(The) relationship between differentiation of self and mental health status of normal adolescents in Seoul

간호학과/석사[한글] 현대 사회에서 우리가 겪는 긴장과 스트레스는 날로 고조되어 가고 있고 이러한 긴장과 스트레스는 갈등과 좌절 등 정서적으로 불안정한 시기에 놓여 있는 청소년기에 역기능적 행동인 청소년 비행, 정신과적 증상이 나타날 수 있다. Bowen은 개인이 "미분화된 가족자아(Undifferentiated family ego mass)"로부터 분화되지 못하고 또 자기 정서적 기능에서 지적기능이 분화되지 못했을 때 역기능적 행동을 나타내고, 자아분화 수준이 어느 정도 높은 사람은 정서적으로 안정되어 있고 원칙지향적, 목표지향적 삶을 살며 타인의 가치와 신념을 있는 그대로 존중하면서 자기의 가치와 신념을 견지해 나가고, 자기 잘못을 타인에게 전가시키지 않고 책임질 줄 알며 타인과 친근한 정서적 관계를 갖는 잘 정의된 자아인 높은 수준의 정신건강 상태를 유지해 나간다고 하였다. 따라서 본 연구에서는 과연 자아분화 수준이 높을수록 정신건강 정도는 높게 나타나는지를 파악함으로써 청소년 간호중재에 필요한 기초자료를 제공하고자 본 연구를 시도하였다. 본 연구는 서술적 상관관계 연구로 서울시에 소재하고 있는 1개 남자 고등학교 1,2,3학년 146명, 1개 여자 고등학교 1.2,3 학년 153명을 대상으로 하였다. 자료 수집 기간은 1992년 3월 31일부터 4월 4일까지 5일간이었으며 자료수집 방법은 구조화된 질문지를 사용하였다. 연구 도구로는 자아분화를 측정하기 위하여 제 석봉(1989)이 Bowen의 자아분화 척도를 한국형으로 표준화한 척도를 사용하였으며, 개발 당시 신뢰도는 .90 이었다. 정신건강 수준을 측정하기 위하여 이 훈구(1986)가 간이화한 간이 정신진단 검사를 사용하였다. 선형 연구에서 이 도구의 신뢰도는 김범준(1987), 최정훈 등(1988), 김연숙(1991)의 연구에서 Cronbach's α=.96, .97. .95로 나타났다. 수집된 자료는 SPSS를 이용하여 실수와 백분율, 평균과 표준편차, Pearson Correlation Coefficient, ANOVA, Stepwise Multiple Regression등의 통계 방법으로 분석하였다. 본 연구의 결과를 요약하면 다음과 같다. 1. 대상자의 자아분화 정도의 점수는 101.58로 나타났다. 2. 대상자의 정신건강 수준은 99.06으로 나타났으며 이는 서울 지역 고교생의 정신건강 수준보다 낮게 나타났다. 3. 대상자의 자아분화 정도와 정신건강 수준과의 관계에 대한 주가설인 "자아분화도가 높은 청소년은 정신건강 수준이 높을 것이다"는 다음의 부가설을 설정하여 검증하였다. 제 1 부가설: "인지 정서적 기능이 높은 청소년은 정신건강 수준이 높은 것이다"는 지지되었다(r=-.53 p<0.001). 제 2 부가설: "자아 통합 정도가 높은 청소년은 정신건강 수준이 높은 것이다"는 지지되었다(r=.08 p>.05). 제 3 부가설: "가족 투사 과정이 낮은 청소년은 정신건강 수준이 높은 것이다"는 지지되었다(r=-.30 p<.001). 제 4 부가설: "정서적 단절 정도가 낮은 청소년은 정신건강 수준이 높은 것이다"는 지지되었다(r=-.36 p<.001). 제 5 부가설: "가족 퇴행 정도가 낮은 청소년은 정신건강 수준이 높은 것이다"는 지지되었다(r=-.41 p<.001). 4. 대상자의 하위 자아분화 정도에 영향을 미치는 하위 정신건강 수준은 1) 인지.정서적 기능과 관련된 제변수로는 적대감, 우울증, 편집증의 순서로 41.5%(p<.001) 설명되었다. 2) 자아 통합과 관련된 제변수로는 우울증, 적대감의 순서로 9%(p<.001) 설명되었다. 3) 가족 투사 과정과 관련된 제변수로는 적대감, 편집증, 정신증의 순서로 15%(p<.001) 설명되었다. 4) 정서적 단절과 관련된 제변수로는 적대감, 우울증, 강박증, 편집증의 순서로 21%(p<.001) 설명되었다. 5) 가족 퇴행과 관련된 제변수로는 적대감, 편집증의 순서로 18%(p<.001) 설명되었다. 5. 일반적 특성에 따른 대상자의 자아분화 정도와 차이를 분석한 결과는 1) 대상자의 자아분화 정도는 성별(F=6.88 p<.001), 학년(F=6.08 p<.001), 부모의 교육 정도(F=8.87. F=5.01 p<.001)에 따라 통계적으로 유의한 차이를 보였다. 2) 대상자의 정신 건강은 학년(F=6.70 p<.001)에 따라 통계적으로 유의한 차이를 보였다. 6. 추가분석으로 대상자의 하위 자아분화 수준별 하위 정신건강 수준과의 차이 검증 결과는, 자아분화 수준이 높은 그룹에서 자아분화 수준이 낮은 그룹보다 하위 정신건강 수준이 높게 나타났다(p<.001). 7. 추가분석으로 대상자의 정신건강 수준에 영향을 미치는 하위 자아분할 변수를 확인한 결과는 인지.정서적 기능. 가족 퇴행, 정서적 단절 순서로 41%(p<.001) 설명되었다. [영문] The purpose of this study was to test Bowen's family theory. Adolescents are in a vulnerable stage of mental health and hate many mental health problems and psychosis. In Bowen's theory differentiation of self is related to mental health status. So this study was designed to identify the relation between differentiation of self and mental health in adoleescents. Therefore the point of this study was to