    A Study on the characteristics of Chinese medical care consumers and choice of medical care providers

    Purposes: This study identifies local Chinese consumers' standard for selecting medical care provider and their standard for choosing medical staff, as well as their expectations and concerns regarding Korean medical care providers. Methodology: A survey was conducted in China, to identify Chinese medical care consumers' standards for selecting a medical provider and the factors that influence their use of general hospitals. A total of 1,500 people across three cities, between the ages 18 and 60 participated in the survey. Moreover, a multiple logistic regression analysis was used to analyze the factors that affect Chinese medical care consumers' use of general hospitals. Findings: A total of 75.5 percent respondents chose general hospitals as their most frequently-used medical provider. Those who have health insurance, visit general hospitals as outpatients or are hospitalized more frequently than those who do not have a health insurance. Furthermore, those who have private insurance visit general hospitals as outpatients or are hospitalized more frequently than those who are not signed up for private insurance. Major standards for selecting a hospital included: the doctor's skills, word-of-mouth regarding the hospital, and distance to the hospital from the respondents' home. Standards for choosing medical personnel included word-of-mouth regarding the medical team, recommendations from family members or acquaintances, and medical team's notoriety. Friends and neighbors, family members, television and other media outlets were the channels for acquiring information on a hospital. It was found that Chinese people mostly visit the cardiovascular department of Korean hospitals for treatment. For using Korean hospitals in China, the majority of respondents answered that they were concerned about the cost. Practical Implications: Backed by highly skilled medical experts and cutting-edge technology, Korean medical care providers are attempting to enter China's medical care market.     Analysis of variations in anesthesia cost according to severity of physical status and cancer stage in colorectal cancer patients

    Background: Since the late 2000s, the number of anesthesia performed has increased yearly. However, there has not been research into the appropriate cost of anesthesia based on the difficulty of anesthesia performed, the number of medical personnel participated, and materials administered for anesthesia. The purpose of this study was to analyze the variations in anesthesia cost according to severity of their physical status and cancer stage in patients who had undertaken colorectal cancer surgeries. Methods: In order to analyze the cost of anesthesia for colorectal cancer surgery, we used Electronic Data Interchange data from 2011 to 2012 of the three superior general hospitals in Seoul. Colorectal cancer codes were limited to seven codes included in the cancer screening statistics of the Health Insurance Review and Assessment Service. Based on these data, a frequency analysis and a multiple linear regression analysis were performed. Results: There was no variation in the cost of anesthesia according to gender and age. However, the ASA physical status (PS) class and the cancer stage variables were confirmed to modify the anesthesia cost. Conclusions: These study imply that there is a difference in the technology used for anesthesia by medical personnel as well as the anesthesia related materials used according to the ASA PS and the cancer stage among patients having the same disease.ope

    Dept. of Public Health/박사Background: Changes in economic activity status from employed to unemployed or becoming economically inactive can increase the risks of stress and disease, affecting the morbidity and mortality associated with disease. The aim of this study was to analyze impacts that changes in economic activity status and regional socioeconomic characteristics from 2003 to 2008 had on health and death. It also examined the effects of occupational change on mortality rate. Methods: The data from the 6th wave (2003) to the 11th wave (2008) of the Korean Labor and Income Panel Study (KLIPS), which surveyed 19,305 people, were analyzed to identify the relationship between regional socioeconomic characteristics, changes in economic activity status and health. The impacts that changes in occupation had on the mortality rate were examined by analyzing subjects who had stayed employed from the 6th wave to the 11th wave of the KLIPS. Economic activity status was first classified into employed and unemployed / economically inactive and used to study the correlation with mortality rate. In the process, demographic and socioeconomic variables such as gender, age, education level, household income, marital status, and self-rated health status were adjusted for. Generalized estimating equations (GEE) were employed to determine the correlation between changes in economic activity status and death which were repeatedly measured.Results: Those who became unemployed or economically inactive showed higher mortality rate than those who stayed employed (OR 4.95 [95% CI 3.26-7.52]). In the case of death by disease, those who became unemployed or economically inactive showed higher mortality rate than those who stayed employed (OR 4.95 [95% CI 3.26-7.52]). In the case of death by other causes than disease, those who became unemployed or economically inactive also had higher mortality rate than those who stayed employed (OR 4.94 [95% CI 3.26-7.51]). As for the correlation between the change in occupation and death, engineers, service providers, salespeople, and technicians had higher but statistically insignificant mortality rate than that of high-ranking employees, managers and professionals. Conclusions: A shift to unemployment or an economically inactive status lead to higher mortality rate compared to staying employed. The change from employed status to unemployed/economically inactive status is seen as hardships experienced in personal lives, such as loss of job, decline in social position, restricted activity and decreased quality of life.     본 발명은 중형기공성 실리카 물질의 제조방법, 제조된 중형기공성 실리카 물질 및 이 물질로된 판상, 미세 섬유 및 박막에 관한 것이다. 본 발명의 중형기공성 실리카 물질을 제조하기 위하여 구조 유도체로는 다양한 종류의 계면활성제, 실리카 원은 테트라에틸오르토실리케이트를 사용하고 여러 가지 유기용매를 선택하여 단일 또는 혼합 용매 상에서 제조 과정을 처리하는데 그 제조 과정중에 게면 활성제의 교질(micelle) 입자와 실리케이트 사이의 약한 상호작용을 이용한다. 본 발명에서 제조된 중형기공성 판상형 실리카 물질은 투명한 특성을 보이며 제조 조건을 조절함으로써 기공이 규칙적으로 배열되거나 또는 불규칙적으로 배열된 물질을 제조할 수 있는데 기공의 배열이 균일한 판상형 실리카 물질은 기공이 불규칙하게 연결된 경우와는 다르게 특이한 편광 현상을 보였고 전자주게와 전자받게를 갖고 이중결합 등으로 결합된 분자를 흡착시켰을 때 비선형 광학 특성을 보였다
