(The) effects of growth hormone therapy in adult patients with growth hormone deficiency

의학과/석사[한글] 성장호르몬 결핍 성인에서 성장호르몬을 투여하였을때 내분비 및 대사 측면에서 개선여부를 관찰하기 위하여 성장호르몬이 결핍된 28예를 대상으로 연구를 수행하였다. 대상자는 모두 20세 이후에 후천적으로 성장호르몬이 결핍된 환자로서 뇌하수체 종양이 13예, Sheehan 증후군이 13예, 그리고 두부 손상으로 인한 뇌하수체 기능 저하가 2예이었다. 28예중 무작위로 선출한 10예에게 체중 1 kg당 0.06 U의 성장 호르몬(rhGH, 럭키제 약)을 1주에 3회 6 개월간 투여하였고(제1군), 9예는 같은 용량의 성장호르몬을 매일 투여하였으며(제2군), 그리고 대조군 9예에게는 위약을 투여하였다(제3군), 성장호르몬 투여전과 투여후에 인체계측, 일반화학 검사, 경구당부하 검사, 혈중 성장호르몬, IGF-1, C-peptide 및 인슐린 측정, 운동능력 검사, 골밀도 검사, 전산화 단층 촬영등을 시행하여 다음과 같은 결과를 얻었다. 1. 제 1, 2, 3 군의 실험 개시전 체중, 체지방량, lean body mass 등의 인체계측치는 차이가 없었으나, 실험 6 개월후 세군 모두 체중의 변동은 없이 제 1, 2 군은 lean body mass가 의의있게 증가하였고, 대조군은 체지방량이 증가하였다. 2. 제대수준과 대퇴 중간부에서 시행한 전산화 단층 촬영상 제1군은 대퇴부의 근육량이 의의있게 증가하였으며, 제2군은 제대부분의 내장지방이 의의있게 감소하였고 대조군은 대퇴부의 지방량이 증가하였다. 3. 혈청 성장호르몬과 IGF-1을 측정한 결과 제1군에서는 3개월째 IGF-1치가, 6개월에는 IGF-1과 성장호르몬치가 의의있게 증가하였고, 제2군에서는 6개월째 IGF-1이 증가하였으며, 제3군은 변화가 없었다. 4. 제2군에서 당부하후 1시간 및 2시간의 혈청 C-peptide치가 성장호르몬 치료후 6개월째 의의있는 증가를 보였고 제 1, 3군은 유의한 차이가 없었다. 혈청 인슐린치는 제1군에서 당부하후 1시간 인슐린치가 6개월째 의의있게 증가하였고 제 2, 3군은 차이가 없었다. 5. 세군 모두에서 치료 전후에 혈청 지질, 경구당부하시 혈당치, 혈청 전해질 및 삼투압치에는 유의한 변화가 없었다. 6. 운동능력 평가를 위하여 악력, 배근력 측정과 답차검사 (treadmill test)를 시행한 결과, 대조군은 6개월전과 차이가 없었으나 제1, 2군은 최대산소소모량이 증가하는 경향을 보였고, 제1군에서는 운동시의 rate-pressure product가 6개월후 증가하였고 악력 및 배근력이 의의 있게 증가하였으며, 제2군은 배근력이 의의 있게 증가하였다. 7. 양광자 흡수기를 이용하여 요추와 대퇴부에서 골밀도를 측정하였으나 세군 모두 6개월 후 의의 있는 차이는 없었다. 이상의 결과로 성장 호르몬이 결핍된 성인에서 성장 호르몬의 보충투여는 lean body mass(근육량)을 증가시키고, 혈청 성장호르몬과 IGF-1치를 증가시키며 운동능력을 향상시킴을 관찰할 수 있었다. [영문] In a placebo controlled trial, we studied the effects of six months treatment of recombinant human growth hormone (rhGH) in 28 adults with GH deficiency. The patients had acquired GH deficiency due to pituitary tumors (13 cases), Sheehan's syndrome (13 cases), and head trauma (2 cases). They were random1y divided into three groups. The group 1(10 cases, M:F = 5:5) received 0.06 U of rhGH per kilogram of body weight subcutaneously three times a week at bed time, the group 2(9 cases, M:F = 4:5) received saute amount of rhGH daily, and the group 3(9 cases, M:F = 4:5) received placebo. In al1 three groups the total body weight did not change, whereas mean lean body mass significantly increased and adipose tissue mass decreased in rhGH treatment groups. The plasma glucose and glycated hemoglobin levels were not increased during rhGH treatment. The bone mineral densities of spines and femurs were net changed. According to computed tomography on two levels (umbilicus and mid-thigh), the mean mid-thigh muscle mass was significantly increased in group 1, while visceral fat mass significantly decreased in group 2. In groups 1 and 2, rhGH slightly increased exercise maximum oxygen uptake, significantly increased exercise rate-pressure product, hand grip strength and back lift strength after rhGH. treatment, but the placebo group had not any change. These results suggest that rhGH hart various potentially beneficial effects in GH-deficient adults, especially in changing body composition and increasing exercise capacity.restrictio