contribute to the theoretical understanding of the relationship between these two concepts and eventually to the enhancement of theoretically directed nursing care . The subjects for this study were 299 high school students attending a high school in Seoul;the number of first year, second year and third year students included was 101, 100 and 98 students respectively. The data were collected during the period from March 31, 1992 to April 4, 1992. For measuring the differentiation of self as described by Bowen a questionnaire by Je (1989) was used. A Simplified Mental Diagnostic Test adapted for Koreans from Derogatis, et al(1976) by Lee (1986) was used to measure the level of mental health. The data were analysed with the. SPSS program using descriptive statistics. Pearson Correlation Coefficients, ANOVA and Stepwise multiple Regression. The results of this study can be summarized as fellows: 1. The mean score for the differentiation of self scale was 101.88. 2. The mean score for the level of mental health was 99.06, which was higher than the results from other age-groups for which this tool has been used. 3. Hypothesis 1) "the higher the level of cognitive function vs emotional function, the higher the level of mental health" was supported(r=-.53 p<.001). Hypothesis 2) "the higher the level of integration of self, the higher the level of mental health" was not supported(r=.08 p>.05). Hypothesis 3) "the lower the level of family projection process, the higher the level of mental health" was supported (=r-.36 p<.001). Hypothesis 4) "the lower the level of emotional cutoff, the higher the level of mental health" was supported(r=-.36 p<.001). Hypothesis 5) "the lower family regression, the higher the level of mental health" was supported(r=-.41 p<.001). 4. By using Stepwise Multiple Regression Analysis, the main influencing factors for the fire different categories of differentiation of self were identified, as fellows: 1) For the first subcale of differentiation of self the "Cognitive function vs Emotional functional"; three main variables; hostility, depression, and paranoid ideation explained 41.5%(p<.701) of the variance. 2) For the second subcale "Integration of self": two main variables; depression and hostility explained 9%(p<.001) of the variance. 3) For the third suhcale wFamily projection process": three main variables hostility, paranoid ideation, and psychosis explained 15%(p<.001) of the variance. 4) For the fourth subcale "Emotinal cutoff"; four main variables hostility, depression, obscession, and paranoid ideation explained 21%(p<.001) of the variance. 5) For the fifth subcale "Family regression"; tee main variables hostility and paranoid ideation explained 18%(p<.001) of the varlarlce. 5. 1) The results from the tests for the differentiation of self in the area of demographic variables showed that there were significant differences related to sex(F=6.88 p<.001), school year(F=6.08 p<.001), and educational level of parents(F=8.87, F=5.01 p<.001). 