라틴아메리카 중산층 정치와 경제

이 글은 라틴아메리카 중산층과 협치(Plan Nacional de ItD, CSO 2009-09233) 연구 프로젝트의 일부분이며 베드로교회재단, CISC, 오르테가 이 가셑 대학연구소가 조직한 세미나(2009년 11월, 마드리드)에서 발표한 글이다.국가간에 큰 차이가 존재하지만 현재 라틴아메리카에서는 경제성장과 소득이전 정책에 힘입어 중산층이 증가하고 있는 추세이다. 그리고 이는 중하 계층의 팽창 같은 새로운 현상들을 불러 일으켰다. 본고에서는 중산층의 정치적 행동과 기대, 그리고 그들이 거부하는 정부의 정책에 대한 부분들을 살펴보고자 한다. 그리고 사회 통합과 경제성장이라는 균형 잡힌 정책이 실행하기 위해서는 대표성의 측면에서 어려움이 있음에도 불구하고 중산층과 사회 다수계층간의 연계 정치로 나아가야 한다는 것을 결론으로 제시한다

Effects of home care services on discomfort in patients with cancer and on caregiver burden.

지역사회간호학과/석사[한글] 우리 나라에서 암질환은 사인 1위이며, 지속적으로 증가하는 추세이고, 가정간호 대상자 중에서도 약 30%를 차지하는 1순위 질환이다. 항암화학요법은 암환자 치료의 중요한 방법으로 널리 이용되나 그 치료에 따른 여러 가지 신체적, 정신적 부작용으로 인해 치료를 중단하거나 연기하는 경우가 발생하며, 치료 시작 단계에는 질병에 대한 지식과 정보 부족으로 불안감이 증폭되고 질병과 죽음에 대한 부정을 동시에 경험하게 된다. 그러므로 가정에서 암환자와 가족들이 질환 및 항암화학요법과 관련한 충분한 정보와 대처방안을 배울수 있는 기회를 제공받아 자가 간호 능력과 가족의 돌봄을 지지하고 증진시킬 수 있는 간호중재가 필요하다. 이에 본 연구는 2차 또는 3차 항암화학요법 치료를 받은 경험이 있는 암 환자와 가족을 대상으로 가정간호 제공이 암환자의 불편감과 주 간호자의 가족부담감 변화 정도에 미치는 영향과 간호 만족도를파악하였고, 실험군 32명, 대조군 35명을 대상으로 대조군 전후 설계의 유사실험연구이며, 자료는 2002년 9월 12일부터 2002년 11월 2일까지 수집하였다. 본 연구의 결과는 다음과 같다. 첫째, 가정간호 이용 양상을 조사한 결과, 가정간호 의뢰 후 첫 방문은1.88일째였고, 가정간호 이용 목적은 입원 후 회복과 건강상태 유지가 주를 이루었다. 가정간호제공 기간은 평균 14.6일이었으며, 1회 방문 당 가정간호 제공시간은 평균 35.78분이었고, 평균 4회의 가정간호가 제공되었다. 둘째, 제공된 가정간호 서비스 내용은 기본간호, 교육 및 상담, 치료적 간호 영역으로 총 47개 항목이며, 1회 방문 당 평균 19개 항목의 가정간호 서비스가 제공되었고 치료적 간호가 7개 항목으로 가장 많았다. 셋째, 암환자의 불편감은 실험군에서는 가정간호 제공 전 2.99점에서 가정간호 제공 후 2.10점으로 유의하게 감소한 반면((t=11.34, p=.000), 대조군에서는 사전, 사후 검사 결과 통계적으로 유의한 차이를 보이지 않았다(t= -5.966, p=.080). 넷째, 암환자 주 간호자의 가족부담감은 실험군에서는 가정간호 제공 전 3.04점에서 가정간호 제공 후 2.41점으로 유의하게 감소한 반면((t=.916, p=.000), 대조군에서는 사전, 사후 검사 결과 통계적으로 유의한 차이를 보이지 않았다(t= -3.67, p=.069). 다섯째, 실험군의 가정간호 만족도를 조사한 결과, 4점 만점에 평균 3.88점으로 매우 높았으며, 4개의 하위 영역인 교육 요구에 대한 가정간호사의 지도, 가정간호사의 지식, 기술, 태도, 대인관계 및 정서적지지, 접근용이성 모두에서 높은 만족도를 보였다. / 결론적으로 2차 또는 3차 항암화학요법을 받은 암환자는 가정간호를 제공받음으로써 불편감이 완화되었고, 주 간호자의 가족부담감이 감소되었으며, 가정간호 제공에 대한 간호 만족도가 높게 나타나, 기본간호, 교육 및 상담과 치료적 간호를 포함한 가정간호 제공이 암환자와 가족에게 효과적인 간호중재로 긍정적 영향을 주었음을 입증하였다. [영문] The incidence of cancer has gradually increased so that now cancer is a leading cause of death in Korea. A high proportion of all home care service recipients are patients with cancer (about 30%). Chemotherapy is an important method for cancer treatment, but there are various adverse effects, both physical and mental. These cause patients to give up or put off treatment. In the initial period of treatment, patients who do not have sufficient knowledge and information about their disease usually suffer from both the disease and the denial of seemingly imminent death. In this context, it is necessary to provide them with a nursing intervention that offers an opportunity to obtain sufficient information and to develop coping strategies for both the disease and chemotherapy. Adequate information and good coping strategies enhance the self-care capacity of the patients and promote family care. In the present study the researcher considered the effects of providing home care service to alleviate the discomfort experienced by patients with cancer and the burden faced by family caregivers who provide most of the care. The researcher also assessed satisfaction with nursing care by both patients and their families. This is a pre- and post-design, quasi-experimental study with a control group. The participants were patients who had completed secondary or tertiary chemotherapy sessions and the families of these patients. Data collection was done from September 12th to November 2nd, 2002. The result are as follows: 1. Characteristics in the use of home care: After applying for home care service, patients were visiting an average of 1.88 days later. The major purpose of using home care service was to help recovery after hospitalization or to maintain present health. The average period for home care service was 14.6 days, with 4 visits over this period, for 35.78 minutes per visit. 2. Content of home care service: A total of 47 items of service were provided - basic nursing care, education and counseling, and therapeutic nursing care. On every visit, an average of 19 items of home care service were provided, and the majority (7 items) were therapeutic nursing care. 3. Discomfort in patients with cancer: Discomfort decreased significantly in the experimental group, from 2.99 to 2.10, after home care (t=11.34, p=.000), but no significant difference was found between pre- and post-test scores in the control group (t=-5.966, p=.080). 4. Caregiver burden in the major care provider: Family strain decreased significantly, from 3.04 to 2.41, after home care (t=.916, p=.000), but no significant difference was found between the pre- and post-test scores in the control group (t=-3.67, p=.069)./ 5. Satisfaction with home care services: Satisfaction was very high, an average of 3.88 on a scale of 4 points. Both patients and families expressed high satisfaction with all sub-domains of nursing care: guidance by the home care nurses, knowledge, skill, attitude, interpersonal relationships, emotional support, and accessibility. In conclusion, patients with cancer going through secondary or tertiary chemotherapy showed a decrease in discomfort and caregivers experienced less burden. Their satisfaction with home care services was high showing that provision of this service - including basic nursing care, education, counseling, and therapeutic nursing care - has a positive influence on patients with cancer and their families and is an effective nursing intervention.ope

