A Clinical Study of Induction Chemotherapy with a Low-Toxic Regimen for Squamous Cell Carcinoma of the Head and Neck

의학과/석사[영문] [한글] 두경부 편평상피세포암의 치료로서 수술 및 방사선 요법이 오랜 동안 시행되어 왔고 발 전을 거듭하였으며, 최근에는 이들 국소요법의 효과를 높이기 위한 수단으로서의 유도화 학요법의 중요성이 점차 인식됨에 따라 여러 종류의 새로운 섭생법이 고안된 바 있다. 이에 저자는 1984년 3월부터 1986년 2월 사이에 본원 이비인후과를 방문하여 병리조직 학적 검사에서 두경부 편평상피세포암으로 확인된 43예의 환자에게 cia-platinum, vinbla stin 및 5-fluorouracil 3제 병합요법으로 저독성 섭생 유도화학요법을 시행하여 그 결과 를 분석 합으로써 앞으로의 치료에 기여하고자하였으며, 그 결과, 다음과 같은 성적을 얻 었다. (1) 전체 49예중 완전관해 5예, 부분관해 27예, 안정 상태 11예로서 74.4 %의 객관적 관해율을 보였다. (2) 원발병소의 위치에 따른 관해율은 구강이 85.7%로 가장 높았고, 그 다음으로 후두, 구강인두, 하인두, 비인강의 순이었다. 그러나 이들 사이에서 통계학적인 의의는 찾을 수 없었다. (3) 원발병소 및 경부 임파절의 상태, 병기 그리고 세포의 분화정도에 따른 관해율에 유의한 차이는 없었다. (4) 유도화학요법으로 인한 국소요법상의 장애는 없었으며, 진행된 예도 없었다. (5) 유도화학요법에 의한 독작용으로는 74.4%에서 오심 혹은 구토를 보였으며, 간혹 설 사, 구내염 및 백혈구 감소증을 나타냈으나, 이로 인하여 치료를 중단하거나, 국소치료에 장애를 주지 않을 정도의 경미한 것이었으며, 그 외에 신독성, 이독성등은 나타내지 않 았다. (6) 전체 43예중 20예인 46.5%가 60세이상의 고령자였으며, 28예인 65.1 %가 폐질환을 동반하고 있었으나 저자의 저독성 섭생 유도화학요법으로 충분한 양의 약제를 투여할 수 있었다. (7) 유도화학요법에 부분관해를 보인 2예에서 추후의 국소요법을 거부한 예를 경험하였 으므로, 보다 완전한 치료를 위해서는 환자와 가족들에 대한 충분한 사전설명이 필요할 것으로 사료되었다. (8) 병기 및 유도화학요법에 대한 잔해정도에 따른 생존기간의 차이를 알기 위해서는 계속적인 추적조사와 관찰 및 분석으로 원격성적이 필요하다고 본다. A Clinical Study of Induction Chemotherapy with a Low-Toxic Regimen for Squamous Cell Carcinoma of the knead and Neck Kyoung Lee, M.D. department of Medical Science The Graduate School Yonsei University (Directed by Prof. Gill Ryoung Kim, M.D.) A variety of adjuvant chemotherapy programs have been used to try and improve the effectiveness of local treatment such as surgery and radiation therapy for squamous cell carcinoma of the head and neck. A clinical study was done using statistical analyses to in-vestigate the effectiveness and feasibility of induction chemotherapy with cis-platinum, vinblastin and 5-fluorouracil. Forty-three patients who were diagnosed pathologically as squamous cell carcinoma of the head and neck from Mar. 1984 t? Feb. 1986 were evaluated . The results were as followed : 1. Among a total of 43 cases, 27 cases (74.4%) showed objective remission and there were 5 cases, of complete remission and 22 cases of partial remission. 2. Remission rates related to various primary tumor sites ravealed that oral cavity (85.7%) had the highest remission rate fellowed by larynx, oropharynx, hypopharynx, and nasopharynx. 3. Remissions were not significantly Influenced by primary tumor status, nodal status or the degree of differentiation of tumor cells. 4. During or after local treatment, there were no significant disturbance related to induction chemotherapy. 5. For patient who showed objective remission to induction chemotherapy, there was a need for adequate explanation of the significance of local treatment. 6. Long-term follow-up was suggested so that the relationship could be determined between survival rate and remission rate to induction chemotherapy, and survival rate and status of the disease. 7. Induction chemotherapy produced only mild toxic reactions such as nausea, vomiting, diarrhea and mucositis. These reactions were easily controlled.restrictio
