    Three Cases of Spine Fractures after an Airplane Crash

    While injuries to the spine after an airplane crash are not rare, most crashes result in fatal injuries. As such, few studies exist that reported on spine fractures sustained during airplane accidents. In this report, we demonstrate three cases of spine fractures due to crash landing of a commercial airplane. Three passengers perished from injuries after the crash landing, yet most of the passengers and crew on board survived, with injuries ranging from minor to severe. Through evaluating our three spine fracture patients, it was determined that compression fracture of the spine was the primary injury related to the airplane accident. The first patient was a 20-year-old female who sustained a T6-8 compression fracture without neurologic deterioration. The second patient was a 33-year-old female with an L2 compression fracture, and the last patient was a 49-year-old male patient with a T8 compression fracture. All three patients were managed conservatively and required spinal orthotics. During the crash, each of these patients were subjected to direct, downward high gravity z-axis (Gz) force, which gave rise to load on the spine vertically, thereby causing compression fracture. Therefore, new safety methods should be developed to prevent excessive Gz force during airplane crash landings.ope

[영문] Despite evidence that depression influences quality of life for older people, much of the research literature has focused on surveys for epidemiological data and contributing factors, but not on exploring depression as a whole. Additionally, interventions for depression are provided without considering the contextual nature of life. So this study was done in order to provide an understanding of depression in context and the use of a narrative approach in the practice of nursing The purpose of this study was to understand depression in the context of the lives of elderly people and to help them in the process of evaluating their past, accepting the present, and recreating meaning in their lives. The data were gathered from audio-taped, in-depth interviews with five elderly people from a low-income community. The interviews were carried out in the person's home and were analyzed based on Kvale(1996)'s approach to the interpretation of interviews. Through the narrative approach, the elderly described their experience of something which had meaning in their past life now being lost. Because of this loss, they experienced negative emotions and gradually became isolated from society. Participants found release from their emotions through telling, and retelling their story in an interactive process with the researcher and thus moved toward society by recreating meaning. That is, elderly depression occurred from the experience of loss, and the depressive phenomena appeared from living with the loss. They could break loose from the loss by telling their life story and recreating the meaning. This study allows nurses to see depression as a contextual emotion in the life of human beings, and not as one separate health problem, and see that elderly people as the subjects of the study could be empowered to resolve their own depression through in-depth interviews with the researcher.     Thermal Dynamics of a Novel Radio-Frequency Device for Endoscopic Spine Surgery: An Experimental Model

    Study design: Experimental study. Objective: In this study, the ambient temperature of a radiofrequency (RF) electrode tip was compared and analyzed in terms of products, mode, flow quantity, and flow rate. Summary of background data: Endoscopic spine surgery is a widely used operation for degenerative lumbar stenosis and herniated lumbar disc. To perform endoscopic spine surgery, dedicated instruments like a RF generator and electrode are essential. Methods: An evaluation system capable of measuring temperature under equal conditions at a certain distance from the electrode tip was manufactured. The distance between the electrode tip and the temperature sensor was set to 1, 5, and 10 mm. The flow quantities of 0, 50, 100, and 150 mL/min and the flow rates of 0, 0.20, 0.53, and 0.80 m/s were compared and statistically analyzed. Results: The temperatures measured in the experiments conducted on the four combinations of RF device showed similar values, and showed differences according to the characteristics of each mode of the RF. As the distance between the electrode tip and the temperature sensor increased, the temperature decreased, and as flow quantity or flow rate increased, the temperature decreased. The maximum temperatures differed significantly according to flow quantity, between flow quantities of 0 and 100 mL/min (P = 0.03) and between 0 and 150 mL/min (P ≀ 0.01). The maximum temperatures also differed significantly between the flow rate of 0 m/s, and the flow rates of 0.20, 0.53, and 0.80 m/s, with P ≀ 0.01 in all three comparisons. Conclusion: This is the first study in which we made a customized RF temperature evaluation system and verified the temperature changes in various environments. When irrigation was performed, we could confirm that the maximum temperature was less than 60Β°C. Irrigation is considered essential in endoscopic spine surgery. Level of evidence: 3.restrictio
