A Severe Retropharyngeal Hematoma after Stellate Ganglion Block: A case report

A 73-year-old man with sudden sensory neural hearing loss received a stellate ganglion block. Two hours after the block, the patient complained of newly developed neck discomfort. After an additional two hours, the neck swelled up gradually and neck pain and dyspnea developed. A plain radiograph of neck revealed narrowing of the upper airway; a tracheostomy was performed and the dyspnea was improved. On the next day, the pain site extended to the right scapula and a CT image revealed a huge retropharyngeal hematoma. Hematoma evacuation and bleeder ligation were then performed and the patient was discharged on the fourth day after admission without any complications. A practitioner should always remember to educate the patients about possible complications and undertake intensive observation when performing procedures, even in patients who do not initially present with a compromised airwayope

Design of Artificial Drawing Artist Using Deep Reinforcement Learning

학위논문(석사) -- 서울대학교대학원 : 인문대학 협동과정 인지과학전공, 2022. 8. 장병탁.드로잉은 미적 감각과 심사숙고 능력을 동원한 인간 고유의 예술적 표현 방법이다. 그동안 컴퓨터 그래픽을 활용한 규칙기반의 여러 자동 드로잉 기법이 개발되었지만 다양한 드로잉 기법을 묘사하는 데에서 그 정교함과 예술성에 한계가 있었다. 그러나 최근에는 딥러닝 기술이 드로잉에 응용되면서 이전의 간극을 채우기 시작하였고, 예술 영역에서 인간과 인공지능의 경계가 빠르게 무너지고 있다. Stroke-based rendering (SBR)은 컴퓨터를 이용해서 스트로크라고 불리우는 붓터치와 같은 드로잉 요소의 위치나 색감 등을 고려해 가며 순차적으로 그림을 완성하는 문제이다. 본 학위논문에서는 주어진 목표 이미지에 대하여 자동적으로 SBR을 수행할 수 있도록 에이전트를 강화학습하는 내용을 다룬다. 그림의 표현 기법은 예술적 의도에 따라 다양하기 때문에, 각 기법에 강화학습이 적용되는 양태 또한 다양할 수 있다. 본 논문에서는 4가지의 드로잉 시나리오를 제시하고 각 시나리오에 맞춰 강화학습을 적용한다. 제시되는 SBR 시나리오는 크게 펜 드로잉과 고급 드로잉 기법으로 나누어진다. 펜 드로잉에서는 단색의 펜을 활용하여 그리는 기법을 다루며, 행동의 단순성을 이용해 실제 로봇을 이용한 드로잉으로 확장시켰다. 고급 드로잉 기법에서는 여러 스트로크에 대하여 일반화된 스트로크-조건부 페인팅 및 여러 재료를 모아 그림을 완성하는 콜라주 기법을 다룬다. 제시된 방법을 참고하면 드로잉과 관련한 인공지능 에이전트를 설계하고자 할 때 도움이 될 수 있다. 이러한 기술은 드로잉 소프트웨어, 영화, 애니메이션, 장난감, 공연 및 전시 등의 다양한 예술 산업에 활용될 수 있다.People easily draw to express their thoughts, while drawing requires some level of aesthetic sense or even pondering ability. We can find drawing is a high-intellectual behavior when we recall the fact that drawing is common in people but not in animals. For that reason, hand-crafted algorithms devised for automated drawing have limitations representing the subtle pattern of diverse drawing styles. However, several recent artworks have engaged deep learning techniques, filling the gap between computational automation and aesthetic sense. Stroke-based rendering (SBR), one of the computer graphics-based drawing tasks, is a task placing ``stroke" such as brush touches and stipples on the digital canvas to complete target drawings. In this article, total 4 kinds of reinforcement learning algorithms for each style of SBR scenarios are presented. Those scenarios are divided into pen drawings and high-level drawings: pen drawing denotes doodle-like drawings drawn with single-colored pen, while high-level drawing denotes painting-like drawings drawn with diverse realistic strokes or some parts of magazines. A study on validity of for-profit medical corporation : focused on legal and ethical assessment : 법적·윤리적 평가를 중심으로

