Plasma somatomedin-C levels in normal childhood

의학과/석사[한글]성장 호르몬은 신체의 성장 촉진에 중요한 역할을 담당하며 이런 성장호르몬의 많은 생리적 작용은 혈장내의 Somatomedin에 의해 조절된다. 현재까지 다양한 정제과정을 거쳐 Sm-A, Sm-C(IGF-Ⅰ), IGF-Ⅱ등이 추출되었다.Sm-C는 사람의 proinsulin과 비슷한 구조를 가진 peptide 호르몬이며, 성장호르몬에 의존적이고, 성장 호르몬의 영향으로 간에서 합성이 된다. 성장호르몬은 pulsatile fashion으로 분비되고, 낮동안 농도 변화가 심해 성장호르몬 결핍성 왜소증을 진단하기 위해서는 24시간 혈풍 성장 호르몬의 농도를 측정하거나, clonidine, L-dopa, insulin등을 사용하여 20-30분 간격으로 여러 번 채혈을 해야하는 어려움이 있다. 그러나 Sm-C는 성장 호르몬에 의존적이고 반감기가 16시간으로, 대부분 혈장단백과 결합되어 천천히 혈장으로부터 제거되기 때문에 혈중 농도가 언제나 일정하므로, 한번 채혈로 농도 측정이 가능하여, 왜소증의 진단에 도움이 되고 있다. 또한 성장 호르몬 투여 후 그 치료의 반응 정도를 보는 지표로도 사용될 수 있다. 그러나 현재한국인 아동을 대상으로 한 Sm-C의 정상치의 통계가 없는 실정이므로, 정상아동 및 사춘기 연령을 대상으로 하여 Sm-C 농도를 측정하여 왜소증의 진단 및 치료에 도움이 되는 표준치를 만들고저 본 연구를 시행하였다. 연령이 7-20세 사이의 신장과 체중이 정상 범위에 있는 건강하고 만성질환이 없는 남녀 186명을 대상으로 하여 7-9새, 10-12세, 13-15세, 16세 이상의 4군으로 나누었으며, 신장과 체중 및 Tanner기를 측정하였다. 모든 대상은 공복에 오전 10시경에 채혈하여 혈장을 분리한 뒤 방사면역측정법에 의해 Sm-C 농도를 측정하였다. 본 실험의 결과는 다음과 같다. 1) 혈중 Sm-C 농도는 남녀 모두 연형이 증가함에 따라 증가하기 시작하여13-15세 연령군에서 최고간을 보이며, 16세 이상군을 제외하고는 전 연령군에서 여자가 남자 보다 의의있게 높았다. 2) 혈중 Sm-C 농도는 여자는 13세 전후, 남자는 14세 전후에서 최고값을 보여 여자에서 1년 정도 빠르게 최고간을 나타내었다. 모든 연혁에서 여자가 남자보다 Sm-C 농도가 높았으며, 특히 9세, 12세, 13세에서 의의있게 높았다. 3) Tanner기에 따른 혈중 Sm-C 농도는 Tanner기가 증가하면서 증가하여 남녀 모두 Tanner 4기에서 최고값을 보인 후 점차 감소하였으며, 전 Tanner기에서 여자가 남자보다 높았다. 이상의 결과로 보아 혈중 Sm-C 농도는 남녀 모두 연령이 증가함에 따라 증가하기 시작하여 급격한 성장이 이루어지는 사춘기 시기에 최고값을 보인 후 감소함을 알 수 있으며, 이 정상치의 통계를 바탕으로 성장장애나 왜소중의 진단 및 향후 치료효과 판정에 도움을 줄 수 있으리라 생각된다. [영문]Somatomedin-C, the insuline-like growth factor I is suggested to mediate the growth-promoting effect of growth hormone, Sm-C, mostly bound to plasma protein forming an IGF-I protein complex, is slowly cleared from plasma and thus its plasma concentration is quite constant. A single sampling of blood for Sm-C is sufficient for the measurement of its concentration. For the evaluation of growth hormone deficient and non-deficient dwarfs, measurements of plasma Sm-C may bea useful adjunct to assessment of growth hormone secretion. To determine the 느-C reference ranges of the age- and sex-matched norms in Korea, plasma level of Sm-C/IGF-I were measured by specific radioimmunoassay in 186 childrens and adolescents between the ages of 7 and 20 yr. All subjects were in good health and between the 3rd and 97th percentiles for height using a standard growth chart. Pubertal stages were determind by the method of Tanner. The results were as follows; 1. The mean levels of plasma Sm-C gradually increased over the consecutive age groups and reached maximum at 13-l5yr of age in both males and females. 2. The mean levels of plasma Sm-C in females were significantly higher than those in males at all age groups except over 16 year of age. 3. The mean levels of plasma Sm-C reached maximum at 13yr in females and 14yr in males. 4, According to Tanner stage, the mean levels of plasma Sm-C reached maximum at Tanner stage 4 in both males and females. 5. The mean levels of plasma Sm-C in females were higher than those in males at al1 Tanner stage. In conclusion, our studies indicate that plasma Sm-C levels in normal children can be used as a reference value for the diagnosis of growth hormone deficiency and the response after growth hormone therapy.restrictio
