Single-photon emission computed tomography in a patient with ictal metamorphopsia

Metamorphopsia is a type of visual illusion, which has been reported as a rare ictal manifestation. The patient presented with a simple partial status epilepticus characterised by continuous facial metamorphopsia, intermittently accompanied by elementary visual hallucinations or other types of visual illusions. Subtraction single-photon emission computed tomography images showed an increased perfusion in the ventrolateral aspect of the right temporo-occipital junction (middle and inferior occipital, and inferior temporal gyri). The result suggests that the anatomical substrate involved in the generation of ictal facial metamorphopsia is located in the visual association areas at the right temporo-occipital junction.ope

학위논문 (석사)-- 서울대학교 대학원 : 지리학과, 2015. 2. 손정렬.최근 보육에 대한 관심이 많아짐과 동시에 국공립어린이집 확충 요구가 증대하여 보육시설의 확충이 중요한 정책적 목표가 되었다. 국공립어린이집이 공적 영역에서 제공되고 일정 부분 공공재의 성격을 갖는다는 점에서 형평한 배분을 고려해야 한다. 그러나 기존의 연구들은 서비스 배분 상태에 대한 고찰 없이 입지적정성이나 서비스 권역 설정과 같은 문제에 치중하였기 때문에 공공서비스의 공간적 형평성을 평가하는 분석 틀의 제시와 평가가 필요하다고 판단하였다. 따라서 본 연구는 공공보육서비스가 공간적으로 형평한지 평가하고 향후 보다 형평한 공공보육서비스의 배분을 위한 우선 설치지역을 탐색하여 제안하는 것을 목적으로 하였다. 이를 위해 서울특별시의 2009년과 2014년의 국공립어린이집을 대상으로 분석 및 평가를 진행하였다. 본 연구의 결과는 다음과 같다. 첫째, 격자에 기반을 둔 공간 단위에서 포텐셜 모형으로 측정한 보육 기회의 분포는 불균등하였다. 집중 곡선을 이용해 이러한 불균등이 형평한지 비형평한지 평가하였고, 대시메트릭 매핑을 이용하여 추정한 영유아 인구의 분포와 비교했을 때, 보육 기회의 분포가 형평하다고 평가하였다. 그러나 소득을 의미하는 주택실거래가의 분포와 비교했을 때 보육 기회의 분포는 비형평하였다. 그럼에도 불구하고 두 기준을 바탕으로 보육 기회의 분포를 살펴보았을 때, 모두 2009년보다 2014년에 좀 더 형평한 방향으로 변화하였다. 따라서 서울시의 국공립어린이집 설치가 적절한 방향으로 이뤄지고 있다고 평가하였다. 둘째, 2009년과 2014년 사이 국공립어린이집의 신설이 형평성을 달성하는 데 있어서 효과적이었는지 분석하였다. 이변량 국지모란지수(BiLISA)를 이용하여 2009년을 기준으로 보육 기회가 적고 영유아 인구가 많은 격자와 보육 기회가 적고 주택실거래가가 낮은 격자를 탐색하였다. 이들을 보육 필요 지역으로 정의하고 이들 격자들이 해당 시기에 평균적으로 얼마나 많은 보육 기회를 획득했는지 확인하였다. 그 결과 각 기준의 보육 필요 지역이 다른 유형들에 비해 평균적으로 더 많은 보육 기회를 획득하였기 때문에 이 시기에 보육 기회가 효과적으로 배분되었다고 평가하였다. 셋째, 그러나 앞서 살펴보았듯 주택실거래가를 바탕으로 한 보육 기회의 배분은 비형평하였기 때문에, 이러한 비형평을 극복함과 동시에 보육 수요를 효과적으로 충족시킬 수 있는 우선 설치지역을 탐색하여 제안하였다. 담당 공무원 인터뷰와 문헌 자료를 통해 확인한 결과 본 연구가 제안한 우선 설치지역에 국공립어린이집 부지 확보 문제와 민간부문 어린이집과의 관계와 같은 문제들이 있는 것으로 나타났다. 따라서 민간투자 유치, 부지 무상 확보, 민간 전환, 민간 매입과 같은 다양한 방법들이 모색되어야 하며, 적극적인 민간 전환 방식과 보육수급률의 계산 방식의 수정이 필요함을 제안하였다. 이러한 연구 결과는 공공보육서비스의 분포를 확인하고, 확인된 분포를 국공립어린이집 설치와 관련된 법을 토대로 설정한 두 가지 평가 기준과의 비교를 통해 평가하여 공간적 형평성을 평가하는 분석 틀을 제시하였다는 점과 단순히 우선 설치지역을 제안하는 것에 그치지 않고 설치와 관련된 문제들을 살펴봄으로써 향후 형평한 배분을 위한 정책적 근거를 제시하였다는 점에서 의의가 있다.As the public interest in childcare and demand for childcare centers have been increased, expanding national/public childcare centers is the utmost goal of urban government. The urban government should consider equitable distribution of national/public childcare centers. However, in a spatial perspective, earlier studies have focused on issues such as conditions of its location and service boundary delineation. Because of this reason, an analytical framework that evaluates spatial equity of public childcare services is needed. Therefore, the purpose of this study is to assess spatial equity of public childcare services and further to suggest priority regions for equitable distribution of public childcare services. The study is based on national/public childcare centers in Seoul in 2009 and 2014. The results of the study are summarized as follows. First, in spatial units based on grid, distribution of childcare opportunities measured by a potential model is uneven. Such