의료법윤리학협동과정 법학전공/석사[한글] 영리법인의료기관을 윤리적으로 바라보면 영리법인의료기관의 도입으로 인해 의료공급에 있어 평등주의가 깨질 수 있다고 우려하는 바가 있다. 의료공급의 평등주의를 이유로 의료산업이 강화되지 못하는 것은 바람직 하지 못하다. 산업의 경쟁력을 통해 수익의 창출로 공공의 대책을 세우는 것이 옳다. 이는 요사이 실용정부의 정책이념에서도 잘 읽을 수 있다. 현실의 모습에서 자유주의냐 평등주의 인가를 논하는 것은 의미가 없다. 변화하는 흐름에 따라 성장하는 모습 속에서 정책을 펼칠 필요가 있다. 싱가폴, 태국과 같이 국가주도적으로 의료 산업을 육성하고 국가의 원동력으로 삼으려는 연구와 움직임이 보인다. 의료 산업을 중심으로 한 고부가가치 산업(생명공학, 의료기기, 의약산업)의 육성과 의료 허브의 모습을 구상하고 계획되고 있는 것이다. 의료산업의 발전을 통해 국부창출을 이루고 산업형성에 따른 일자리 창출, 고비용, 고급의료 서비스 실현으로 인한 부의 분배효과면에서 긍정적으로 생각한다. 또한 전체 보건의료산업의 수익에 대한 세목과 특별부담금 형식의 재원조달로 공공의료로의 순환구조도 가능할 것이다. 의료윤리의 4원칙으로 영리법인의료기관을 바라보면 다음과 같다. 자율성 존중의 원칙에서 살펴보면 영리법인의료기관 개설에 있어서도 의료인, 환자, 투자자가 자율성이 존중되어야 한다는 견해가 있다. 의료기관개설 형태에 대한 결정권, 의료소비자의 선택의 자유, 의료기관을 개설하고 싶어하는 의사아닌 일반인의 직업선택의 자유가 제약되어 있다. 악행금지의 원칙에서 살펴 보면 영리법인의료기관의 허용으로 의료비의 부당한 징수와 과잉진료, 탈세등의 ‘악행’을 더 조장할 수 있다는 이야기를 많이 한다. 그러나 영리법인의료기관의 도입으로 의료기관이 회사의 개념으로 활동하여 투명하고 건전하게 발전됨으로서 세제와 경영에 있어 신뢰할 수 있는 분위기를 조성시키고 ‘악행’이 제거되어 악행금지의 원칙에 부합 될 수 있을 것이다. 선행의 원칙에서 살펴보면 ‘적극적 선행의 원칙’은 영리법인의료기관제도의 도입으로 영리법인의료기관 운영과 보건의료산업의 수익에 대한 세금으로 공공의료에 대한 투자를 늘리고 저소득층에 대한 의료자원을 늘리는 것으로 적용할 수 있을 것이다. ‘효용원칙’에서 의료에 있어 ‘효용’은 적은 비용으로 질높은 의료서비스를 제공하는 것으로 볼 수 있는데 영리법인의료기관이 도입되면 환자유치를 위하여 환자들이 원하는 수준의 의료의 질을 유지할 것이나, 이윤추구를 위하여 적정한 비용투입이 이루어 질 것이므로 ‘효용’이 높아 질 수 있을 것이다. 영리법인의료기관 도입을 롤즈의 정의의 원칙에서 살펴 보면 ‘차등의 원칙’을 적용 했을 때 국민에게 돌아가는 혜택은 영리법인의료기관 운영과 보건의료산업의 수익에 대한 세금으로 공공의료에 대한 투자를 늘리고 저소득층에 대한 의료자원을 늘리는 것이다. 또한 보건의료산업을 통한 국부의 창출, 산업형성에 따른 일자리 창출, 고비용, 고급의료 서비스 실현으로 인한 부의 분배효과면에서 긍정적으로 생각할 수 있다. 또한 영리법인의료기관의 도입으로 상법상 회사로 활동한다면 시장원리의 공급자의 한축으로 투명성과 그에 따른 경쟁력을 가지고 의료서비스를 한다면 국민은 보다 나은 의료서비스를 낮은 가격으로 받을 수 있을 것이다. 두 번째로 기회의 균등에서 균등함 이란 공정함을 의미하고 어떠한 현안이나 문제에 대해 어느 누구도 차별 받지 않고 접근할 수 있는 기회의 균등을 말한다. 영리법인의료기관의 허용으로 기회의 균등함을 바라볼 때 의사에게는 회사형태의 의료공급자로 이윤을 추구할 수 있는 기회를 투자자에게는 의사가 아닌 일반인으로서 의료기관을 개설할 수 있는 기회를 갖게 할 것이다. 의료 소자자인 일반국민에게는 영리법인의료기관 허용으로 의료에 접근할 수 있는 기회인 접근성이 떨어 진다는 우려를 많이 한다. 그러나 현재 영리의료법인은 의료기관의 해외브랜드 수출을 위한 자금확보의 수단이나, 고비용·고급의료의 시도에서 논의되고 있고 대형종합병원은 비영리기관으로서 차후에도 건재해 접근성이 떨어지지 않을 것으로 예상된다. 법학적 관점에서는 우리나라는 자유민주주의 공화국이며(헌법 제1조) 자유주의를 천명하고 있는바 시장을 인정하고 사유재산권을 보호한다.(헌법 제119조 시장경제와 사적재산권 보장) 의료영역 또한 이러한 자유주의 원리가 적용된다.(헌법 제10조 행복추구권, 15조 직업의 자유) 그러나 의료의 특성상 사회적으로 구성원에게 필수 · 필요적으로 서비스가 제공되어 져야 함이 있어 평등주의적 분배개념이 강조된다. (헌법 제36조 3항 보건권, 31조~36조 사회국가원리) 이러한 의료영역에 있어 헌법상 자유주의와 평등주의의 대립에 대해 헌법재판소 99헌바76, 2000헌마505(병합) 요양기관 강제지정제 심판에서 였볼 수 있다. 의료법상 의료기관은 의료인과 법인으로는 비영리법인인만이 개설할 수 있다. (의료법 제30조 2항) 또 의료법인에 대해서는 민법의 재단법인의 내용을 준용한다. 그러나 세법상 법인세법에서는 의료업 자체를 수익사업으로 규정하고 과세함으로써 기본적으로 일반 영리법인과 동일한 과세부과 원칙이 적용된다. 이렇게 실질적으로 비영리법인의 형태를 취하고 있지만 실질적으로 세법상 영리법인으로 취급하는 것에 대하여, 법인의료기관에 영리법인을 허용하여 회계와 경영을 투명화 시켜 적합한 세제를 취하고 탈세와 위법행위를 근절하는 것이 바람직할 것이다. 