Kambins triangle approach for percutaneous transforaminal epidural adhesiolysis with an inflatable balloon catheter: comparison with safe triangle approach

학위논문 (석사)-- 서울대학교 대학원 : 의과대학 의학과, 2018. 8. 이평복.배경 척추관 협착증은 고령인구에서 매우 흔한 질환이지만 기존의 치료법으로 는 잘 치료되지 않는 경우가 많다. 풍선 확장 카테터를 사용한 경피적 풍선 확장술은 경막외 부위의 유착을 제거하여 신경의 눌림을 완화시켜 주고 효과적으로 약물의 전달을 할 수 있게 한다. 최근 들어 소개된 추 간공을 통한 경피적 확장술은 추간공 협착 환자들에 있어 통증 정도와 기능 개선에 효과가 있음이 알려져 있다. 우리는 경추간공 경피적 풍선 확장술에 있어 기존의 안전 삼각 접근법뿐만 아니라 캠빈 삼각 접근법의 효과와 의의를 알아보고자 한다. 방법 기존 치료에 반응하지 않는 3개월 이상의 편측 제 5 요추부 신경 영역의 방사통을 호소하는 환자들을 대상으로 하였다. 24명의 환자를 모집하여 안전 삼각 접근법과 캠빈 삼각 접근법 두 군으로 분류한 뒤 시술 성공 률, 시술 전과 시술 1개월 후, 그리고 시술 3개월 후의 NRS, K-ODI를 측정하였다. 결과 시술 성공률은 안전 삼각 접근법에서 80%, 캠빈 삼각 접근법에서 90% 로 큰 차이가 없었다. NRS는 시술 전에 비해 두 군 모두 3개월 후 통계 적으로 유의미한 감소를 보였으나(p-value<0.05) 두 군간 차이는 없었다 (p-value 0.441). Above of all, integrated network should not degrade QoS(quality of service). For the purpose, BS(base station) should allocate appropriate bandwidth to each traffic by using good scheduler. In wireless networks, SNR is changing with time due to many reasons like fading. If users use fixed modulation for time varying SNR, then for low SNR large amount of error can occur, so QoS may not be guaranteed. But exact estimation of SNR is difficult and estimating method is complex and expensive. So we proposed SNR estimation algorithm using previous error rate, and by using the algorithm we propose time slot allocation algorithm.