uneven distribution is evaluated by using concentration curve and index. The distribution of childcare opportunities is equal compared to distribution of infants estimated by dasymetric mapping while it is unequal compared to distribution of housing market price, which means income. Nevertheless, from the concentration curves and index of 2009 and 2014, it can be said that distribution of childcare opportunities has been directed to be more equal. Thus, national/public childcare centers were effectively established between 2009 and 2014. Second, the study analyzes whether additional establishment of national/public childcare centers between 2009 and 2014 is effective enough to achieve the equal distribution of childcare opportunities. Two types of grids in 2009 are explored by using Bivariate Local Indicators of Spatial Association: one that has many infants and less childcare opportunities and the other one that has lower housing market price and less childcare opportunities. In other words, these can be defined as areas in need of childcare services and further verify how many childcare opportunities gain. As a result, the study shows childcare opportunities were well distributed between 2009 and 2014 as areas in need of childcare services obtained more opportunities than other types of grids. Third, the study suggests priority regions to establish national/public childcare centers. As distribution of childcare opportunities based on housing market price is unequal, the suggestion will help to overcome such unequal distribution and to satisfy demand for childcare services effectively. According to the interviews with public officials and literature materials, issues such as securing land for national/public childcare centers and relationship with private childcare centers are found in the priority regions this study suggested. To resolve the issues, the city government should seek many ways such as detaining private investment, securing land for free, converting private centers to national/public centers, and purchasing private centers. Therefore, the study recommends converting privates to national/public actively and revising calculation method of supply and demand rates of childcare services. As discussed above, the results of the study have significance because of two reasons. One is that the study proposes analytic framework for spatial equity of national/public childcare centers by assessing two appraisal standards which are based on law related to establishment of national/public childcare centers. (The) correlation between diet patterns, physical activity and physical fitness scores at a middle school in Seoul, Korea

역학 및 건강증진학과/석사[한글] 본 연구는 중학교 학생들의 체력 상태를 파악하고, 생활 형태, 생활 습관, 운동참여 정도 및 운동이나 체육에 대한 의식, 식습관 그리고 여가 형태와 체력과의 관련성을 알아보고자 하였다. 본 연구의 대상은 서울시 소재의 N중학교에 재학 중인 2, 3학년 학생 600명을 대상으로 하였으며, 생활 형태, 생활 습관, 운동참여 정도 및 운동이나 체육에 대한 의식, 식습관 그리고 여가 형태에 대해서는 구조화된 설문지를 이용하여 조사하였으며 체력 상태는 학생체력검사를 통하여 측정하였다. 총 연구대상자는 520명이었다. 주요 연구 결과는 다음과 같다. 대상학생들의 체력수준은 2학년이 18.75점, 3학년이 17.91점으로 모두 평균적인 3급(1～5급제도)에 해당하는 점수이며, 남학생은 18.20점, 여학생은 18.49점으로 역시 평균에 해당하는 3급에 해당하는 체력점수를 보였다. 학생특성에 따른 체력점수를 분석한 결과 학년별(2학년-18.75점, 3학년-17.91점), 비만도별(저체중-19.24점, 과체중-15.92점), 주관적 건강상태별(매우좋다-19.07점, 매우나쁘다-17.83점)로 통계적으로 유의한 차이를 보였다. 학생의 건강생활습관(운동)에 따른 체력점수를 분석한 결과, 개인의 신체활동정도(활동적-20.23점. 비활동적-16.65점), 체육시간 선호도(좋다-19.34점, 싫다-17.11점), 수업빈도 만족도(부족하다-19.20점, 충분하다-17.16점), 운동학원 경험(있다-18.93점, 없다-18.18점) 그리고 지난 한달간 운동경험(있다-19.28점, 없다-17.96점)이 통계적으로 유의한 차이를 보였다. 