    Clinicopathological Characteristics of Well-differentiated Papillary Mesothelioma of The Peritoneum: A Single-institutional Experience of 12 Cases

    Background/Aim: Well-differentiated papillary mesothelioma (WDPM) is histologically characterized by papillary architecture with fibrovascular cores, lined by bland mesothelial cells. We recently experienced a case of WDPM associated with multiple peritoneal inclusion cysts, which prompted us to initiate a comprehensive review of previously diagnosed WDPM cases. Materials and Methods: The clinicopathological characteristics and immunophenotype of 12 cases of peritoneal WDPM were investigated using a review of electronic medical records, pathological examination, and immunostaining. Results: The patients' ages ranged from 23 to 75 years. No patient had endometriosis or a previous history of asbestos exposure. Ten tumors were detected incidentally during surgery for other causes. Most tumors appeared as a small, single nodule on the peritoneal surface, but in three cases, WDPM presented as multiple lesions. All but one patient had no symptoms. All the patients examined are still well without postoperative recurrence. Histologically, all cases demonstrated typical papillary architecture with fibrovascular cores. The mesothelial cells lining the papillae consisted mostly of single row of cells, although areas of proliferation to multiple layers were observed in a few cases. Their nuclei appeared bland, but two cases exhibited mild nuclear atypia and prominent nucleoli. Immunostaining revealed that the mesothelial cells were positive for D2-40, cytokeratin 5/6, cytokeratin 7, and Wilms' tumor 1. Conclusion: We herein demonstrated the clinicopathological characteristics of peritoneal WDPMs. WDPM has distinct pathological features. Although all cases we examined were uneventful after surgery, further surveillance is recommended since the biological behavior of WDPM is still uncertain.ope

    Renal intravascular large B cell lymphoma: the first case report in Korea and a review of the literature

    Herein, we describe the first case of renal intravascular large B cell lymphoma in Korea occurring in a 66-year-old female. She presented with mild fever and dyspnea. On physical and laboratory evaluations, hemophagocytic lymphohistiocytosis was suspected, but the bone marrow biopsy results were unremarkable. During the work-up, massive proteinuria developed, which led to a renal biopsy. The renal architecture was relatively well-preserved, but the glomeruli were hypercellular with the infiltration of atypical, large lymphoid cells with increased nucleus-cytoplasm ratio and clumped chromatin. Similar cells were also present in the peritubular capillaries. The tumor cells exhibited membranous staining for CD20 and CD79a. After the diagnosis of intravascular large B cell lymphoma, the patient received rituximab-based chemotherapy under close follow-up.ope

    Primary effusion lymphoma (PEL) is a B-cell non-Hodgkin's lymphoma that is usually characterized by lymphomatous effusions in the body cavity without any detectable tumor masses. According to the World Health Organization (WHO) schema for tumor classification, PEL is defined by the presence of human herpesvirus 8 (HHV8) in malignant lymphoid cells. However, a subset of effusion-based B-cell lymphoma is not HHV8-positive and exhibits different clinicopathological characteristics. The 2017 WHO Classification of Tumors of Hematopoietic and Lymphoid Tissues does not list HHV8-negative effusion-based lymphoma, which remains an underappreciated B-cell lymphoma, as an individual entity. The present study reports a case of this rare type of lymphoma with indolent clinical behavior in a 75-year-old male patient receiving only symptomatic treatment.     Background: The purpose of this study was to investigate the relationship between biceps medial subluxation/dislocation on the magnetic resonance arthrography (MRA) imaging and subscapularis full-thickness tear confirmed arthroscopically. We hypothesized that presence of a biceps medial subluxation or dislocation would strongly indicate a subscapularis full-thickness tear. Methods: A total of 432 consecutive patients who underwent arthroscopic repair for rotator cuff tears with/without subscapularis tears at our institute were retrospectively reviewed. The inclusion criterion of this study was preoperative MRA images taken within 6 months of arthroscopic repair. The presence of medial subluxation/dislocation was evaluated on the preoperative MRA images, and subscapularis tear was confirmed on arthroscopic examination. Results: Biceps subluxation/dislocation was identified in 46 of the 432 patients on MRA. Forty-five of these 46 patients also had a subscapularis full-thickness tear identified in arthroscopic examination. Among the 386 patients who did not have biceps subluxation or dislocation, 54 patients had a subscapularis full-thickness tear diagnosed arthroscopically. The presence of a biceps subluxation/dislocation could predict a subscapularis full-thickness tear with sensitivity of 45% (45/99), specificity of 99% (332/333), positive predictive value of 98% (45/46), negative predictive value of 86% (332/386), and accuracy of 87% (377(45 +332)/432). Conclusion: Medial subluxation/dislocation of the biceps on MRA images was highly associated with a concurrent subscapularis full-thickness tear which was confirmed arthroscopically. This association had 99% specificity and 98% positive predictive value. Therefore, if a biceps subluxation/dislocation is identified on MRA images, there is a high chance that a concurrent subscapularis full-thickness tear exists.ope

In this study, a Monte Carlo simulation model was developed for the department whose cancellation rate was the highest in the hospital in order to determine the optimum number of beds with respect to reducing the cancellation rate as the well as the empty by rate. Before the simulation run, the model was verified by the Kolmogorov-Smirnov test. The fellowing results were generated by four alternative models according to the different bed occupancy policies. 1. When long-term patients were admitted to the wards beloning to the department(wards 52 and 77) and when 1/3 of these patients, whose conditions were initially too poor for surgery, were assumed to be improved so that they could undergo surgery, the optimum number of beds and the cancellation rate were 15 beds and 26.5%, respectively. 2. When the situation was same as in model 1 except for the fact that no patients, conditions were improved in time for surgery, the optimum number of beds the and cancellation rate were 14 beds and 29.0%. 3. When the situation nab same as in model 1 except for the fact that long-term patients were net admitted to these two wards, the optimum number of beds and the cancellation rate were 15 beds and 26.3%. 4. When the situation was same as in model 2 except for the fact that long-term patients were not admitted to these two wards, the optimum number of beds and the cancellation rate were 14 beds and 28.8%. While the above bed policy can reduce the cancellation rate by as much as 26.3%, there is a need for a more comprehensive policy to further reduce the rate.     Malignant mesothelioma is a highly lethal cancer. V-set immunoregulatory receptor (VSIR, also known as V-domain Ig suppressor T-cell activation, VISTA), a negative immune checkpoint regulator, was reported to be expressed in malignant mesothelioma; however, its detailed expression pattern and clinicopathological significance have not been elucidated. We examined the expression of VSIR and CD274 and CD8+ tumor-infiltrating lymphocytes in a total of 124 samples from 66 patients with malignant mesothelioma and analyzed the clinicopathological characteristics and their relationship with the immunohistochemical findings. A total of 553 non-small cell lung carcinomas were also evaluated for VSIR expression. VSIR expression was higher in epithelioid type mesothelioma (pโ€‰<โ€‰0.001), whereas CD274 expression was higher in sarcomatoid type (pโ€‰<โ€‰0.001). CD8+ tumor-infiltrating lymphocytes were more abundant in sarcomatoid mesotheliomas (pโ€‰<โ€‰0.001), VSIR-low tumors (pโ€‰=โ€‰0.045), and CD274-high tumors (pโ€‰<โ€‰0.001). VSIR and CD274 were differentially expressed in each histological component of the biphasic type. VSIR expression was associated with favorable survival (pโ€‰=โ€‰0.008). VSIR expression may be used as an immunohistochemical diagnostic marker for epithelioid mesothelioma.