또 의료법인으로서 비영리의료법인으로 남는 기관은 공익성을 더욱 강화 시키고 적합한 세제의 이익을 주는 것이 올바르다 [영문]In the past, the emphasis of medical field was put on factors such as publicism, egalitarianism and approach, establishing 'equal state control' rather than market or competitive principles. But, it doesn't make sense that medical field becomes standardized downwardly, preventing the field itself from becoming standardized upwardly, due to the cause of egalitarian-oriented medical supply. Accordingly, it needs to take public measures against the downwardly-standardized medical field trend, implementing upwardly-standardized medical system. The necessity of such upwardly-standardized medical field trend is plainly reflected in policy lines and ideas that the pragmatism-driven government pursues in recent years. It is meaningless to discuss whether liberalism will come before egalitarian or vice versa. Instead, it needs to make policies in the process of growing development in accordance with the changing current. It is expected to induce national wealth by developing health and medical industry, create industrial employment foundation, and provide high-cost and advanced services will have positive effects on wealth division. Also, recycling structure in public medical field would possibly be built, by financing specifications of profits from the extensive health and medical industry scope in the form of special charge amount. Competition, efficiency and market principles are required for growing medical field. Obviously, it is difficult to expect cost and efficiency will be formed following the created cost and services, owing to competition between demander and supplier appearing when buys and sells medical equipment in some store, in the aspect of medical field characteristics. But, it is deemed to offer the scope of multi-choices to medical consumers, promoting medical field-related advertisements and strengthening rights of choices among the general consumers, so that they can give an access to much information, and then introducing private insurance system as the payment form. Also, introducing the profit-corporation supplier agency will enable competitiveness to become enhanced as the commercial law-related firm, thanks to massive capital inputs. And, it will help make transparent management and taxation and developing competitive agency and industry, as well as providing low cost and good-quality services to medical consumers or patients. This application of the extensive market principles will give birth to competitive medical agency besides state-leading non-profit medical agency. And, based on it, medical field will be fostered and expanded, with the result of the reinforced health and medical industry field, such as life engineering, medical equipment and pharmaceutical industry. And, subsequently, medical hub will be built. The situation in profit-corporation medical agency overseas shows that special medical services and old-aged medical care services are run to a considerable degree. If profit-corporation is introduced in South Korea, added-value services and non-regulation medical services will be created. A wide range of medical advertisements necessary for medical information, and private medical insurance must be introduced with various payment forms, and medical care agency selecting system has to be facilitated for a better institutional climate. Sonographic estimation of needle depth for cervical epidural blocks.

