(The) effect of capsaicin upon DMBA induced cancer of the buccal pouch in hamsters

치의학과/박사[한글] 화학적 암 예방이란 하나 또는 그 이상의 화학 물질을 약제나 음식물로 투여하여 암을 예방하거나 진행을 억제하는 것으로 정의되어질 수 있다. 1846년 Thresh에 의해 처음으로 명명된 capsaicin(trans-8-methyl-N-vanillyl-6-nonenamice)은 한국인의 식단에서 빠질 수 없는 식품인 고추의 주성분으로서 음식물의 첨가제, 방부제, 또는 여러 약제에 사용되어지고 있다. Capsaicin에 대한 많은 동물실험에서 위암, 피부암 등에서는 억제작용이 있다는 결과들을 보여주고 있으나 구강암 발암과정에서의 효과에 대해서는 보고가 미흡한 실정이다. 이에 저자는 향후 구강암 예방에 도움을 얻고자 9,10-dimethyl-1,2-benz(a)anthracene(이하 DMBA) 유도 햄스터 협낭 발암과정에서 고추의 주성분인 capsaicin의 효과를 알아보기 위하여 생후 4-5주된 체중 80-90g의 Golden Syrian Hamster 90마리를 2-3주간의 적응기간을 거친 후, 생존된 87마리를 정상군 2마리, 대조군 15마리 및 실험군 70마리로 분류하였고, 실험군은 다시 실험 I군 23마리, 실험 Ⅱ군 23마리, 실험 Ⅲ군 24마리로 나누어, 대조군 및 모든 실험군에 DMBA를 햄스터 협낭에 도포하였고, 실험 Ⅰ, Ⅱ, Ⅲ군 중 실험 I군에서는 capsaicin을 도포, 실험 Ⅱ군에서는 경구투여, 실험 Ⅲ군에서는 도포 및 경구 투여하였다. 모든 약제의 투여는 주 3회 17주간 시행하였으며 5, 8, 11, 14, 17주에 동물을 회생하여 광학현미경적 검경과, Bromodeoxyuridine(이하 BrdUrd)를 이용한 면역조직학적 검색을 통해 다음과 같은 결과를 얻었다. 1. 육안적 관찰에서 실함 I군과 Ⅲ군은 5주째에는 백반증, 8주째에는 유두종이 대조군과 실험 Ⅱ군에 비해 빠르게 출현하였다. 2. 육안적 관찰에서 대조군과 모든 실험군은 14주째부터 돌출성 종물의 소견을 보였으나, 돌출성 종물의 수와 발현 범위가 실험군에 비해 대조군에서 크게 나타났다. 3. 광학현미경적 관찰에서, 실험 8주째 대조군을 제외한 모든 실험군에서는 유두종이 발생하였다. 4. 실험 14주째부터는 침윤성 상피암의 침윤범위나 정도가 모든 실험군에 비해 대조군에서 심한 소견을 나타냈으며, 17주째에는 실험 Ⅰ, Ⅱ, Ⅲ군간의 조직소견은 차이가 없었다. 5. BrdUrd 표지지수는 실험 8주째 대조군에 비해 실험 Ⅱ군은 별차 없었고, 실험 Ⅰ, Ⅲ군은 통계학적으로 유의한 증가를 나타내었다. (p<0.05) 6. BrdUrd 표지지수는 대조군에 비해 실험 Ⅲ군은 11주째부터, 실험 I군은 14주째부터, 실험 Ⅱ군은 17주째에 통계학적으로 유의한 감소를 나타내었다.(p<0.05) 7, BrdUrd 표지 세포들은 정상군에서는 기저층에서, 상피 이형성이 나타나는 시기에는 기저층파 부기저충에, 상피내암이 보이는 시기에는 상피 전층에 분포되었다. 이상의 결과에서 capsaicin의 매우 자극적인 성질이 초기에는 오히려 발암과정을 촉진 시키나 상피내암 발현 이후에는 억제하는 효과를 나타내 한국인의 음식에서 큰 비중을 차지하는 고추가 구강암 예방물질로서의 가능성이 있음을 보여 주었다.그러나 capsaicin의 투여 경로와 용량이 발암과정에서의 억제 정도에 영향을 미치는 결과를 볼 때, 향 후 구강암의 예방효과를 위한 capsaicin의 적정 용량과 투여 경로에 대한 계속적인 연구가 요하리라 사료된다. [영문] Cancer chemoprevention can be defined as prevention or intervention of cancer by the administration of one or more chemical entities, either as individual drugs or as naturally occurring constituents of the diet. The name capsaicin(trans-8-methyl -N-vanillyl -6- nonenamide) was given by Thresh in 1848, and capsaicin is a primary pungent and irritating principle present in red poppers which are widely used as spices in korean food. The inhibitory effect of capsaicin on stomach or skin cancers had been reported in lots of animal studies, but there were few reports in oral cancer carcinogenesis. This study is aimed to see the effect of capsaicin upon DMBA induced cancer of the buccal pouch in hamsters. In this study, 87 Golden Syrian hamsters, weighing about 80-9Og, were used and divided into 5 groups :normal group(n=2)-untreated ; control group(n=15)-DMBA painted ; experimental group I(n=23)-capsaicin(10 μmol/2ml) and DMBA painted ; experimental group Ⅱ(n=23)-capsaicin administered and DMBA painted ;experimental group Ⅲ(n=24)-capsaicin administered