Immunohistochemical study on the distribution of human papillomavirus (HPV)16/18 in oral squamous cell carcinomas, leukoplakias and papillomas

치의학과/석사[한글] 구강은 시진, 촉진 및 조직검사시 신체의 다른 부위보다는 비교적 용이함에도 불구하고 약 60%의 구강암종이 상당히 진행된 상태에서 발견된다. 그러므로, 수술시 광범위한 절제가 불가피하게 되며 5년생존률도 낮아지고 술후의 안모추형으로 사회로의 복귀가 어려워 져 조기진단자 원인규명을 통한 예방이 필요하다. 구강내에서 발생하는 종양의 원인으로 다양한 인자가 알려져 있으나 최근 종양바이러스에 의한 발암기전의 연구가 활발히 이루어지고 있으며, 그 중 Human Papillomavirus (HPV)는 암종을 비롯한 다양한 구강질환의 원인이라는 많은 연구결과가 보고되고 있다. 한편 HPV의 검색방법으로 육안이나 광학현미경을 이용하는 형태학적 방법, 전자현미경을 이용하는 방법, 면역조직화학적 검색법, DNA hybridization 등이 이용되어 왔다. 이에 저자는 24예의 편평세포암종, 7예의 백반증과 6예의 유두종에서 HPV유형 16/18의 분포를 알아보아 HPV에 의한 발암기전의 규명 및 구강암의 조기발견에 도움이 되고자, 기존의 파라핀 포매된 생검조직을, 추적연구가 가능하고 유형특이성이 있는 항HPV유형 16/1 8 E6 단주항체를 이용한 면역조직화학염색을 시행하여 다음과 같은 결과를 얻었다. 1. 편평세포암종 24예중 6예에서 HPV 유형16/18 E6을 검색할 수 있었다. 양성반응을 보인 6예의 편평세포암종중 3예는 암종세포와 인접 상피세포 모두에서 양성염색상을 보였고, 나머지 3예는 인접상피세포에서만 양성염색상을 보였다. 2. 백반증 7예중 4예에서 HPV 유형16/18 E6를 검색할 수 있었으며, 이들은 모두 이형성을 보이지 않았다. 3. 유두종 6예중 4예에서 HPV 유형16/18 E6를 검색할 수 있었다. 이상의 소견을 종합해보면 악성병소인 편평세포암종에서 보다 양성병소인 유두종과전암병소인 백반증에서 훨씬 높은 빈도로 HPV유형16/18이 검색된 점으로 보아 HPV가 구강암의 발암인자로 유추되나 단일요인이기 보다는 화학인자 등과의 복합적 작용에 기인하는 것으로 사료되며, 따라서 구강암의 발생에 있어서 HPV의 명확한 역할이 밝혀지기 위해서는 계속적인 연구가 필요하리라 사료된다. [영문] There have been many reports on the etiological role of HPV in various oral diseases including carcinomas. Present study was performed to elucidate the distribution of HPV 16/18 in oral squamous cell carcinomas, leukoplakias and papillomas of Koreans.24 cases of squamous cell carcinomas,7 cases of leukoplakias and 6 cases of papillomas were reviewed through immunohistochemical staining using anti-HPV 16/18 E6 monoclonal antibody, Obtained results were as fellows : 1. Positive reactions were observed in 6 cases out of 24 squamous cell carcinomas. Among the 6 positive cases 3 cases showed positive reaction broth in the tumor cells and the adjacent epithelial cells, whereas the remaining 3 cases only in the adjacent epithelial cells. 2. 4 cases out of 7 leukoplakias reacted positively, all of which showed no dysplasia. 3. Positive stainings were observed in 4 cases out of 6 papillomas. These results confirm HPV involvement in oral papilloma, leukoplakia and squamous cell carcinoma, and support the hypothesis of HPV as an etiological agent in oral carcinogenesis. However, the frequency of positive reaction was much higher in benign lesions such as papillomas and leukoplakias, than in malignant lesions like squamous cell carcinomas. Therefore, HPV may be thought to be a co-factor in oral carcinogenesis together with chemical factor, rather than a single factor.
