Usefulness of qualitative cardiac markers assay in screening and early diagnosis of acute myocardial infarction

의학과/석사[한글] Acute myocardial infarction is a typical emergent disease in emergency department, but until present, there is a no single tool definitively diagnostic. Point of care tests (POCT) were already introduced in most emergency departments because of it''s use of whole blood, rapid turn around time (TAT) of blood sampling to result, and technical convenience. So, this study was designed to investigate the usefulness of qualitative myoglobin creatine kinase-MB (CK-MB) Troponin I (TnI) assay in screening and early diagnosis of acute myocardial infarction. Material & Method: Cardiac STATusTM (Spectral Diagnostics, Toronto, Canada) as one of qualitative myoglobin CK-MB TnI assay kits was used in this study. In 87 patients, Cardiac STATusTM and central laboratory serum tests for CK-MB and troponin T (TnT) were performed. The sensitivity and the specificity for acute myocardial infarction were evaluated. Based on patient''s record, turn around time (TAT) and kappa statistic for correspondence between the results of Cardiac STATusTM and central laboratory serum tests was evaluated. Result: The sensitivity and specificity of Cardiac STATusTM for acute myocardial infarction was 61.7% and 90%, respectively. There was not different in results between Cardiac STATusTM and serum tests. Kappa statistic of 0.650 showed statistically meaningful correspondence of results between Cardiac STATusTM and serum tests. TAT of Cardiac STATusTM was average 15 minutes, but TAT of serum tests was average 67.3 ¡3/4 20.6 minutes(range: 20～126minutes). Conclusion: Cardiac STATusTM was not superior diagnostic tool than central laboratory serum tests in diagnosis of acute myocardial infarction. However, the result of Cardiac STATusTM was meaningful correspondent with that of serum tests and may help medical team to make fast decision and establish early treatment guideline. [영문] 급성 심근경색은 응급실에서 접할 수 있는 대표적 응급질환이지만 현재까지 조기 진단에 유용한 단일검사는 없는 실정이다. 현장검사는 전혈을 사용할 수 있으며, 검사 및 결과에 소요되는 시간이 짧고, 기술적으로 쉽다는 장점이 있어 이미 많은 응급실에서 도입 되어 사용하고 있다. 본 연구는 현장검사인 myoglobin, CK-MB, Troponin I의 정성분석 기구가 급성 심근경색의 선별 및 조기진단과 조기 치료에 도움을 주는지를 알아보고자 하였다. 재료 및 방법: Myoglobin, CK-MB, Troponin I 정성분석 기구로 Cardiac STATusTM (Spectral Diagnostics, Toronto, Canada)를 사용하였다. 총 87명의 환자를 대상으로 Cardiac STATusTM와 혈청 CK-MB, TnT 검사 및 심전도 검사를 시행하였다. Cardiac STATusTM의 진단 정확도를 알아보기 위해 민감도와 특이도를 조사하였고, 혈청검사와의 결과 일치 정도는 카파 값(kappa statistic)을 이용하였다. 환자 기록지를 바탕으로 혈청검사에 소요되는 시간을 조사하였다. 결과: 급성 심근경색에 대한 Cardiac STATusTM는 민감도 61.7%, 특이도 90%였으며, 혈청검사와 큰 차이를 보이지 않았다. Cardiac STATusTM와 혈청검사의 결과는 카파 값 0.650으로 유의한 일치성을 보였다. 또한, Cardiac STATusTM의 결과를 얻기까지 평균 15분 소요되는 것에 비해 혈청 검사는 평균 67.3 ± 20.6분(20～126분)이 소요되었다. 결론: Cardiac STATusTM는 급성 심근경색의 진단에 있어 민감도와 특이도가 기존의 혈청검사에 비해 우월하지는 않았으나, 기존의 혈청검사와 유의한 일치성이 있고, 혈청검사에 비해 빠른 결과 확인으로 의료진의 빠른 의사결정과 조기 치료지침 설정에 도움을 줄 수 있을 것으로 판단된다.ope
