[์˜๋ฌธ]The present study is a descriptive correlation research that divided nursesโ€™ attitude toward basic life support into cognitive components (belief, knowledge), affective component and behavioral component, surveyed them multi?dimensionally, and analyzed factors influencing the components. For this study, we selected 280 nurses working at a general hospital in Seoul through convenient sampling and surveyed their general characteristics and attitude toward basic life support during the period from the 30th of November to 6th of December, 2007, and analyzed a total of 249 questionnaires. The survey used a structured questionnaire, which was composed of 21 questions on the subjectsโ€™ general characteristics, 36 on their attitude toward basic life support, so a total of 57 questions. The reliability of the questionnaire was Cronbachโ€˜s a = .64 and .63, respectively, as the scales of cognitive components belief and knowledge, Cronbachโ€˜s a = .69 as the scale of affective component, and Cronbachโ€˜s a = .77 as the scale of behavioral component. Collected data were statistically processed using SAS (Statistical Analysis System) 9.1, and analyzed through descriptive statistics, correlation analysis, t?test, one?way ANOVA, Tukey post hock test, and stepwise multiple regression analysis. The results of this study are as follows.1. The nurses who participated in this study were 28.47 years old on the average, and their mean total length of clinical experience was 5.35 years. Of them, 83.53% experienced cardiac arrest patients in hospital, and 31.73% experienced the direct performance of basic life support.2. As to the cognitive components of the subjectsโ€™ attitude toward basic life support, the mean score of belief was 10.02 points out of 12, showing high belief in basic life support, and the mean score of knowledge was 12.60 out of 20, showing a moderate level of knowledge of basic life support. The mean score of affective component was 40.83 out of 70, showing a moderate level. According to the results of analyzing the subjectsโ€™ answers to questions on their emotion toward the performance of basic life support, they felt that basic life support is โ€˜good, beneficial, useful, important and necessaryโ€™ but, on the other hand, they had negative feelings such as โ€˜tension, stress, anxiety, fear, and frustration.โ€™ The mean score of behavioral component was 9.73 out of 12, showing high behavioral intention toward basic life support.3. As to the correlations among the cognitive components (belief, knowledge), affective component and behavioral component of the subjectsโ€™ attitude toward basic life support, they were all in a positive correlation with one another, and particularly when belief was high behavioral intention toward basic life support was high (r=0.59, p< .001).4. As to belief in basic life support, a cognitive component of attitude toward basic life support, according to the subjectsโ€™ general characteristics, belief in basic life support was higher in those aged between 30~34 than in those younger than 25 and those aged between 25~29, and in those aged 35 or older than in those younger than 25 (F=4.94, p< .01), in married nurses than in unmarried ones (t=-2.11, p< .05), in those with 5~9 yearsโ€™ clinical experience than in those with less than 1 yearโ€™s experience, and in those with 10 yearsโ€™ or longer clinical experience than in those with less than 1 yearโ€™s experience and those with 1~4 yearsโ€™ experience (F=6.99 p< .001), and in those with 3~4 yearsโ€™ experience in the current ward than in those with less than 1 yearโ€™s experience (F=2.71, p< .05). In addition, belief in basic life support was higher in those with experience in emergency room than in those with experience in intensive care unit and those without experience in emergency room/intensive care unit (F=4.09, p< .05), in those with 3~4 yearsโ€™ experience in emergency room/intensive care unit than in those with less than 1 yearโ€™s experience (F=4.45, p< .01), in those with experience in the direct performance of basic life support (t=3.63, p< .01), and in those with a certificate related to cardiopulmonary resuscitation (t=2.09, p< .05).As to knowledge of basic life support, another cognitive component of attitude toward basic life support, according to the subjectsโ€™ general characteristics, knowledge of basic life support was higher in those aged between 30~34 than in those younger than 25 and those aged between 25~29 (F=4.68, p< .01), in those working at the emergency room than in those working at the internal medicine ward or the surgical ward (F=2.44, p< .05), in those with 5~9 yearsโ€™ clinical experience than in those with 1~4 yearsโ€™ experience (F=3.81, p< .05), and in those with 5 yearsโ€™ or longer experience in the