and painted, DMBA painted. According to groups, the animals were sacrificed at 5, 8, 11, 14, 17week. Microscopic examination was done and BrdUrd labeling index was calculated. The results are as follows; 1. According to gross examination, the leukoplakia at 5 week and the papilloma at 8 week were shown in the experimental group Ⅰ, Ⅲ. Those feature were appeared rapider than those of the control and experimental group Ⅱ. 2. According to gross examination, the exophytic tumorous lesions were shown in the control and all of the experimental groups from 14week, but the features of the control group were severer than those of the experimental groups 3. According to histopathologic features, the papilloma was shown in all of the experimental group except the control group at 8 week 4. Histologically, the features of the control group were severer than those of the experimental groups from 14 week and there were similiar features among the experimental group Ⅰ, Ⅱ, Ⅲ at 17 week 5. At 8 week, BrdUrd labeling index of experimental group Ⅰ, Ⅲ were higher than that of control group and it was statistically significant(p<0.05) 6. The BrdUrd labeling index of experimental group Ⅲ from 11 week, experimental group I from 14 week, experimental group Ⅱ from 17 week were lower than that of the control group respectively and it was statistically significant(p<0.05) 7. In the distribution of the BrdUrd labeled cells, BrdUrd was more significant in the basal and parabasal cells in dysplatic changes, but in carcinoma in situ, BrdUrd showed a irregular arrangement throughout the whole thickness of the tumor epithelium According to the above results, the irritating property of the capsaicin accelerated the carcinogenesis in early phase, but the inhibitory effect was shown from carcinoma in situ phase. So the chemopreventive effect of the capsaicin which is a major ingredient of red pepper can be expected. However, the further studies including the proper amount and method of capsaicin administration for preventive effect of oral cancer, should be followed.restrictio

(A) clinical study on patients with acute supprative infection in the oral and maxillofacial region

치의학과/석사[한글] 구강악안면 영역의 화농성 감염증은 빠르게 진행되며 또한 종창에 의한 기도 폐쇄와 두개강내로의 확산에 의한 뇌막염 및 패혈증등을 야기하는 치명율이 높은 중증의 급성질환이 되기도 하여 이에 대한 연구가 계속되고 있다. 화농성 감염증의 치료는 원인과 해부학적 부위가 연관된 정확한 진단과 신속하며 적절한 외과적 처치와 전신상태를 호전키 위한 대중요법 및 항생제 투여로 이루어져야 한다. 그동안 구강악안면 영역의 화농성 감염에 대한 맡은 지식과 경험이 축적되어 비교적 합리적인 치료가 이루어져오고 있으나 원인 치아에 대한 인식결여 및 해부학적 독성이 고려되지 않는 부적절한 배농술, 통상적인 호기성 농배양법 및 경험적 항생제 투석시의 문제점과 합병증 등의 심각한 후유증이 나타나고 있는 것 이 현실이다. 이에 저자는 1981년 3월 부터 1991년 3월까지 10년간 정부를 포함한 구강 악안면 부위의 급성화농성 감염으로 인하여 연세의료원에서 입원가료 받는 남자 108명, 여자 105명, 총 213명의 병록지를 검토하여 다음과 같은 결론을 얻었다. 1. 원인별로는 치성감염이 71예(57.7%), 비치성 감염이 13예(6.1%), 원인미상이 77예(36.2%)순이었으나 원인미상군은 타과에 입원한 88명중 구강검진을 받지 못한 77명으로 병록지상으로도 원인을 찾을 수 없었다. 2. 