한나 아렌트(Hannah Arendt )는 독창성의 측면에서 20세기 정치 사상계의 가장 위대한 아웃사이더 중 한 명이다. 아렌트는 한편으로는 현대 대의제 민주주의로 인한 정치 위기를 극복할 수 있는 참여 정치의 고전적 옹호자로서, 다른 한편으로는 근대에서 정치의 핵심 주제가된 사회적, 경제적 문제를 정치에서 배제하는 비현실적인 몽상적 이론가로서 극단적으로 평가받고 있다. 아렌트 옹호자들은 정치가 일반적으로 사회 경제적 관심사에 종속된 시대에서 정치의 독립적 가치를 재확인시킨 점을 높이 평가하는 반면, 비판자들은 아렌트가 고대의 도시 국가나 근대의 혁명 경험에 경도 된 나머지 일상적인 민주 정치를 거부하고 이로 인해 현실(now and here) 정치에 대해 말해주는 바가 별로 없다고 평가한다

(The) effects of growth hormone treatment on body composition and metabolism in adult hypophysectomized rats

의학과/박사[한글] 성장호르몬은 전형적으로 동물의 성장에 관여하는 것으로 알려져 왔으나 최근에 성장작용 이외 대사작용에 대한 연구가 활발히 진행되고 있다. 본 연구는 성장호르몬이 결핍된 흰쥐에서 성장호르몬 투여시 체조성의 변화와 체내 당대사 및 골대사에 미치는 영향에 대하여 알아보고자 하였다. 생후 20주된 웅성 Sprague Dawley rat를 뇌하수체 절제한 후 부신피질호르몬(cortisol)과 갑상선호르몬(thyroxine)을 8주간 투여한 후 유전자재조합사람성장호르몬(recombinant human growth hormone)을 2주간 투여하였다. 제 1군은 뇌하수체를 절제하지 않은 정상 대조군(n=15), 제 2군은 뇌하수체 절제후 부형제(vehicle)만을 투여한 군(n=12), 제 3군은 뇌하수체 절제후 성장호르몬을 4IU/kg/day로 투여한 군(n=13)으로 구분하였다. 성장호르몬 투여 전과 투여 후 체중, 체조성, 혈당, 혈장 인슐린양성장인자-Ⅰ 농도, 간세포 인슐린양성장인자- Ⅰ 전령리보핵산 발현, 정상혈당클램프검사, 골격근의 당원합성효소 활성도, 골밀도를 측정하여 다음과 같은 결과를 얻었다. 1. 성장호르몬 2주간 투여로 혈장 인슐린양성장인자-Ⅰ 농도와 간세포 인슐린양성장인자-Ⅰ 전령리보핵산의 발현이 정상대조군의 수준으로 회복됨을 알 수 있었다. 2. 성장호르몬을 2주간 투여후 지방량은 의미있게 감소하였으며, 제지방량은 의미있게 증가하여 성장호르몬이 체조성을 변화시킴을 알 수 있었다. 3. 정상혈당클램프검사 결과 성장호르몬투여군은 뇌하수체절제대조군 띤 정상대조군과 말초조직의 포도당이용율에 차이를 보이지 않았으며, 비복근에서 당원합성효소의 활성도도 차이를 보이지 않아 생리적 용량의 성장호르몬 투여는 당대사에 유의한 변화를 주지 않음을 확인할 수 있었다. 4. 뇌하수체절제군(제2군 및 제3군)은 정상대조군(제1군)에 비하여 골밀도가 감소하였으며 2주간의 성장호르몬 투여는 골밀도를 증가시키지 못하였다. 이상의 결과로 성장호르몬이 결핍된 흰쥐에서 사람성장호르몬 1일 4IU 2주간 투여는 혈장 및 간세포의 인슐린양성장인자-Ⅰ을 정상화시키며, 체조성을 변화시키고, 당대사를 악화시키지 않으며, 골밀도를 증가시키지 못함을 알 수 있었다. [영문] It is well known that growth hormone(GH) stimulates growth, but studies on metabolic effects of growth hormone have recently been increasing. The purpose of this study was to clarify the effects of growth hormone treatment on body composition, glucose and bone metabolism in hypophysectomized growth hormone-deficient rats. The 20-week-old male Sprague-Bawler rats were hypophysectomized and replaced with cortisol and thyroxine for S weeks, then administered with recombinant human growth hormone for 2weeks. Group 1 consisted of intact controls(n= 15), while group 2 consisted