Significance of whole body scan in bone and joint tuberculosis

골·관절 결핵은 결핵균이 골과 관절을 침범하여 점진적인 파괴를 일으키는 질환으로 거의 대부분의 경우에서 폐나 임파절등 다른 조직이나 장기의 결핵에 속발되어 발생한다.
골관절 결핵의 진단에 흔히 이용되는 평면 방사선 검사에서 초기에는 독특한 소견이 없고, 병이 상당히 진행된 후에야 발견되므로 평면방사선 검사를 통한 병의 조기 진단은 어려운 실정이다.
이에 저자는 정형외과 영역에서 널리 이용되고 있는 보조적 진단 방법의 하나인 전신골주사의 골관절 결핵에서의 진단적 가치를 알고자 1978년 1월부터 1987년 12월까지 골관절 결핵으로 확진되었던 환자중, 수술전 전신골주사를 시행하였던 35명의 환자 40예를 대상으로 하여 다음과 같은 결과를 얻었다.
1. 결핵성 골관절염에서는 급성 화농성 골관절염 보다는 흡수정도가 감소해서 조기진단적 가치는 낮았으나 만성골수염 보다는 증가된 소견을 보였다.
2. 남자보다 여자에서 흡수정도가 증가되었으며, 10세 미만과 60대의 연령층에서 다른 연령층에 비해 흡수정도가 증가된 소견을 보였다.
3. 병의 경과가 길수록 흡수 정도가 감소되었다.
4. 골결핵이 관절결핵보다 흡수정도가 감소되었으며, 골결핵에서는 척추보다 장관골에서 흡수정도가 감소되었다.
5. 14.3 %로 산재성 골결핵을 감지할 수 있었다.

For confirmative diagnosis of bone and joint tuberculosis, it is made by culture method, inoculation to guinea pig, or histologlc examination of the specimen obtained from a lesion site.
But plain reontgenogram is not valuable for early detection of the disease because specific, abnormal finding is not found in early stage and only found in late stage as progressive destruction of bone and joint.
Many reports were made that whole body bone scan, as diagnostic tool, was valuable in other orthopedic disease, but report for its diagnostic value in bone and joint tuberculosis is very rare.
The study was carried out on 35 patients of bone and joint tuberculosis who whole body bone scan was performed before operation from January, 1978 to December, 1987 in Department of Orthopedic Surgery, Yonsei University college of Medicine.
The following results were obtained.
1. Value of early detection of tuberculosis is not much as pyogenic infection.
Intensity of uptake was decreased In tuberculosis than acute pyogenic osteomyelitis and arthritis, but increased than chronic osteomyelitis.
2. Intensity of uptake was decreased in tuberculosis than chronic osteomyelitis.
3. The longer duration of the disease, the more decreased intensity of uptake.
4, Intensity of uptake is increased in joint tuberculosis than bone, and also increased in tuberculosis spine than long bone.
5. Multifocal disease was detected in 14.3% by whole body bone scan.restrictio

Late Recurrence in a Rectal Cancer Patient Who Underwent Preoperative Chemoradiotherapy Followed by Local Excision: A Case Report
Some patients who have undergone preoperative chemoradiotherapy (CRT) following surgery have been diagnosed with late recurrence more than 5 years after treatment, raising questions about the possible benefit extending surveillance beyond the recommended 5 years. In 2011, a 71-year-old male patient was diagnosed with T3N+ low-lying rectal cancer located 3 cm from the anal verge before undergoing long-course preoperative CRT. After CRT, the patient was reexamined and diagnosed with ycT1?2N0 lesion, so local excision (LE) was performed. The patient underwent intensive surveillance for up to 5 years, and no evidence of recurrence was found. At 74 months after surgery, the patient was hospitalized for a hematochezia, and local recurrence at the excision site and peritoneal seeding nodules were identified. Considering the late recurrence in this patient, it might be necessary to long-term follow-up beyond 5 years in patients with preoperative CRT followed by LE

학위논문(박사) - 한국과학기술원 : 물리학과, 2014.2, [ v, 50 p. ]We theoretically study many-body effects on electron transport through three different systems consisting of quantum dots; Kondo cloud in a quantum dot, suppression of the interference in a quantum dot with a 2=3 filling fractional quantum Hall edge, and electron pair tunneling in a double-dot interferometry.
We propose a setup to directly measure the Kondo screening cloud formed by a quantum dot coupled to semi-infinite quantum wires, on which an electrostatic gate voltage is applied at distance L from the dot. We show that the Kondo cloud, and hence the Kondo temperature and the conductance through dot, is affected by the gate voltage, as L increases below the Kondo cloud length. Based on this behavior,
the cloud length can be experimentally identified by changing L with a keyboard type of gate voltages or tuning the coupling strength between dot and the wires.
The fractional quantum Hall system with filling factor 2/3 has an edge state composed of counterpropagating chiral charge and neutral modes. We propose an Aharonov-Bohm(AB) interferometry consisting of a large-size quantum dot with a 2/3 filling fractional quantum Hall edge in order to investigate how the neutral mode spoils the AB interference signals and estimate the velocity of neutral mode. The dot is coupled to two leads via electron tunneling. In the sequential tunneling regime, we
find that the fractionalization results in the dephasing of AB interference. In addition, in the case of the small velocity of neutral mode compared to charge mode, the AB oscillation with the period of one flux quantum is severely suppressed by a topological effect, and hence the AB oscillation with the period of one half flux quantum dominantly appears.
Finally, we propose an Aharonov-Bohm interferometry consisting of two quantum dots for detecting an electron pair tunneling resonance. We find that in the second harmonics of the interference current, pair tunneling processes give a leading nonmonotonous contribution ...한국과학기술원 : 물리학과