213예중 농배양 검사를 받은 190예서 연쇄균주가 170예(81.0%), 포도균주가 25예(11.7%)로 거의 대부분을 차지 하였고 감염균주도 단일균주이었는데 이는 엄격한 가검물 채취방법의 오류 및 동정방법의 부정확성에 기인하는 것으로 사료된다. 3. 치성 감염시는 cephaloaporin이나 penicillin과의 aminoglycoside 병용요법(66.0%)이 선호되었으나 aminoglycoside 단일요법도 3예 (2.4%) 있었으며 비치성과 원인미상의 감염시도 cephaloaporin이나 penicillin과의 aminoglycoside 병용요법(66.7%)이 주로 시행되었으나 aminoglycoside단일요법도 2예(2.2%) 있었다. 그러나 aminoglycoside만의 단일요법은 명확한 투약근거를 찾을 수 없어 부적절한 항생제요법이라고 사료된다. 4. metronidazole 투여와 연관한 입원일수를 살펴보면 투여군은 38.9%, 비투여군은 50.3%가 6일 이내에 퇴원하여 비투여군에서 오히려 효과가 높았으며 19일 이상의 장기투석도 5예(13.9%)가있었다. 이는 병용 타약제와 투석시기 및 기간을 고려하지 않은 요법이라고 생각된다. 5. 감염시 농배양 방법이 주로 호기성 방법으로 시행되었고 배양결과와는 무관한 항생제요법이 이루어 졌다. 6. 하악대구치는 하악하근막간극, 상악 대구치는 협측근막간극과 가장 높은 연관성을 보였다. 7. 감염 이 빈발한 근막간극은 하악하근막간극 108예, 설하근막간극 48예, 이부하근악간극 47예등의 순이었으며 근막간극별 병발간극수는 설하근막간극이 2.5, 이부하근막간극이 2.2, 하악하근막간극이 1.3개의 순이었으나 이 세간극의 인접간극 병발빈도는 상호 연관성이 높아 간극간의 원발근막간극 여부는 확인하기 어려웠다. 8. 총 213예중 13예의 Ludwig's angina에서 3예가 사망하였다. [영문] This is a retrospective study on patients with acute suppurative infection of the oral & maxillofacial region who were encountered between the period of Mar. 1981 to Mar. 1991. at Yonsei Medical Center. The number of patients involved in this study were 213 in total, induding 108 male patients and 105 female patients. The obtained results were as follows. 1. The most frequent cause of pyogenic infection of this region in decreasing frequency, was odontogenic(57.7%), unknown origin(36.2%) and Non-odontogenic(6.1%). the 77 cases among the 88 cases which were admitted to dept. other than the dept. of oral and maxillofacial surgery without oral examination were designated as unknown orign. 2. Pus cultures were done on 190 cases and streptococci(n= 170, 81.0% ) was the most frequent causative organism isolated from these cultures, followed by staphylococci(n=25, 11.7% ). We concluded that most of the results of pus cultures have single causative organisms but this was due to errors of culture and incubation methods. 3. The combination therapy of cephalosporin or penicillin and amino히ycodie was 66.0% in the group of odontogenic infections and 66.7% in the group of Non-odontogenic and unknown origin infections. The single drug therapy with aminoglycosides was seen in 3 cases(2.4%) in the group of odontogenic infections and 2 cases(2.2%) in the group of non-dontogenic and unknown origin idections. However we could not find the reason why only aminoglycosides were used and there was no apparent reason for this treatment regiment. 4. Amon the patients who were discharged within 6 days, 38.9% were in the group which were given metronidazole and 50.3% were in the group which were given other antibiotics. Also, there were 5 cases(13.9%) of long-term treatment which were given more than 19 days of metronidasole. This result suggests that there was inconsistancy in determining the effective treatment period and time. 5. Almost all pus cultures were taken by aerobic methods and antibiotic administration was not according to results of pub cultures. 