of hypophysectomized controls(n= 12), and group 3 consisted of those with GH treatment(n= 13). The body weights, body composition, blood glucose levels, plasma insulin-like growth factor-I(IGF-I) levels, IGF-I mRNA expression in liver, euglycemic hyperinsulinemic clamp test, glycogen synthase activities in gastrocnemius muscle, and bone mineral densities were measured before and after growth hormone treatment. Both plasma IGF-I levels and liver IGF-I mRNA expression in GH-treated group increased to intact control group levels after 2 weeks of GH treatment. There were significant changes in body composition after the treatment (fat mass significantly decreased and lean body mass significantly increased). There were no changes in glucose metabolism in peripheral tissue after 2weeks of GH treatment. Bone mineral densites significantly decreased in hypophysectomized rats(group 2 and 3) compared with intact control group. Two weeks of GH treatment did not increase bone mineral density. In conclusion, human GH treatment(4IU/kg/day) in adult hypophysectomized GH-deficient rats changed the body composition, but did not alter the glucose metabolism in peripheral tissue and the bone mineral density.restrictio

Liquid jet impingement on rotating substrates of various wettability

학위논문 (석사) -- 서울대학교 대학원 : 공과대학 기계공학과, 2021. 2. 김호영.고체 표면에 액체 젯이 충돌하면 액막이 방사 방향으로 퍼지며 hydraulic jump를 생성하는 것은 그동안 활발히 연구되어왔다. 본 연구에서는 액체 젯이 회전하는 소수성 표면에 충돌할 때 finger가 생성되며 배수로 역할을 하여 액막의 성장이 멈추게 되는 현상을 발견하였다. 이러한 현상은 반도체 공정 내의 회전 공정을 사용하는 경우, 액막이 표면을 다 덮지 못하게 되어 불균일한 공정 결과를 나타내는 등의 문제가 발생한다. 이러한 현상이 나타나는 이유를 파악하기 위하여 접촉각, 각속도, 액체의 물성, 유량이 액막의 성장에 끼치는 영향을 실험을 통하여 정성적으로 파악하였다. 또한 이를 바탕으로 steady-state에 도달한 액막의 반지름을 정확히 예측할 수 있는 이론 모델을 완성하였다.Prior studies have shown the formation of a hydraulic jump at a particular radius in flows impinging stationary surfaces. In the case of liquid jets impinging rotating surfaces, however, fingers form at a critical radius. These fingers act as drainage ducts, eventually causing the liquid film to reach a constant radius. Here we experimentally deduce that substrate wettability, angular velocity, fluid properties, and flow rate to affect the film's radius.
