A study on the influence factors on the percentage of uncovered health care services cost:Using requesting data for medical Fee Verification service in 2019

배경 및 필요성: 우리나라의 건강보험제도 하 모든 진료는 요양급여 적용을 받는 항목과 요양급여 받지 않는 비급여 항목으로 나뉜다. 요양급여 적용을 받는 진료 항목은 청구시스템을 통해 체계적으로 관리되고 있으나, 이와 달리 비급여 진료 항목은 가격과 진료량이 요양기관의 자율적인 의사에 따라 결정된다. 비급여 서비스는 요양기관의 수익성 향상에 중요한 부분이고, 관리가 어려워 급여 보장성 강화 정책에도 불구하고 우리나라 건강보험 보장률이 오차 범위 내에서 정체하고, 국민의료비를 증가하게 하는 주요 요인이다. 또한, 여러 가지 비급여 비용 관리를 위한 정책에도 불구하고 현실적으로 비급여 관련 실태 파악에 한계가 있으며, 비급여 비용에 대한 사회적 관심과 관리의 필요성이 대두되고 있다. 따라서, 2019년 요양급여대상여부확인 제도를 통하여 처리된 진료비확인요청 건을 바탕으로 총 진료비에서 비급여 비용이 차지하는 비율을 연구함으로써, 향후 관련 보건정책에 대한 계획과 평가를 하는 데에 기초자료로써 보탬이 되고자 한다. 설계: 건강보험심사평가원 HIRA PLUS SYSTEM에서 2019년에 처리된 진료비확인요청 건에 대한 연간 데이터를 이용하였다. 총 진료비에서 비급여 비용이 차지하는 비율을 구하고 이에 영향을 미치는 요인을 파악하기 위해 의과를 다루는 요양기관을 대상으로 심사 후 정당하게 지불된 금액을 기준으로 총 23,290건을 분석하였다. 연구대상의 요인 별 상관성을 고려하여 선형회귀분석을 실시하였다. 모든 분석은 SAS Version 9.4(SAS Institute, Cary, NC, United States)를 사용하였고, 유의수준은 5%로 설정하였다. 결과: 수진자 특성, 요양기관 특성, 진료 내역 특성을 모두 통제하여 다변수 분석을 실시한 결과, 요인 별로 성별에서는 남성보다 여성에서 비급여 비율이 높았고, 연령에서는 6-18세, 의료보장 형태에서는 건강보험, 요양기관 종별에서는 병원급 요양기관, 요양기관 위치에서는 서울특별시, 진료 형태에서는 외래 진료, 진료 과목에서는 성형외과, 질병 분류에서는 근육 계통과 연결 조직의 질병 분류, 수술을 하지 않은 경우에서 총 진료비에서 차지하는 비급여 진료비의 비율이 유의하게 높은 것으로 확인되었다. 결론: 본 연구로 비추어 볼 때, 여성, 6-18세, 건강보험, 병원 급 요양기관, 서울특별시에 위치한 요양기관, 외래 진료, 성형외과, 근육 계통과 연결 조직의 질병 분류, 수술을 하지 않은 경우 등 비급여 비율이 높게 나온 항목에 대하여는 비급여 비율이 높은 요인에 대한 원인을 심층적으로 분석하여 이에 대한 관리 방안 및 관련 고시 정비가 필요하다. 합리적이고 적절한 비급여 진료를 유도하여 적정한 진료 환경이 조성될 수 있도록 많은 관심과 알맞은 정책이 필요하며, 본 연구가 이에 조금이나마 보탬이 되었으면 한다. Background: Under Korea health insurance system, all medical care is divided into items subject to medical care benefits and items that do not receive medical care benefits. Items of care subject to care benefits are systematically managed through the review and assessment system by HIRA, but, uncovered health care services are determined by the caregiver's autonomous will. Uncovered health care services are an important part of improving the profitability of nursing institutions, a major factor that causes the nation's health insurance coverage rate to stagnate within the margin of error and increase the national health care costs despite the policy to strengthen the coverage of benefits. Therefore, it is intended to contribute to the planning and evaluation of related health policies in the future by studying the ratio of uncovered health care services costs in total medical expenses based on the request for medical fee verification service processed through the 2019 system for medical care benefits. Methods: Annual data on requests for medical fee verification service processed in 2019 were used by HIRA PLUS SYSTEM of the Health Insurance Review and Assessment Service. 23,290 cases were analyzed based on the amount duly paid after the examination of nursing institutions dealing with medical departments in order to obtain the proportion of uncovered health care services costs in total medical expenses and to identify the factors affecting them. Linear regression analysis was performed considering the correlation of factors in the study. All analyses used SAS Version 9.4 (SAS Institute, Cary, NC, United States), with a significant level of 5%. Results: As a result of multivariate analysis by controlling all of the characteristics of the patient, hospital, and medical care details, the ratio of uncovered health care services costs to women was higher than that of men in gender, 6-18 in age, health insurance in health insurance, Seoul Metropolitan Government in hospital-level nursing homes, outpatient care in medical care type, plastic surgery in medical department. Conclusions: In light of this study, women, 6-18 years of age, health insurance, hospital-level nursing institutions, nursing homes located in Seoul Metropolitan City, outpatient care, plastic surgery, disease classification of muscle systems and connective tissues, and surgery, need to be done after in-depth analysis of the causes of factors with high uncovered health care services costs rates. A lot of attention and policies are needed to create a reasonable and appropriate medical environment.open석
