    Neonatal mitochondrial respiratory chain defect and vaginal embryonal rhabdomyosarcoma : possibility of oncogenesis?

    Mitochondrial disorders are rare metabolic diseases. They often present during neonatal period but with nonspecific clinical features such as feeding difficulties, failure to thrive, and seizures. Mitochondrial defects have also known to be associated with neurological disorders, as well as cancers. We report the first case of neonatal mitochondrial respiratory chain defect with sarcoma botryoides confirmed by pathologic diagnosis, suggesting another possible link between mitochondrial dysfunction and cancer.ope

    Diabetes mellitus due to agenesis of the dorsal pancreas in a patient with heterotaxy syndrome

    Heterotaxy syndrome (HS) is a congenital disorder resulting from an abnormal arrangement of visceral organs across the normal left-right axis in the embryonic period. HS is usually associated with multiple anomalies, including defects of the major cardiovascular system and the extracardiovascular system such as intestinal malrotation, abnormal lung lobulation, bronchus anomalies, and pancreatic dysplasia. Although pancreatic dysplasia is occasionally accompanied with HS, the occurrence of diabetes mellitus (DM) due to pancreatic dysplasia in HS is rarely reported. We here report a case involving 13-year-old girl with DM caused by agenesis of the dorsal pancreas and HS diagnosed on the basis of the presence of a double-outlet right ventricle with bilateral pulmonary stenosis and intestinal malrotation with duodenal cyst. Timely diagnosis and treatment with insulin improved glycemic control.ope

    Insulin resistance and bone age advancement in girls with central precocious puberty

    PURPOSE: Precocious puberty has significantly increased recently. While obesity is associated with puberty timing, the relationship between obesity and central precocious puberty (CPP) remains controversial. The purpose of this study was to determine whether insulin resistance is associated with bone age (BA) advancement in girls with CPP. METHODS: We retrospectively analyzed the records of 804 girls referred for puberty evaluation. Anthropometric measurements, BA, sex hormone, sex hormone binding globulin (SHBG), and insulin levels, lipid profiles, and gonadotropin releasing hormone stimulation tests were assessed. Insulin resistance parameters were calculated using the homeostasis model assessment-insulin resistance (HOMA-IR) and quantitative insulin sensitivity check index (QUICKI) models. RESULTS: BA, BA advancement, free estradiol index, insulin, and HOMA-IR increased significantly in girls with high body mass index (BMI) compared with that of girls with low BMI in cases of CPP. HOMA-IR was positively correlated with BA advancement and BMI but negatively correlated with SHBG. QUICKI was negatively correlated with BA advancement and BMI and positively correlated with SHBG. When HOMA-IR increased by 1, the odds for BA advancement increased 120% after adjusting for age and BMI (P=0.033). CONCLUSIONS: Insulin resistance could be associated with BA advancement in girls with CPP.ope

A programming system is an integrated system consisting of a programming language, the supportive kernel, and the programming environment. A programming system for distributed computation should address the distribution and parallelism issue. There have been many approaches to build such programming systems. However, we set forth the thesis that the programming system for distributed computation should be defined from an integrated system architecture viewpoint based on a unified model. In this dissertation, we propose as an effort to provide a programming system for distributed computation a programming language and a configuration language based on a uniform object-based computation model. CornerStone is an object oriented language that addresses the parallelism issue based on the notion of cooperating objects. The language embodies notable features in two respects. it distinguishes the inheritance and the subtyping as distinct concepts for the code sharing and the specialization, respectively. It also provides versatile object interaction primitives to coordinate the execution of concurrent objects. Objects programmed in CornerStone reveals that the language is appropriate for medium-grain parallelism. The distribution of the objects in distributed systems is addressed by the configuration language which maps the objects onto capsules. Capsule is an engineering concept for mapping objects onto real distributed systems. A capsule is the basic unit of the resource allocation in the underlying system; Objects in a capsule share resources allocated to the capsule. The capsule concept also bridges the granularity gap between the computation model and the underlying system. These two languages proposed in this dissertation cooperate closely to solve given problems in terms of cooperating objects in distributed systems.
