Topographic anatomy of the depressor anguli oris muscle and mental foramen

치의학과/석사[한글]입술아래턱주름 (labiomandibular fold)은 입꼬리에서 아래턱까지 이어진 주름으로 나이가 들면서 더 두드러지게 나타날 수 있다. 입술아래턱주름은 입꼬리내림근의 안쪽모서리와 가쪽모서리에 의해 생기며, 이 주름이 깊을 경우 입꼬리내림근에 BTX-A를 주사하여 주름을 없애기도 한다. 그러나 입꼬리내림근과 턱끝구멍의 위치와 변이에 관한 임상해부학적 지식은 충분치 못하다. 이 연구의 목적은 입꼬리내림근의 형태와 크기, 입꼬리내림근과 턱끝구멍, 턱끝신경의 국소해부학적 관계를 밝히고, BTX-A를 안전하고 효율적으로 주사할 수 있는 부위를 찾기 위함이다. 연구재료는 한국 성인 시신 34쪽 (남자 8구, 여자 9구, 평균나이 71세)의 얼굴을 사용하였다.입꼬리사이선 (intercheilion horizontal line)과 평행한 양쪽 입꼬리내림근 안쪽모서리 사이의 최대폭은 평균 59.9±4.6 mm (최대 70.0 mm, 최소 51.2 mm)였다 (남자: 61.0±5.8 mm, 여자: 58.7±2.7 mm). 턱뼈모서리에서 양쪽의 입꼬리내림근 안쪽모서리 근육섬유가 부착하는 부위 사이의 폭은 평균 29.7±4.8 mm (최대: 39.5 mm, 최소: 22.7 mm)로 계측되었다 (남자: 30.8±4.6 mm, 여자: 28.4±5.2 mm). 또한 턱뼈모서리에서 입꼬리내림근의 근육부착 부위의 폭은 왼쪽과 오른쪽이 각각 36.7±3.1 mm, 36.0±4.1 mm 였다. 위, 아래관계에서 입꼬리점과 턱뼈모서리를 잇는 수직선을 삼등분한 부위에서 중간 1/3에 턱끝구멍이 위치하는 경우는 28예 (90.3%), 중간 1/3과 아래 1/3 부위의 경계에 있는 경우는 2예 (6.5%), 그리고 아래 1/3부위에 위치하는 경우는 1예 (3.2%)가 관찰되었다. 앞뒤관계에서 턱끝구멍이 입꼬리내림근의 안쪽모서리보다 안쪽에 위치하여 근육에 덮여있지 않은 경우는 21예 (67.7%), 입꼬리내림근의 안쪽모서리 경계 상에 있는 경우는 3예 (9.7%), 그리고 입꼬리내림근에 덮여 턱끝구멍이 근육의 범위 내에 위치하는 경우는 7예 (22.6%)가 있었다. 입꼬리내림근과의 국소해부학적 턱끝구멍의 앞-뒤 위치관계를 세분하여 관찰한 결과, 턱끝구멍이 입꼬리점 바로 아래 부위에 위치하는 경우는 3예 (9.7%), 입꼬리점과 볼굴대 사이 부위에 위치하는 경우는 13예 (41.9%), 볼굴대 바로 아래 부위에 위치하는 경우는 10예 (32.3%), 그리고 볼굴대보다 가쪽에 위치하는 경우는 5예 (16.1%)로 각각 관찰되었다. 입꼬리내림근 가쪽모서리를 기준으로 얼굴신경의 가지들이 달리는 높이를 관찰하여 분류한 결과, 얼굴신경 볼가지가 입꼬리내림근 가쪽모서리의 중간 1/3 부위를, 얼굴신경 턱모서리가지가 가쪽모서리 아래 1/3 부위를 지나 깊은층으로 달리는 경우가 17예 (60.7%)로 가장 많았다. 또한 얼굴신경 볼가지는 근육 가쪽모서리의 위쪽 1/3 부위를, 그리고 턱모서리가지는 아래 1/3 부위를 지나는 경우는 7예 (25%)에서 있었다. 한편, 얼굴신경 볼가지가 근육 가쪽모서리의 위쪽 1/3 부위를 지나고, 턱모서리가지가 근육 가쪽모서리의 중간 1/3 부위를 지나 달리는 경우는 4예 (14.3%)에서 관찰하였다. [영문]The labiomandibular fold is the area of the face that extends from the corner of the mouth above to the mandible below and that tends to become more prominent with age. The labiomandibular fold is formed by the medial and lateral borders of the depressor anguli oris muscle (DAO) and when this fold become deepened, it can be reduced by injection of the BTX-A. However, the anatomic knowledges are insufficient in the clinical aspects. The aim of this study was to demonstrate the topographic anatomy of the DAO, mental foramen and mental nerve, thereby providing the critical information for the safe and efficient BTX-A injection site. We performed a careful dissection on 34 hemi-faces from Korean adult cadavers (male 8, female 9, average age 71). The maximum distance of the between the medial borders of the bilateral DAO, paralleled to the intercheilion horizontal line, was average 59.9±4.6 mm (maximum 39.5 mm, minimum 22.7 mm). And the distance of the DAO from the mandible margin to the cheilion was 36.7±3.1 mm on the left side, and 36.0±4.1 mm on the right side. In the superoinferior direction, the mental foramen was located in the middle third from the cheilion to the margin of the mandible in 28 cases (90.3%), the border between the middle and lower third in 2 cases (6.5%), and the lower third in 1 case (3.2%). In the anteroposterior direction, the mental foramen was located medially to the medial border of the DAO and not covered by the DAO in 21 cases (67.7%). The mental foramen was located in the medial border of the DAO in 3 cases (9.7%), and within the DAO in 7 cases (22.6%). In the detailed anteroposterior topographic relationship between the mental foramen and DAO, the mental foramen was located below the cheilion in 3 cases (9.7%), between the cheilion and mental foramen in 13 cases (41.9%), below the modiolus in 10 cases (32.3%), and lateral to the modiolus in 5 cases (16.1%). We classified the results of height of the facial nerve passing the posterior border of the DAO. The buccal branch of the facial nerve passed the middle third of the posterior border of the DAO and marginal mandibular branch passed the lower third in 17 cases (60.7%), which type was the most observed in our study. And of the 7 cases (25%) the buccal branch passed the upper third and marginal mandibular branch passed the lower third.