6. There was a high correlation between the mandibular molar and the submandibular space, between the maxillary molar and the buccal space. 7. The most common fascial involved were in descending order of frequency the submandibular space(n= 108), the sublingual space(n=48), the submental space(n=47). When the sublingual space, the submental spice, and the submandibular space were involved, they simultaneously involved 2.5, 2.2, and 1.3 adjacent spaces respectively. Furthermore, the above three spaces had a high incidence of interrelationship and therefore it was difficult to identify the primaryly infected space. 8. 벤처기업의 기업가적 지향성과 조직성과간의 관계에 관한 연구

어떤 벤처기업은 성공하는데, 왜 다른 벤처기업은 그렇지 못하는가? 이 질문은 창업론 분야에서 제기되고 있는 핵심적인 질문 중의 하나이다. 그러나 이 질문에 대한 기존 연구들의 답변은 아직까지 제한적이다. 기존 연구들은어떠한 특성을 가진 창업가가 왜 벤처기업을 설립하는가?라는 질문에 주로 초점을 맞추고 있기 때문에, 벤처기업이 설립된 이후에 창업 조직이 어떻게 성장하며 궁극적으로 성공하는 지를 파악하고 있는 연구는 현재 부족한 실정이다. 그러나 이러한 창업 과정을 파악하고 있는 일련의 연구들을 살펴보면, 사업 성공을 위해 혁신적으로 행동하고 위험을 감수하고 적극적으로 행동하려는, 조직수준의 의사결정 패턴인 기업가적 지향성에 초점을 맞추어 논의를 진행하고 있음을 발견할 수 있다. 기업가적 지향성과 관련된 기존 연구들은 조직성과에 정(+)의 영향을 미치는 기업가적 지향성의 주효과와 조직 외부의 산업환경과 내부 자원과의 상호작용이 조직성과에 미치는 영향을 증대시키는 조절효과들을 다각도로 검토하여 왔다. 그러나 이 개념을 처음 제안한 Danny Miller의 최근 지적들을 검토해 보면 (Miller, 2011), 현존하는 기업가적 지향성 연구들에 조직의구조적인 특성에 대한 고려가 제한되어 나타나고 있음을 파악할 수 있으며, 기업가적 지향성이 조직 내부에 존재하는 기업가정신과 어떠한 차이가 존재하는지를 잘 구분하지 못하고 있음을 확인할 수 있다. 이에 본 연구는 한국의 벤처기업을 대상으로, 기업가적 지향성의 정(+)의 성과 효과를 증대시키는 산업환경 특성들의 조절효과 이외에도 벤처기업이 이용가능한 자원의 구조적인 특성들의 조절효과를 중심으로 파악하였다. 다음으로, 창업가 개인의 영향력이 상대적으로 강력한 창업기와 성장기의 기업가적 지향성의 성과 효과를 비교하였으며, 창업가 교체 여부에 따른 기업가적 지향성의 성과 효과 차이를 비교하였다. 구체적으로, 본 연구는 우선, 기업가적 지향성의 개념이 대두되게 된 Miller(1983)의 최초 연구와 기업가적 지향성을 주제어로 한 주요 실증 연구들에 대한 이론적 검토와 한국 벤처기업 담당자들과 인터뷰한 내용을 기반으로 기업가적 지향성과 관련된 기초적인 명제들을 수립하였다. 다음으로, 한국 벤처기업을 대상으로 한국 벤처기업 연구원에서 조사한 벤처기업 정밀실태 2009년 조사 자료를 이용하여, 기업가적 지향성의 수준 결정에 영향을 미치는 산업환경의 특성, 창업가의 특성 및 이용가능한 자원의 구조적인 특성의 효과를 다각도로 검토하였다. 마지막으로, 동일한 자료를 이용하여 기업가적 지향성의 정(+)의 성과 효과에 영향을 미치는 산업환경의 특성뿐 아니라 조직이 이용가능한 자원의 구조적인 특성 및 조직성장 단계와 창업가 교체라는 조직의 구조적인 변화 맥락을 고려하여 조직의 구조적인 상황적 조건들의 조절효과를 다각도로 검토하였다. 1,502개 한국 벤처기업들을 대상으로 실증 분석한 결과, 창업가의 경력 다양성이 기업가적 지향성 수준을 결정할 것이라는 가설과 제휴 다양성이 기업가적 지향성의 수준을 결정할 것이라는 가설이 지지되었다. 또한 기업가적 지향성의 수준이 높을수록 벤처기업의 조직성과가 높을 것이라는 가설과 산업의 동태성이 기업가적 지향성이 조직 성과에 미치는 정(+)의 영향을 조절할 것이라는 가설이 지지되었다. 나아가 기업가적 지향성이 조직성과에 미치는 정(+)의 영향을 제휴 다양성이 조절할 것이라는 가설과 기업가적 지향성이 조직성과에 미치는 정(+)의 영향은 조직성장 단계와 창업가 교체 여부에 따라 달라질 것이라는 가설이 지지되었다. 이와 같은 본 논문의 분석결과는 다음과 같은 세 측면에서 기업가적 지향성에 대한 연구와 창업론 연구분야 전반에 기여할 수 있을 것이다. 첫째, 본 연구는 기업가적 지향성과 관련된 기존 문헌들을 검토하고 기업가적 지향성의 정(+)의 성과 효과를 증대시킬 수 있는 구조적인 측면을 강조하고 이를 실증 분석을 통해 확인함으로써 기업가적 지향성과 조직성과간의 관련성을 보다 심도 있게 파악하고 있다는 점이다. 