BACKGROUND: Cervical epidural steroid injections are often used to treat acute and chronic pain syndromes involving the face, neck, and upper extremities. Ultrasound has evolved as a valuable tool for performing neuraxial blocks, providing useful prepuncture information on the structure. Our goal was to evaluate the accuracy and precision of ultrasound by comparing skin to dura distance from ultrasound with the actual skin to epidural depth. METHODS: We enrolled 50 patients undergoing cervical epidural blocks at the pain clinic. Ultrasound images with transverse and longitudinal median views of the C6/7 area were taken. The epidural needle was inserted, reproducing the direction of the ultrasound beam on the longitudinal median view. Measured distances from skin to dura on each ultrasound view were compared with the actual needle depth. Additionally, we examined ultrasound visibility, the number of puncture attempts, and any complications related to the procedure. RESULTS: Concordance correlation coefficients between the measured distances on ultrasound and actual needle depth were 0.9272 and 0.9268 on transverse and longitudinal median view, respectively. The cervical epidural block was successfully performed on 48 patients (96%). There were two incidents (4%) of dural puncture. No bloody taps, postprocedure complications, or hemodynamic instability related to cervical epidural blocks occurred. CONCLUSIONS: Ultrasound provides very accurate information on the skin to dura distance for epidural blocks in the cervical spine. Numerical Analysis of Steam-methane Reforming Reactio for Hydrogen Generation using Catalytic Combustion

A steam reformer is a chemical reactor to produce high purity hydrogen from fossil fuel. In the steam reformer, since endothermic steam reforming is heated by exothermic combustion of fossil fuel, the heat transfer between two reaction zones dominates conversion of fossil fuel to hydrogen. Steam Reforming is complex chemical reaction, mass and heat transfer due to the exothermic methane/air combustion reaction and the endothermic steam reforming reaction. Typically, a steam reformer employs burner to supply appropriate heat for endothermic steam reforming reaction which reduces system efficiency. In this study, the heat of steam reforming reaction is provided by anode-off gas combustion of stationary fuel cell. This paper presents a optimization of heat transfer effect and average temperature of cross-section using two-dimensional models of a coaxial cylindrical reactor, and analysis three-dimensional models of a coaxial cylindrical steam reformer with chemical reaction. Numerical analysis needs to dominant chemical reaction that are assumed as a Steam Reforming (SR) reaction, a Water-Gas Shift (WGS) reaction, and a Direct Steam Reforming(DSR) reaction. The major parameters of analysis are temperature, fuel conversion and heat flux in the coaxial reactor.N