더블 다이아몬드모델을 통한 제조산업경쟁력의 결정요인 연구

학위논문 (석사)-- 서울대학교 국제대학원 : 국제학과(국제지역학전공), 2012. 8. 김종섭.18세기 후반부터 제조산업은 성장의 주요동력으로 여겨져 왔다. 특히 최근의 금융위기 이후, 제조산업의 중요성이 다시 강조되고 있다. 그간 산업경쟁력의 결정요인에 대한 많은 연구가 행해졌지만 주로 국내 요소의 결정요인수준에서 연구들이 많이 이루어진 반면 자국의 산업경쟁력에 영향을 미치는 주변국가의 요소에 대해서는 많은 연구가 이루어지지 않고 있다. 이에 본 논문에서는 수정된 더블 다이아몬드 모델을 사용하여 자국 제조산업의 국제경쟁력의 결정요인으로서 국내결정요인과 함께 주변국가의 요소를 국외 결정요인으로 분석하였다. 한편 분석을 위해 제조업 중 하이테크, ICT, 자동차산업을 연구대상으로 범위를 한정하였다. 연구결과에 따르면 각 산업마다 경쟁력에 영향을 미치는 결정요인 중 특히 주변국가들이 미치는 영향이 매우 상이한 것으로 나타났다. 하이테크산업은 주변국가의 영향 중 수요조건의 영향을 가장 많이 받는 것으로 나타났으며 ICT 산업은 수요조건, 기업전략, 구조 및 경쟁의 영향을 많이 받는 것으로 나타났다. 한편 자동차 산업의 경우 다른 산업들에 비해 주변국의 영향을 가장 많이 받는 것으로 나타났으며, 주변국의 조건 중 요소조건, 관련 및 지원산업, 수요조건의 영향을 크게 받고 있는 것으로 나타났다. 따라서 본 연구는 제조업의 경쟁력 강화를 위해서는 세부산업별로 나누어 접근할 필요가 있으며 주변국가들의 영향도 다루어야 한다는 점을 시사하고 있다. 기존 연구들과 비교했을 때 본 연구는 그 동안 많이 다루어지지 않았던 주변국가들의 영향을 다루었다는 점에서 가치가 있으며, 본 연구에서 사용된 분석틀로 향후 다른 제조산업의 경쟁력 및 특정 지역의 제조산업경쟁력의 결정요인분석으로 연구를 확대해 볼 수 있을 것이다.The manufacturing industry has been considered as the main engine of growth since the late 18th century. After the recent financial crisis, the importance of manufacturing industries has resurged. To find out the determinants of global competitiveness of manufacturing industry, many attempts have been made by the scholars and economists. However, not many studies have accounted for the impact of changing conditions in neighboring countries in addition to the more conventional home countrys domestic conditions. This paper brings together the ideas of domestic competitiveness and global competitiveness of the manufacturing industry focusing on high-tech, ICT and automotive sectors. To find out the determinants, the modified double diamond model is used as a key tool to carry on the panel regression analysis. According to the results, each industrys determinants of competitiveness appear to be different. In particular, the impact from neighboring countries turned out to be very distinct. The high-tech industry is affected by neighboring countries GDP while the ICT industry is affected by neighboring countries industrial competitiveness, GDP and business start-up cost. The case of the automotive shows a strong correlation with neighboring countries conditions due to being affected by neighboring countries industrial competitiveness, GDP, GDP per capita, tertiary education and internet users. The result implies that the determinants of competitiveness are different from industry to industry. Thus, strategies to enhance competitiveness should be respectively designed considering neighboring countries conditions and carried out for each industry. Compared to the previous studies, this study is worthy of investigation since it tries to identify what kind of impact neighboring countries have on the competitiveness of home countrys industries. While this study limits the scope to arguably the three most important manufacturing industries – High Tech, ICT and Automotive, the same method could be applied to investigate determinants of competitiveness of other manufacturing industries. Moreover, this studys unique feature of accounting for the impact from other countries could be further developed to identify industrial characteristics of each region.Table of Contents I.