이는 Miller(1983)에 의해 논의되고, Covin & Slevin(1991)에 의해 채택되고, Lumpkin & Dess(1996)에 의해 심화된 연구 모형을 종합하고 확장하고 있다는 점에서 기업가적 지향성과 관련된 연구 개발에 직접적으로 공헌할 수 있을 것이다. 특히 본 연구는 기업가적 지향성의 성과 효과를 결정하고 조절하는 제휴 포트폴리오의 다양성 효과를 검토함으로써, 벤처기업의 기업가적 지향성의 효과 증대 방안에 관심을 가지고 있는 연구자들과 실무자들에게 시사점을 제공할 수 있을 것이다. 둘째, 본 연구는 기업가적 지향성이 창업가 개인의 특성에 의해 영향을 받지만 조직이 성장하면서 기업가적 지향성의 효과가 더욱 크게 나타나며, 창업가가 교체되는 경우에 성과 효과가 더욱 증대된다는 분석결과를 도출함으로써 기업가적 지향성이 기업가정신과 다른, 조직 수준의 의사결정 패턴임을 확인하였다는 점이다. 창업가가 교체되는 경우 조직에 형성된 의사결정 루틴이 더욱 효과를 발휘할 수 있다는 본 연구의 분석결과는 조직 성장에 관심을 가진 연구자들뿐만 아니라,배태된 행위의 역설에 관심을 가진 신제도론 연구자들에게 이론적인 시사점을 제공할 수 있을 것이다. 셋째, 본 연구는 기존 연구들에서 제안하고 있는 타당한 분석 방법을 사용하고 있다는 점에서 방법론적인 강점을 가지고 있다. 또한 본 연구는 기업가적 지향성을 측정하는 방법을 달리하여 성과 효과를 파악하였고 기업가적 지향성을 구성하는 하부 차원으로 구분해서 개별적인 성과 효과 및 기업가적 지향성의 수준 결정에 대해 논의하고 있다는 점에서 기업가적 지향성의 하위 차원의 효과에 관심을 가지고 있는 연구자들과 효과적인 기업가적 지향성의 함양에 관심을 가진 실무자들에게 시사점을 제공할 수 있을 것이다. 본 연구의 한계와 미래의 연구 방향에 대해서도 기술하였다.Why do some firms succeed, while other firms fail? This question is one of the critical questions in the area of entrepreneurship research. However, the answer for this question is limited. As previous literatures have usually focused attention on who initiates venture start-ups and what are the key characteristics of the founders? we do not know much of the entrepreneurial process after start-ups founded. Thus, researches were needed to explore the process which firms grow and finally succeed. Fortunately, a stream of research exists, which focused on the entrepreneurial process and emphasized the dimension of innovative, proactive, and risk-taking nature of starts-ups. The research area was known as entrepreneurial orientation. Previous empirical researches of entrepreneurial orientation have focused on the main effect of entrepreneurial orientation on firm performance, but, at the same time, emphasized the role of contingent factors, which moderate the positive performance effect of entrepreneurial orientation. These contingent factors range mainly from external factors such as environmental hostility and dynamism, to internal characteristics, such as organizational size and organic structure. However, the recent review of Miller(2011) indicates that endeavors of past 30 years showed some limitations, the structural and embedded characteristics of organization. Thus, I thoroughly reviewed the empirical papers published in the major journals of entrepreneurship, and identified two major gaps in those research area of entrepreneurial orientation; one of which can be found in the article of Miller(1983, 2011), which suggests to identify the effect of configurational characteristics of organizations, and the other suggests not to distinguish the concept of entrepreneurial orientation from entrepreneurship existing in organizations. In order to identify the concept of entrepreneurial orientation from entrepreneurship, I compare the performance effects of entrepreneurial orientation by growth