식습관에 따른 체력점수를 분석한 결과, 규칙적인 아침식사(규칙적-19.00점, 비규칙적-16.49점)와 간식의 회수(매일먹는다-19.94점, 비규칙적-17.71점)가 통계적으로 유의한 차이를 보였다. 통계적으로 유의한 차이를 보인 변수들을 사용하여 회귀분석을 실시하였을 때 성별, 학년, 비만도, 개인의 신체활동정도, 지난 한달간 운동경험, 규칙적인 아침식사, 간식의 회수 등의 변수들이 유의하였다. 이상의 결과, 학생들의 체력을 증진하기 위해서는 학교에서 많은 신체활동의 기회를 제공하고, 비만을 예방할 수 있는 교육을 실시하여야 할 것으로 생각된다. 또한 균형있는 식생활을 통하여 올바른 영양의 섭취를 할 수 있도록 지도하여야겠다. [영문]The purpose of this study was to examine the physical fitness of middle school students and to investigate the relation between their fitness and lifestyle, participation in exercising, opinion on physical education class, eating habits and recreation activities. The subjects for this study were 600 who were in the 8th and 9th grade at middle school in 2003 in Seoul. A structured questionnaire was used for measurement of lifestyle, participation in exercising, opinion on physical education class, eating habits and recreation activities. Physical fitness were measured Test of student''s Physical fitness. A total of 520 students were analyzed in this study. The results of this study are as follows : The average physical fitness score of the 8th grade was 18.75, the 9th grade 17.91, male students 18.20, and female students 18.49. All of these scores were regarded as the third level of physical fitness. When compared the fitness scores based on the student''s characteristics, there were significant differences in grades, obesity and subjective health status. After comparing the fitness scores according to the student''s exercise habits, significant differences appeared in the factors of the amount of physical activities, preference for p.e. class, satisfaction of the frequency of p.e. class, extracurricular physical activities and experience of physical activities during the last month. After comparing the fitness scores according to the student''s eating habits, significant differences appeared in the factors of regular breakfast and snacks. By regression analysis, gender, grade, obesity, amount of physical activities, experience of physical activities during the last month, regular breakfast and snacks were significantly related to the fitness scores. In conclusion, to promote student''s fitness there should be more opportunities for physical activities and educational programs on preventing obesity in school. Also guidance for good nutrition through regular eating habits is needed.ope

의과학 사업단/석사[한글]크립토코크스 수막염은 중추신경계를 침범하는 Cryptococcus neoformans에 의한 뇌수막의 염증 질환이다. 뚜렷한 치료 방법이 없었던 과거에는 대부분 환자가 사망하게 되는 매우 심각한 질환이었으나 amphotericin B (이하 AmB라고 함)가 분리되어 사용됨으로써 치료에 혁신을 가져왔다1. 또한 피리미딘 길항제로 진균 DNA 합성에 장애를 일으키는 flucytosine (이하 5- FC라고 함)을 병용하여 70%의 치료 성공률을 보여 현재 가장 널리 치료에 사용된다2. 그러나 이러한 병용 요법으로도 환자의 30%는 치료가 안되며, 신장 독성과 같은 각종 부작용과 6주 이상 정맥 주사를 해야 하는 등, 임상 치료 시 어려움이 있으며 경우에 따라서는 투여를 중지해야 하는 경우도 있다3-5. 1990년 대 초반부터 fluconazole (이하 FCZ라고 함)이 새로운 치료제로 사용이 점차 증가하는데, 이는 경구 복용이 가능하고, 환자가 잘 견딘다는 장점이 있다. 급성 감염 시 FCZ단독 요법만으로도 효과적이었다고 하며6, AmB와 FCZ를 비교했을 때 효과가 비슷하다는 보고와 AmB가 치료 및 사망률 면에서 더욱 우수하다는 보고 등 연구자마다 의견이 엇갈리고 있다7-9. 국내에서는 임상 양상과 예후에 대한 보고는 있으나, 두 약제의 치료 효과를 비교한 연구는 없다10-11. 크립토코크스 수막염의 치료 시 AmB와 5-FC의 병용 요법과 FCZ의 선택 중 약물 자체의 신장 독성이 있고 장기간 정맥 주사하는 약제보다는 부작용이 적으며 복용이 간편한 FCZ을 투여하는 것이 환자에게 부담을 적게 줄 것이다. 이 연구의 목적인 크립토코크스 수막염의 초기 치료제로 FCZ이 AmB 정도의 치료 효과가 있다는 것을 입증하기 위해 AmB 단독 또는 5-FC 병용 요법 군 (이하 AmB 군이라 함)과 FCZ 단독 요법 군 (이하 FCZ 군이라 함)의 치료 효과와 부작용을 비교하고, 두 군 간에 있어서 치료 실패 요인을 분석하였다.
