Prediction of Prognosis in Patients Mechanically Ventilated due to Acute Respiratory Failure Hyung Gil Kim Department of Medical Science The Graduate School, Yonsei University (Directed by Professor Sung Kyu Kim) Acute respiratory failure is a condition in which an illness or impairment of the respiratory system results in inadequate oxygenation or ventilation, or both. It occurs in all age-groups. The causes for acute respiratory failure are various, and even though patients are managed intensively including mechanical ventilation, the mortality rate has been 20-60% even until today. Therefore, early prediction of outcome in such critically ill patients should be considered a high priority so that corrective measures or alternative support methods may be rapidly instituted. The patients who were treated with a mechanical ventilator longer than 7 days due to acute respiratory failure in intensive care unit were listed, and only patients whose initial PaO^^2 was less than 50 mmHg during breathing the ambient air in the absence of an intracardiac shunt were included. From the daily collected data, the following variables were calculated: buffer base deviation (measured predicted), minute ventilation(tidal volumeร—respiratory rate), dynamic complance [tidal volume/(peak pressure-PEEP)], static compliance [tidal volume/(plateau pressure-PEEP)], pulmonary insufficiency index, indices for physiologic shunt or gas wxchange such as PaO^^2 /FiO^^2 , PaO^^2 /PAO^^2 , D(A-a)O^^2 /FiO^^2 and PAO^^2 /FiO^^2 . These variables between the survivors and nonsurvivors were compared daily, The following result were obtained: 1) There was no significant differences in age and ICU care was longer in the survivors than the nonsurvivors. 2) There was no significant difference in buffer base deviation and minute ventilation between the two groups. 3) There was no significant difference in dynamic compliance between the two groups. 4) Static compliance showed higher values in the survivors and from the 4th day after mechanical ventilation, there was a significant difference between the two groups. 5) Pulmonary insufficiency index, the ratio of D(A-a)O^^2 /FiO^^2 and PAO^^2 /FiO^^2 showed significantly higher values in the nonsurvivors throughout the first seven days. There was a tendency of gradual decrease in the survivors but almost no change in the nonsurvivors. 6) The ratio of PaO^^2 /FiO^^2 and PaO^^2 /PAO^^2 showed significantly higher values in the survivors throughout the first seven days. There was a tendency of gradual increase in the survivors. but almost no change in the nonsurvivors. Static compliance, pulmonary insufficiency index, D(A-a)O^^2 /FiO^^2 , PAO^^2 /FiO^^2 ,PaO^^2 /FiO^^2 and PaO^^2 /PAO^^2 showed a significant difference between the survivors and nonsurvivors, and each value also showed characteristic changing patterns throughout the first 7 days after mechanical ventilation. Much has been written concerning these fractures during last thirty years or more. In this series, 92 cases of the patella fractures were reviewed, which were treated at Yonsei University Severance Hospital from 1964 to 1979. Most or the fractures occurred in males and the mean age was 38 years. Traffic accident was the most common cause and most fractures were comminuted or transverse type. Of the 92 cases, 32 cases were treated by conservative means and 29 cases had open reduetion and internal fixation(of these, circumferential wire loop fixation: 15 cases, Magnuson wire loop fixation: 3 cases, tension band wiring: 11 cases). 9 cases had total excision of the patella and 22 cases had partial excision. Average follow-up period was 26.2 weeks and average flexion range of motion of the knee at the end of the period was 118.7 degree. The end results of tension band wiring method was much better than those of other methods.