stage of start-ups and founder succession. Also, in order to highlight the structural and embedded characteristics of organization, I explore one positive moderating effects of structure of available resources, such as diversity of human resource function and strategic alliances portfolio, and another positive moderating effects of changing context of organization, such as organizational life cycle and founder succession. Using the survey data of Korean Venture Research Institutes, I analyze the effects of antecedent factors which determine the level of entrepreneurial orientation, and the effects of structural and contextual moderators of organization. The results of 1,502 Korean venture starts-ups showed that the diversity effects of founders experience and strategic alliance portfolio determine the level of entrepreneurial orientation, and the effects of environmental dynamism of industry and the diversity of strategic alliance portfolio moderate the performance effect of entrepreneurial orientation. Moreover, organizational changing context, such as organizational life cycle and succession of founders, found to be another positive performance moderator of entrepreneurial orientation. These results suggest following implications. First, this study highlights the diversity effect of strategic alliance portfolio with entrepreneurial orientation on firm performance, which reflects the structural features of available resources. The results may extend the research frame suggested by Covin & Slevin(1991), and Lumpkin & Dess(1996), in that current study explores and extends the structural effects of organizations. Likewise, the contextual embedding effects of organizational life cycle and founder succession were found to be other moderators. Second, this study identifies the effect of founders experience on entrepreneurial orientation, and the positive moderating effects of the life cycle of organization and the founder succession. These results show that the firm-level concept of entrepreneurial orientation can be determined by the founder characteristics, but the positive performance effects of entrepreneurial orientation can be strengthened after founder succession. These seemingly paradoxical effects of founder can be better explained by the concept of paradox of embedded agency and thus provides theoretical implication for the researchers of new institutional theory. Third, current study identified the diversity effect of alliance portfolio as a moderator on firm performance, and at the same time as an antecedent of entrepreneurial orientation. These results showed the nature of venture starts-ups, which experience the liability of newness, and suggest the effective method to enhance entrepreneurial orientation. The limitation of this research and further research directions are also discussed.Docto
