[์˜๋ฌธ]The purpose of this study was to investigate whether the evaluation value of lung cancer markers; CEA, CYFRA 21-1, and NSE before surgery have relation between total survival time and recidivism survival time after surgery.The subjects for study were 109 among 554 lung cancer patients who were sent and surgically operated at Medical Center in Seoul from Jan. 1, 2000 to September 30, 2005. This study used final data about 109 patients who were examined CEA, CYFRA 21-1, and NSE in 3 months before operation among 554 patients who experienced surgical operation for treatment. Statistical analyses were conducted using SAS 9.1 for Window. The results were as follow:1. The distribution of TNM stage showed 34.9%, 14.7%, 35.8%, and 14.7% from stageโ…  to stage โ…ฃ, respectively. The distribution of pathology was 50.5% in squamous cell carcinoma, 39.5% adenocarcinoma, and 10% in the others. In addition, Tumor markers was distributed 67.0% less than CEA 4.60, 33% over 4.61, 59.1% less than CYFRA 21-1 3.30, 40.9% more than 3.31, and 58,3% less than NSE 12, 41.7% over 12.1.2. As to pathology profiles, abnormal value of CEA in TNM stage of the distribution of tumor markers showed 13.2%, 31.3%, 41.0%, and 62.5% from stage โ…  to stage โ…ฃ, respectively. So the higher the stage was, the more percentage increased. This showed significant difference (p=0.003). Abnormal values of CYFRA 21-1 were stage โ…  28.6%, stage โ…ก 57.1%, stage โ…ข 46.4%, stage โ…ฃ 45.5%. The higher the stage was, the more percentage increased, too. But there was not significant difference(p=0.243). Those of NSE were stage โ…  51.5%, stage โ…ก 41.7%, stage โ…ข 32.1%, stage โ…ฃ 36.4%. Also, there was not statistically significant difference, either(p=0.477).3. In the distribution of pathology, CEA abnormally was 55.8% in adenocarcinoma higher than 16.4% in squamous cell carcinoma or 27.3% in the others. And there was significant difference in the result(p=0.001). In contrast to 54.5% in squamous cell carcinoma, 42.9% in the others, and 24.3% in adenocarcinoma. Abnormal value of CYFRA 21-1 had significant difference(P=0.022). Abnormal value of NSE was 50.0% in adenocarcinoma, 37.2% in squamous cell carcinoma, 28.6% in the others. As a result, it was not significant difference(p=0.403).4. In CEA, the result of log-rank test about tumor markers showed the difference between death rate 10.9% and recidivism rate 19.2% after surgery among the less than 4.6 subjects. And it also showed the difference between death rate 8.3% and recidivism rate 27.7% after surgery among over 4.61 subjects. However, the result of log-rank test didn''t have significant difference statistically. In case of CYFRA 21-1, there was the difference between death rate 5.8% and recidivism rate 17.3% after surgery among the less than 3.3 subjects. There were the difference between death rate 2.8% and recidivism rate 19.4% among over 3.31 subjects, but there was not significant difference as a result of log-rank test. In case of NSE, there was the difference between death rate 0% and recidivism rate 18.4% after surgery among the less than 12 subjects. And there was the difference between death rate 8.6% and recidivism rate 17.1% over 12.1 subjects, but there was not significant difference as a result of log-rank test.5. As a result of Cox''s proportional hazard''s model about whole survival, the risk rate for Women''s whole survival against men was low and the more the age was, the more the risk increased. However, there was not statistically significant difference. Besides, in case of the physical activity, the more uncomfortable action is, the more the risk increased. and there was significant difference in the relationship. In TNM stage, The risk rates of death was higher 1.413 times(p=0.708) in stage โ…ก than in stage โ… , higher 1.054 times(p=0.955) in stage โ…ข than in stage โ… , and higher 4.095 times(p=0.123) in stage โ…ฃ than in stage โ… . Death risk rates of the subjects who was over CEA 4.61 is higher 0.840 times than under CEA 4.6, However there was not significant difference statistically (p=0.815).6. As a result of Cox''s proportional hazard''s model about recidivism survival time, sex and age didn''t have any relation with survival time until recidivism. Besides, in case of physical activity, the recidivism risk rate of uncomfortable subjects had twice times higher, but there was not significant difference. And the risk rate of recidivism in stage โ…ก, โ…ข, โ…ฃ each was higher than TNM stage โ… . Especially the risk rate of stage โ…ฃ was 8โˆผ20 times high according to models but There was significant difference statistically. Under controlling the influence of others risk factors, the risk rate of recidivism was 0.889 times in CEA of the group over 4.61, was 1.770 times in CYFRA 21-1 of the group over 3.31, and was 1.518 times in NSE of the group over 12.1, respectively. But These didn''t statistically show significant difference.The result of this study showed that CEA, one of tumor markers of lung cancer, had significant relation between TNM stage and pathology right after surgery, but it appears that this result is not helpful to expect survival time up to death or recidivism after surgery by tumor marker level before surgery. However, because of limitations included the number of subjects and data of only one medical center, it appears that in the future the study based on representative many data are needed.ope
