its epidemiology in the hospital. METHODS: We describe the clinical characteristics of 25 episodes of S. maltophilia bacteraemia which occurred from 2000–2004. The strains were characterised using pulsed field gel electrophoresis (PFGE). RESULTS: All episodes were hospital-acquired and malignancy and central venous catheters were predisposing factors. Catheter-associated infection comprised 88% infection. Catheter removal was important for the successful management of catheter-associated infection. The results of PFGE suggested that there were as many strains as patients. S. maltophilia strains isolated from the same patient had indistinguishable PFGE profiles. CONCLUSION: PFGE is a valid and reproducible typing method for S. maltophilia. The precise sources and modes of spread of S. maltophilia in the hospital are still not known. Knowledge that person to person transmission was not a major mode of transmission enabled infection control interventions for S. maltophilia to be targeted more effectively

    On-Line Detection And Measurement Of Partial Discharge Signals In A Noisy Environment

    In extracting partial discharge (PD) signals embedded in excessive noise, the need for an online and automated tool becomes a crucial necessity. One of the recent approaches that have gained some acceptance within the research arena is the Wavelet multi-resolution analysis (WMRA). However selecting an accurate mother wavelet, defining dynamic threshold values and identifying the resolution levels to be considered in the PD extraction from the noise are still challenging tasks. This paper proposes a novel wavelet-based technique for extracting PD signals embedded in high noise levels. The proposed technique enhances the WMRA by decomposing the noisy data into different resolution levels while sliding it into Kaiser's window. Only the maximum expansion coefficients at each resolution level are used in de-noising and measuring the extracted PD signal. A small set of coefficients is used in the monitoring process without assigning threshold values or performing signal reconstruction. The proposed monitoring technique has been applied to a laboratory data as well as to a simulated PD pulses embedded in a collected laboratory noise

    Immediate breast reconstruction with a saline implant and AlloDerm, following removal of a Phyllodes tumor

    <p>Abstract</p> <p>Background</p> <p>Phyllodes tumors are uncommon tumors of the breast that exhibit aggressive growth. While surgical management of the tumor has been reported, a single surgical approach with immediate breast reconstruction using AlloDerm has not been reported.</p> <p>Case presentation</p> <p>A 22-year-old woman presented with a 4 cm mass in the left breast upon initial examination. Although the initial needle biopsy report indicated a fibroadenoma, the final pathologic report revealed a 6.5 cm × 6.4 cm × 6.4 cm benign phyllodes tumor <it>ex vivo</it>. Treatment was a simple nipple-sparing mastectomy coupled with immediate breast reconstruction. After the mastectomy, a subpectoral pocket was created for a saline implant and AlloDerm was stitched to the pectoralis and serratus muscle in the lower-pole of the breast.</p> <p>Conclusions</p> <p>Saline implant with AlloDerm can be used for immediate breast reconstruction post-mastectomy for treatment of a phyllodes tumor.</p