current ward than in those with less than 1 yearโ€™s experience (F=3.01, p< .05). In addition, knowledge of basic life support was higher in those with 5 yearsโ€™ or longer experience in emergency room/intensive care unit than in those with 1~2 yearsโ€™ experience (F=5.26, p< .01), in those with experience in cardiac arrest patients (t=2.24 p< .05), and in those with a certificate related to cardiopulmonary resuscitation (t=3.73, p< .01).5. As to the affective component of attitude toward basic life support according to the subjectsโ€™ general characteristics, emotion toward basic life support was more positive in those with a masterโ€™s degree or higher than those with a vocational or bachelorโ€™s degree (F=4.79, p< .01), in those with 5~9 yearsโ€™ clinical experience than in those with 1~4 yearsโ€™ experience (F=4.87, p< .01), and in those with 5 yearsโ€™ or longer experience in emergency room/intensive care unit than in those with 1~2 yearsโ€™ experience (F=5.63, p< .01). In addition, emotion toward basic life support was more positive in those with experience in cardiac arrest patients (t=3.25 p< .05), those with experience in the direct performance of basic life support (t=3.74, p< .01), and in those with a certificate related to cardiopulmonary resuscitation (t=2.48, p< .05).6. As to the behavioral component of attitude toward basic life support according to the subjectsโ€™ general characteristics, behavioral intention toward basic life support was higher in those with a masterโ€™s degree or higher than in those with a vocational degree (F=3.40, p< .05), those with 10 yearsโ€™ or longer clinical experience than in those with less than 1 yearโ€™s experience (F=3.54, p< .05), and in those with 1~2 yearsโ€™ experience in emergency room/intensive care unit work, those with 3~4 yearsโ€™ experience and those with 5 yearsโ€™ or longer experience than in those with less than 1 yearโ€™s experience (F=6.67, p< .001). In addition, behavioral intention toward basic life support was higher in those with experience in cardiac arrest patients (t=3.28 p< .05) and those with experience in the direct performance of basic life support (t=5.59, p< .001).7. Factors affecting the subjectsโ€™ belief in basic life support were clinical experience, experience in the direct performance of basic life support and whether to have a certificate related to cardiopulmonary resuscitation, and their explanatory power was 11.3%. Among them, clinical experience explained 5.1%, having the highest effect on the subjectsโ€™ belief in basic life support.Factors affecting the subjectsโ€™ knowledge of basic life support were experience in emergency room/intensive care unit, whether to have a certificate related to cardiopulmonary resuscitation, and age, and their explanatory power was 11.7%. Among them, experience in emergency room/intensive care unit explained 5.8%, having the highest effect on the subjectsโ€™ knowledge of basic life support.8. Factors affecting the affective component of the subjectsโ€™ attitude toward basic life support were experience in emergency room/intensive care unit, experience in the direct performance of basic life support, and education level, and their explanatory power was 12.3%. Among them, experience in emergency room/intensive care unit explained 7.3%, having the highest effect on the affective component of the subjectsโ€™ attitude toward basic life support.9. Factors affecting the behavioral component of the subjectsโ€™ attitude toward basic life support were experience in the direct performance of basic life support and clinical experience, and their explanatory power was 13.7%. Between them, experience in the direct performance of basic life support explained 11.2%, having the highest effect on the behavioral component of the subjectsโ€™ attitude toward basic life support. In conclusion, nursesโ€™ clinical experience and their experience in the direct performance of basic life support were found to be important factors for nursesโ€™ performance of basic life support in the future. Accordingly, in order to develop a positive attitude toward the performance of basic life support in nurses, we need to strengthen education on basic life support and develop active education programs particularly focused on practical training for nurses who are young, have short clinical experience and are inexperienced in cardiac arrest patients. What is more, strategies should be established to enhance nursesโ€™ knowledge of basic life support, which was average, programs should be developed to positively reinforce the affective component of nursesโ€™ attitude toward basic life support, which was found partially negative in this study, and affective consideration should be given to nursesโ€™ emotion in educating cardiopulmonary resuscitation.ope