2) The results from the tests for the level of mental health in the area of demographic variables showed that there was significant difference related to school year(F=6.70 p<.001). 6. The relationship of differentiation of self to the nine different categories of mental health status were as follows: The higher the degree of differentiation of self was, the higher the results of the nine different categories of mental health status were(p<.001). 7. By using Stepwise Multiple Regression Analysis; the main infuencing factors on mental health status were found to be congnitive function vs emotional function, family projection process and emotional cutoff. These factors accounted for 4171p<.001) of the variance. In conclusion, this study revealed that differentiation of self is an important factor influencing mental health status. Further multidimensional study is needed to identify ways to improve the mental health of adolescents through the differentiation of self. Future research of these conceptual relationships is essential for the development of theoretical nursing that will give direction to the planning of scientifically sound nursing care.restrictio

Study of the complaint of respiratary symptoms by air pollution shown in middle school students : application

보건학과/석사[한글] 대기오염이 인체에 미치는 영향에는 많은 요인이 관계되므로 원인과 결과의 인과관계를 극명하기에는 어려운 점이 많다. 그러나 London smog 사건등 대형사건과 여러학자들의 보고에 비추어 볼때 대기오염이 그 정도에 따라 인체에 영향을 준다는 것을 알 수 있다. 이에 저자는 대기오염의 정도에 따른 호흡기 장애 호소를 비교 관찰하였다. 아황산가스 오염도 조사 결과 100ppb이상인 서울특별시 영등포구 지역을 대기오염도가 높은 지역으로 선정하고 아황산가스농도가 30∼50ppb로 비교적 낮은 강서구 지역을 대조지역으로 선정하였다. 조사지역에서는 2개학교에서 확률표본추출하여 549명, 대조지역에서는 1개학교에서 확률표본추출하여 541명을 대상으로 하였으며 대기오염에 의한 호흡기 증상에 대한 어떤 호소상태를 1983년10월에 BMRC식 설문서(Standardized questionnaires of BMRC)를 사용하여 조사하였다. 연구 결과는 다음과 같다. 1. 조사된 6가지 호흡기 증상(기침, 담, 가래, 숨소리가 거칠다, 콧물, 코감기, 숨이 차다. 기후의 영향에 따른 증상악화)에 대한 호소율은 대조군에서는 6.31%, 조사군에서는 9.01%로 나타났으며 두 지역간의 숨이 차다, 코감기, 담, 가래 등 증상의 호소율에는 유의한 차이가 있었다. 2. 남·녀간의 호소율에는 유의한 차이가 없었다. 3. 학년간의 호소율을 보면 저학년과 고학년의 호소율에는 유의한 차이가 없었다. 4. 거주연한별 호소율은 조사군에서는 10년이상 거주한 학생에서, 대조군에서는 8∼10년 거주한 학생에서 최고치를 보였으며, 두 지역에서 모두 거주연한간의 호소율에는 유의한 차이를 볼 수 없었다. [영문] Aiming to find out any relationship between the complaint ratio of middle school students and air pollution, standardized questionnaires of BMRC was applied to 549 students in Young-Dung-Po-Ku, Seoul, where most of the area was highly polluted and to 541 students in Kang-Seo-Ku, as the control, which was less polluted, compared with the above mentioned area. The results of the study are as follows: 1. A significant difference in the complaint ratio between the investigated and the control was found that BMRC score of sputum, breathlessness, and nasal catarrh showed higher score in the investigated group than in the control group. 2.
